2.Treatment of the Sequelae of Septic Hip in children: Report of 2 Cases

Septic arthritis of the children's hip causes acute or chronic inflammatory change in structure about the joint, and results in serious destruction. Due to recent advance of the diagnostic technique and of the antibiotics, the incidence of it's sequelae is decreased. The management of acute stage with antibiotics and early surgical drainage has been well established in the literature, but the management of residual anatomic deformity is less documented, and little comprehensive classification or treatment program has been outlined. We experienced two patients who had sequelae induced by delayed diagnosis and improper treatment. One had a complete loss of femoral neck with femoral head remaining in the acetabulum. Femoral neck reconstruction was performed using proximal femur : i.e., Open reduction of femoral head to femur shaft with distal transfer of greater trochanter (1st stage) followed by varus osteotomy(2nd stage) and follow-up period was 3.5 years. Another had loss of femoral head and neck, dislocation of the hip, and Larsen's trochanteric arthroplasty was performed with 6 Years follow-up. Satisfactory results were obtained in both patients who had reconstructive surgery, so we report these cases with related literatures.

3.Incidence and clinical characteristics of thyroid cancer in Korea.

The incidence of thyroid cancer is steadily increasing in many countries including Korea. Especially in Korea, the incidence of thyroid cancer is very rapidly increasing during several decades; from 2005 Annual Report of the Korea Center Cancer Registry, thyroid cancer ranked the fifth among all cancers (incidence rate was 8.9%). Moreover the incidence of thyroid cancer was 16.7% in female and now thyroid cancer was reported the most common cancer in Korean female. Among several thyroid cancers, papillary thyroid cancer (PTC) is rapidly increasing due to the increased detection of papillary thyroid microcarcinoma (PMC). Until now, PMC showed little significant differences among clinical characteristics or prognosis compared with PTC, thus the optimal treatment strategy of PMC remains controversial. Therefore, to establish the optimal treatment and follow up program about thyroid cancer, we should investigate clinical characteristics, prognosis and predictive factors of PMC more closely.

5.A Clinical Study on Urinary Tract Infection in Infants and Children.

This is a report of clinical study on the 63 patients of symptomatic urinary tract infection who were hospitalized at the Department of Pediatrics in Kangnam General Hospital during the period between the May of 1989 and the September of 1992. The observation results were as follows: 1) The frequency of urinary tract infection (UTI) was the highest in children under the age of one year (66.7%). 35 patients were male, while the other 28 were female. The ratio of male and female patients was 1.25:1. 2) Fever was observed in most of the cases. Systemic nonspecific manifestation was predominant in the infancy and early childhood, whereas local symptom of UTI was predominant in the late childhood. Among the 35 male patients, 32 cases were phimoses. As for the other 3 cases, Phimoses were not identified. 3) E. coli had the most frequency (65.3%), followed by Klebsiella, Enterobacter, Pseudomonas in descending order. 4) In the vitro antimicrobial susceptibility test, Gram negative organisms such as E. coli Klebsiella, Enterobacter were sensitive to cefotaxime, amikacin, and ampicillin/sulbactam. Meanwhile, Gram positive organisms such as Staphylococcus, Enterococcus were sensitive to vancomycin and cephradine. 5) The renal ultrasonography test was performed for 59 patients, 13 cases of whom showed abnormal findings. 6 cases out of the 13 abnormal patients had hydronephrosis. 6) DMSA renal scan was performed for 40 patients within 2 months of an acute pyelonephritic attack. Signs of pyelonephritic change were found in 18 patients. DMSA scan was repeated 4~12 months later in 4 of these patients. This showed renal cortical scarring in all patients. 7) Voiding cystourethrographic findings in 38 patients showed vesicoureteral reflux in 13 patients. 8) We performed urine culture again after 48 hours from the beginning of therapy and 85.5% of the cases became sterile. 9) The recurrent percentage of UTI was 23.8% with the ratio of 1:1.5 between male and female. Most of the patients were clildren under the age of one year. 10) Operations were made on two cases showing the increase of reflux during the follow-up of unilateral Grade ll and Grade 3 reflux, respectively. Operations were also performed on other two cases with bilateral Grade IV reflux. All the cases were good after the operations. In the meantime, as for another two cases having the right reflux of Grade lland the bilateral reflux of Grade lll, the refluxes were able to be reduced by prophylaxis only, in the course of following up.

6.CT and MR Findings of Persistent Hyperplastic Primary Vitreous(PH PV).

