1.A Case of Leiomysarcoma.

This paper presents a case of leiomyosarcoma which developed in the retroperitoneal cavity. On admission, this 4 months old male infant disclosed a adult fist sized mass with abdominal distention and scattered nevus lesions on the left sided abdomen on physical examination. IVU examination revealed urinary collecting system to be unremarkable, but right kidney and ureter were displaced to upward and posterior respectively. On 6th hospital day, we performed explorative laparotomy and removed the tumor mass measured 15X11cm in size. Histologic finding revealed leiomyosarcoma. Review of literature was also made breifly.

2.Nutritional Status of Long-term CAPD Patients-Impact of Peritoneal Transport Characteristics.

Protein-calorie malnutrition is common in CAPD patients and is associated with increase in morbidity and mortality in CAPD patients. There are many causes of malnutrition in CAPD patients, and it is well known that a large amount of protein losses through peritoneal membrane is one of them. To investigate the effect of the peritoneal membrane transport characteristics on the nutritional status in long-term CAPD patients, we conducted a cross- sectional study on clinically stable 115 patients (63 males and 52 females) who have been on CAPD for more than 2 years, and assessed nutritional status by subjective global assessment (SGA), biochemical, anthropometric and urea kinetic parameters. Patients were divided into 4 groups according to the results of standard peritoneal equilibration test (PET). The results were as follows: 1) The patients were divided into 4 groups according to the PET results: high transporter (n=16, 14%), high average transporter (n=38, 33%), low average transporter (n=50, 43%), and low transporter (n=11, 10%). 2) The mean age of the patients was 50.1+/-11.6 years (range, 19-75) with sex ratio (M:F) 1.2:1 and mean duration of dialysis was 57.5+/-27.8 months (range, 24-135). The mean body weight (Bwt) was 59.2+/-8.9kg, percent ideal body weight (%IBW) was 104.7+/-15.6%, %lean body mass (LBM)/Bwt was 82.1+/-11.1%, and malnourished patients by subjective global assessment (SGA) were 32.2% (38/115). 3) The mean BUN, creatinine, total protein, and albumin level of the patients were 54.5+/-13.1mg/dL, 12.3+/-3.0mg/dL, 6.7+/-0.8g/dL, and 3.8+/-0.6g/dL, respectively. 4) There were no significant differences in age, sex ratio, CAPD duration, peritonitis rate, %LBM/ Bwt, and SGA among the 4 groups. 5) Total protein (g/dL) and albumin (g/dL) levels in high transporters were 6.4+/-0.5, 3.5+/-0.4, respectively, and they were significantly lower than those of low transporters (7.2+/-0.6, 4.2+/-0.5) (P<0.05). 6) 24 hour dialysate protein (g/day) and albumin (g/day) losses were significantly higher in high transporters (8.10+/-1.85, 4.19+/-1.23) compared to those of low transporters (5.07+/-1.85, 2.78+/-0.99) (P<0.05). 7) The level of IGF-1 (ng/mL) was significantly lower in high transporters (150.5+/-86.2) compared to that of low transporters (310.3+/-162.1) (P<0.05). 8) The level of BUN (mg/dL), Cr (mg/dL) were also lower in high transporters (45.4+/-13.1, 10.1+/-2.0) than those of low transporters (61.6+/-18, 14.7+/-2.7), but there were no significant differences in Hct, total cholesterol, prealbumin, and transferrin among the 4 groups. 9) There were no statistically significant differences in Kt/Vurea, RRF, NPCR, dietary calorie and protein intakes among the 4 groups. 10) Anthrometric parameters such as TSF, BSF, MAC and LBM measured by three different methods (LBMCr, LBMimp, LBManthro) didn't show any significant differences among the 4 groups. In conclusion, increased peritoneal permeability may not adversely affect SGA and anthropometric status of long-term CAPD patients, although it is associated with lower serum albumin, creatinine, and IGF-1 level. Clinical significance of these findings remains to be elucidated.

3.A case of acute eosinophilic pneumonia.

Acute eosinophilic pneumonia(AEP) was first described in 1989 and represents a clinical entity distinct from chronic eosinophilic pneumonia. AEP is characterized by acute respiratory insufficiency, hypoxemia, fever, diffuse radiographic infiltrates, and eosinophilia in bronchoalveolar lavage fluid or lung biopsies in the absence of infection, atopy, or asthma. Rapid response to corticosteroids is characteristic. We experienced a 47-year-old metal driller presenting typical clinical and radiological characteristics of AEP. We confirmed eosinophilic pneumonia with brochoalveolar lavage analysis and transbronchial lung biopsy. We report a case of AEP diagnosed by clinical, radiographic, and histologic findings with a brief review of the literature.

4.Prognostic Factors in Spontaneous Primary Intraventricular Hemorrhage.

OBJECTIVE: TA retrospective study is performed on 28 patients with primary intraventricular hemorrhage(PIVH) to examine the outcome and prognostic factors associated with this disorder. METHODS: Clinical data collected between 1998 and 2002 was used in the present study. The outcomes of these patients were compared by age, etiology, initial Glasgow Coma Score(GCS), Graeb's score, ventriculocranial ratio(VCR), hemorrhagic dilation of the third ventricle, and hemorrhagic dilation of the fourth ventricle. The Glasgow Outcome Scale(GOS) at discharge was used for the comparison of outcomes. RESULTS: The mean age of these patients was 48.4+/-17.4 years. The underlying causes of PIVH were hypertension(53.6%), moyamoya disease(17.9%), arteriovenous malformation(10.7%), cerebral aneurysm(7.1%), and unknown(10.7%). The age and etiology were not correlated with outcome. Patients with a GCS of 13-15 showed a good outcome in 90.5%(p<0.05). Those with a higher VCR(> or =0.23) showed a poor outcome in 64.3%(p<0.05). Patients with a Graeb's score of 9-12 showed a poor outcome in 87.5%(p<0.05). Those with hemorrhagic dilatation of the third ventricle showed a poor outcome in 80.0%, and those with hemorrhagic dilatation of the fourth ventricle showed a poor outcome in 85.7%(p<0.05). The overall mortality rate was 17.9%. CONCLUSION: Low initial GCS, high Graeb's score, high VCR, and hemorrhagic dilatation of the third ventricle or the fourth ventricle are correlated with poor outcome in PIVH.

