Kawasaki disease, an acute febrile illness which primarily affects in children under the age of six, was first described by Tomisaku Kawasaki in 1967. It has been reported that Kawasaki disease is probable driven by abnormalities of the immune system after an infectious insult, but this has not been confirmed. It mainly affects small and medium-sized arteries, particularly the coronary arteries. Deaths may occur at any time with cardiovascular complications. The early recognition and treatment with follow-up evaluation for the coronary arterial lesion is very important in a case of Kawasaki disease.

Since Dr. Angell introduced cryopreservation method for tissue preservation in 1980s, it became a preferred method for cardiovascular tissues. Cryopreservation has been proven to provide better cell viavility and clinical result than any other preservation method. The quality of cryopreserved cardiovascular tissue is a very critical point of patients' survival when transplanted. To obtain high quality cryopreserved tissue, each processing methods from harvesting to delivery should be standardized. Various factors, such as range of possible tissues, acceptable ischemic time, temperature during transportation, facility for processing, method of freezing, use of cryoprotectant, method of storage, condition of delivery, and method of thawing for cardiovascular tissues should be standardized. Different standards should be established and applied to different tissues. Various kinds of tissues can be harvested from each living or cadaveric donor. Therefore, it is reasonable to harvest and handle all tissues in one laboratory by one tissue bank. In U.S.A, tissue banks are operated under standardized condition. The American Association of Tissue Banks affords the certification of specialist and tissue banks on very strict conditions, and provides the Food and Drug Administration with consultation in making regulations regarding tissue transplantation. In Korea, tissues have been simply stored in refrigerator since mid 1990s, and cryopreservation method is getting popular. Tissue banking has been started recently and now six major hospitals treat the cardiovascular tissues and are equipped with good facilities. Korean Association of Tissue Banks was established last year and now setting up regulations and standards. In Conclusion, I suggest that the cardiovascular tissue bank should belong to a university hospital till regulations and standards are completed. And then commercial tissue bank will be able to take it over to manage more tissues. The financial state of tissue banking is also an important point to be considered to balance between quality control and easy access to the public. Also the cost of cryopreserved allograft should be covered by medical insurance.

The patient was a sixty-year-old obese woman with the long history of hypertension, amenorrhea and occasional psychic disturbances. She had eigbt, thumb to adult-fist sized, slightly movable, relatively soft, and painful subcutaneous nodules with tenderness and paresthcsia, of which overlying skin appeared to be normal except senile changes, on the abdomen and both upper extremities for 10 years. There was no evidence of inheritance in her family. Laboratory data revealed no abnormalities in lipid metabolism or in a variety of endocrinological functions. Microscopically, an excised mass from the forearm showed thin connective tissue capsule encircling numerous lobules cornposed entirely of the mature fat. cells vith minimal focal capillary proliferatios.

Helicobacter pylori infection has been shown to be associated with gastric carcinoma. However, despite the frequent detection of seropositivity for H. pylori and histologic detection in biopsy specimen, histologic detection rate of H. pylori in surgical specimens has been low. In this study, we investigated the prevalence of H. pylori infection in gastrectomy specimens bearing gastric adenocarcinoma and compared it with both endoscopic biopsy and serologic results. H. pylori infection was identified by Giemsa stain in the mucosa stripped from the tumor, body, and antrum in 61 gastrectomy specimens. We evaluated the effect of H. pylori infection on gastric mucosal cell proliferation by using monoclonal antibody for Ki-67. H. pylori detection rate using Giemsa stain was higher in gastrectomy specimens (67.3%) compared to that (48.1%) of biopsy specimens (p=0.006). The detection rate was higher in body than that of antrum or tumor site in the same patients (p=0.001). The H. pylori seropositivity was 60.5% and relatively nonspecific. The mean value of Ki-67 labeling index in the H. pylori-positive group was higher than that in the H. pylori-negative group (p<0.05). The increase in gastric epithelial cell proliferation was not influenced by the location of the tumor or the site of the specimen. The results suggest that the actual prevalence of H. pylori infection in patients with gastric carcinoma is considerably higher than that evaluated on endoscopic biopsy specimens. In addition, the increased cell proliferation in the H. pylori-positive group suggests some evidence that H. pylori may be involved in gastric carcinogenesis.

During the period of 9.5 years from September, 1985 to March, 1995, 32 cases with spontaneous closure in simple ventricular septal defect(VSD) were observed and evaluated at Yeungnam University Hospital, and those were confirmed by 2D-echocardiogram. The results were as follows: 1. Among these 32 cases, there were 23 cases(71.9%) of perimembranous VSD, 8 cases(25.0%) of muscular VSD and 1 case(3.1%) of subarterial VSD. Septal aneurysms appears to be main mechanism of spontaneous closure of VSD because of the presence of septal aneurysm in all cases with spontaneous ',;losure of a perimembranous VSD. 2. The size of the defect was variable in diameter, but 27 cases(81.2%) were less than 5 mm. 3. The mean age was 12.1 months at spontaneous closure with the range from 1 month old to 72 months. 4. The mean weight was 9.0 kg at spontaneous closure. 5.--Among these 32 cases, 3 cases had the clinical evidence of cardimegaly or congestive hert -failure during infancy. 6. Gout is not very rare disorder in high economic countries and it is seldom observed in our country and is hard to find its report in Korean dermatologic literature. Gout is a disease of inbom metabolic and/or acquired form of the disease chara-cterized by hyperuricemia, tophaceous deposits of sodium urate and recurrent attacks of arthrit.is which dramatically responds to Colchine. A case of tophaceous gout in a 56 year old Korean male is reported. The patient showed pea to waInut size, yellowish tender subcutaneous masses on right great toe and both solee of 15 year duration which are adherented to the overIying skin. The hiatology of the lesion revealed variously sized, sharply demarcated aggregates of needle shaped urate crystals in bund.his and sheaves which are surrounded by fibrous connective tissue which are in tum surrounded by granulomatous infiltrate containing small round cells and foreign body glant cells. The urate crystals have a brownish color and are doubly refractile on polariscopic examination. The use of colchine, in a initial dose of 1mg foIIowed by 0. 5mg every hour for 6 hours. The patient failed to attend the clinic regularly and was lost for folIow up.

The effect of retinoic acid. on hyperkeratotic diseases and. Acne as being published. To confirm the effect of topical retinoic acid, vre subjected 50 patients with mild to severe Acne vulgaris of the face, The patients chosen were not previously treated. with any remedy at least for the past 2 weeks. Eudyna' cream or gel were applied. once or twice a day to the face keeping some distance from the mucous membrane. We checked the number of closed comedones, open comedones, papules, pustules and scara before the treatment and every week for 14 wks during the treatment, The result were as follows: l. 42 patients began to improve in 2 to 8 wks and markedly improved in 6 to 8 wks. 3 patients dropped out because of severe side effects. Very good: 13 patients(26%) Good: 14 patinets(28%) Satisfactory: 15 patients(80%) None: 5 patients(10%) 2. The side effects: Most of patients suffered from mild to severe erythema, scaling and buming sensation in 2 to 3 days after the initiation of application. In severe cases developed severe erythema, vesicles, pustules, edema and exudation within a week.