PURPOSE: The purpose of this paper is to discuss the characteristic CT and MR findings in persistent hyperplastic primary vitreous(PHPV) and to compare the detectability of those findings in each modality. MATERIALS AND METHODS: We retrospectively evaluated CT and MR findings in 32 patients with PHPV. Twenty-five patients had CT, 13 patients had MR, and 6 patients had both CT and MR. RESULTS: Major findings of PHPV in 32 patients on both imaging modalities were lens deformity(78%), shallow anterior chamber(72%), heterogeneous vitreous opacity(72%), enhancing hyaloid artery or remnant of fibrotic hand(69%), and microophthalmos(67%). Minor findings were retinal detachment(22%), and vitreous hemorrhage(6%). In MRI, lens deformity(92%) and shallow anterior chamber(85%) were detected most commonly whereas in CT, opaque vitreous(80%) was the most common finding. Findings of enhancing hyaloid vessel or remnant of fibrotic band, considered characteristic of PHPV, were more commonly detectable in MR (85%) than CT(52%). CONCLUSION: Characteristic MR and CT findings of PHPV were lena deformity, shallow anterior chanber, heterogeneons vitreons opacity, enhanciny hgalind artery or remnant fibrotic band, and microphthalmos. MR seemed to be more useful than CT in detecting Globe pathology.

7.A case of cyclic neutropenia.

Cyclic Neutropenia is a benign, unusual hematologic disorder characterized by regularly recurring episodes of severe neutropenia occurring approximately every 21 days. Beside neutrophils, monocytes, lymphocytes, platelets and reticulocytes all cycle with strict periodicity suggest that this disease should be viewed as cyclic hematopoiesis, not merely as cyclic neutropenia. During neutropenic periods, patients regularly experience aphthous stomatitis, fever, malaise, cervical lymphadenitis, cutaneous infections and occasional pneumonia and otitis media. The exact cause of cyclic neutropenia is unknown. But it is strongly suggested that cyclic neurtopenia is due to an abnormality in the regulation of early hematopoietic precursor cells. We have documented a case of cyclic neutropenia for a period of 2.5 years in a 11 year-old boy who had suffered from recurrent fever, stomatitis, gingival swelling, cervical lymphadenitis and skin infections at 3 weeks intervals since 5 years of age. A brief review of the related literatures is presented.

9.Immunohistochemical Distribution of Heme Oxygenase Isoenzymes in Developing Rat Retina.

PURPOSE: To analyze and compare the immunohistochemical distributions of HO-1 and HO-2 with the developmental process of retina, focusing on the time of birth and opening eyes because it is the time of the exposure to hyperoxia and light exposure. METHODS: The retina of 16-day-old embryo, 18-day-old embryo, 20-day-old embryo and postnatal rats at 0 day, 5 days, 10 days, 15 days, 20 days, 1 month, 2 months, 3 months were examined by hematoxylineeosin staining and immunohistochemistry of HO-1 and HO-2. RESULTS: Similar distribution of HO-1 and HO-2 from embryo to mature retina were observed. It was observed that HO-1 and HO-2 was contained in ganglion cell layer, inner plexiform layer, inner nuclear layer, outer plexiform layer and pigment layer of the mature retina. In the developmental process, reactivity was observed in the cell body of ganglion cells after 0 day of its birth. The reactivity was increased after 5 days of its birth, staining was observed in nerve cell and nerve fiber of ganglion cell layer, inner plexiform layer, inner segment of neuroblastic layer, pigment layer at this time. Reactivity of ganglion cell layer and pigment layer was increased intensively after 15 days of its birth and weak reactivity in some of horizontal cells and amacrine cells in inner plexiform layer was observed. The same distribution as that of mature retina was observed after 20 days of its birth. CONCLUSIONS: HO-1 and HO-2 expression was temporally and spatially correlated with the normal development of retina in rats. HO was concerned with the structure and function of retinal development by the activation of its products and with anti-oxidant mechanism on the time of birth and opening eyes.

10.Epidemiologic Association between Obesity and Thyroid Nodules.

Recently, the incidence of thyroid nodule has been rising due to the usage of ultrasonography in health check-up. Also, modern society is faced with increasing rates of obesity by sedentary life style and high caloric intake. Obesity is closely related to metabolic syndrome and insulin resistance. Insulin resistance leads to hyperinsulinemia, increased insulin-like growth factor 1 and alteration of adipocytokine level. These factors not only have a role in metabolic regulation, but they also have mitogenic effect in cell proliferation. Epidemiologic and clinical evidence about the association between obesity and thyroid nodule are reviewed in this section.