5.Bcl-2 and Bax Expression and Ki-67 Proliferative Index in Astrocytic Tumors: in Relation to Prognosis.

OBJECTIVE: We report a retrospective investigation of the prognostic value of bcl-2 and bax expression, and Ki-67 proliferative index in 42 astrocytic tumors. METHODS: We classified the astrocytic tumors and reviewed the clinical information and survival time. The sections were taken from surgically resected paraffin-embedded tissue and performed immunohistochemical stains for bcl-2, bax and Ki-67. RESULTS: The immunohistochemical stain for bcl-2 revealed a positivity in only two(4.76%) among forty-two cases. The immunostain for bax was positive in 35 cases(83.3%). However, the correlation between bcl-2 & bax expression and age, sex, tumor location, size, and histologic grade was not found. By Kaplan-Meier analysis, bcl-2 & bax expression and survival time in astrocytic tumors was no significance in log rank test(p>0.05). There were prognostic values between Ki-67 LI and histologic grade and between Ki-67 LI and survival time, respectively(p<0.05). CONCLUSION: Bcl-2 and bax are not significant, whereas Ki-67 LI is suggested as a significant prognostic factor, associated with histologic grade and survival time of astrocytic tumors.

6.Resolution of Third Cranial Nerve Palsy Following Endovascular Treatment of the Posterior Communicating Artery Aneurysm.

Third cranial nerve palsy may indicate the presence of an intracranial aneurysm, most commonly in the posterior communicating artery. The effect of endovascular management of posterior communicating artery aneurysms on recovery from third cranial nerve palsy is not well documented. We report two patients with third cranial nerve palsy caused by posterior communicating artery aneurysms. Endovascular treatment resulted in subsiding of ptosis within two weeks. Resolution of extraocular muscle function occurred within one to three months after treatment. Endovascular treatment, which has recently been introduced into neurosurgical practice, is effective in preventing aneurysmal rupture, and in eliminating manifestations due to mass effects.

8.A Case of Right Atrial Myxoma.

Primary tumors of the heart are uncommon lesions that can mimic almost any type of cardiac disease. Over 90 per cent of myxoma which are the most common cardiac tumor occur in the atria, with 3 to 4 times as many occuring on the left as on the right. The development of nonivasive echocardiographic technique has made it possible to screen large numbers of patients with atrial myxoma with ease and safe. A 28 year old housewife was admitted because of dyspnea for 5 months. Clinical diagnosis of right atrial myxoma was made by echocardiography and right atrial angiocardiogram. Surgical resection was performed successfully. We presented a case of right atrial myxoma with a review of literatures.

9.Comparison of Surgical Treatment with Percutaneous Catheter Drainage of a Pancreatic Pseudocyst.

A pancreatic pseudocyst is a relatively rare complication of pancreatitis. Various factors, such as inflammatory process or trauma, are among the possible cause of pancreatitis. A pancreatic pseudocyst is often defined as a localized collection of pancreatic juice in the retroperitoneal area surrounded by a fibrous membrane devoid of an epithelial lining. The hospital records of 52 cases of pancreatic pseudocyst treated at Kyungpook National University Hospital from 1976 through 1994 were reviewed and analysed. The results are as follows : 1) The age distribution of these cases showed that 57.7% occurred in the 3rd and the 4th decades. The male to female ratio was 3.7 : 1 2) The etiology of the pseudocysts was pancreatitis in 46.2% of the cases and trauma, unknown etiology, pancreatic cancer and hyperlipidemia, in that order, in the other cases. 3) Abdominal pain was the most common symptom (86.4%); a palpable mass (65.4%), abdominal tenderness (65.3%) also occurred. 4) Abdominal C.T. and Ultrasonogram has a diagnostic accuracy as 100%, and U.G.I. showed a diagnostic accuracy of 76.5%. 5) Among the 52 cases, 30 cases received surgical treatment 13 cases were treated by percutaneous catheter drainage. 6) With respect to the operative procedure, internal drainage was performed in 18 cases (60%), external drainage in 8 cases (26.7%), excision in 3 cases (10.0%), and O & C in 1 case (3.3%). 7) Thirteen percutaneous catheter drainage procedures were performed, and the success rate was 92.3%.

Churg-Strauss Syndrome is a disorder of hypereosinophilia and systemic vasculitis in subjects with asthma and allergic rhinitis. Clinically, a multiple organ system can be involved with various manifestations of disease of lung, heart, skin, musculoskeletal system, nervous system, gastrointestinal and hepatobiliary tract. We experienced a case of Churg- Strauss syndrome presenting as the appendicitis and the lower gastrointestinal bleeding in a 37-year-old male patient with acute lower abdominal pain. He also showed peripheral eosinophilia, bronchial asthma, and mononeuritis multiplex. He initially received a high dose corticosteroid and was maintained with low doses of corticosteroid, cyclophosphomide and exchange plasmapheresis.
