1.Immunohistochemical Characterization of the Salivary Gland Tumors.

It has been clarified that myoepithelial cells contain S-100 protein which is known to be a marker protein of neural tissue. To evaluate the participation of myoepithelial cells in the histogenesis of the salivary gland tumors, normal salivary glands and various salivary gland tumors were stained by immuno-peroxidase method. PAP kits (DAKO Co, USA) for the S-100 protein and the Cytokeratin were used and the following resulting were obtained. Acinic cells of the normal salivery gland were negative for both cytokeratin and S-100 protein. The intercalated duct cells were weakly positive for cytokeratin and S-100 protein. The normal myoepithelial cells scattered around the acini and the intercalated ducts were positive only S-100 protein. In contrast, the striated duct were positive only for cytokeratin. In plemorphic adenoma, the S-100 protein positive cells were found in solid sheets of tumor cells, in chondromyxoid areas and in areas of spindle-cell stroma as well as in the outer layer of the tubular structures. Only the inner lining of the tubules were positive for cytokeratin. In basal cell adenoma, the stromal spindle cells were strongly positive for S-100 protein and the epithelial cells weakly positive. When tubules were present within the epithelial sheets, the inner most lining cells were positive for cytokeratin. The peripheral palisaded tumor cells were negative for both substances. By immunostaining of the adenoid cystic carcinoma, S-100 protein containing cells were found focally scattered independently on the variety of histologies. The lining cells of true cystic structure were positive for cytokeratin. Immunostaining of the mucoepidermoid carcinoma demostrated that the squamous cells and the tubular epithelial cells contained cytokeraitn, whereas only a few intermediate cells were positive for S-100 protein. In Warthin's tumor there were no S-100 protein positive cells, although basally located epithelial cells of the papillae were positive for cytokeratin. These findings suggest that salivary gland tumors other than the Warthin's tumor arise from myoepithelial cells or reserve cells having dual potentiality differentating into myoepithelial and intercalcated duct cells.
2.Surgical Treatment for the Shoulder Joint in Rheumatoid Patients.

Rheumatoid arthritis (RA) is a systemic disease with medication as the treatment of choice. However, surgical treatment is recommended when no improvement is noted despite aggressive conservative treatment. Synovectomy provides desirable outcomes for RA patients in the early stage with a glenohumeral joint of Larsen grade II or less; conversely, arthroplasty is recommended for patients with a glenohumeral joint of grade III or higher. RA patients often have attenuation and dysfunction of the rotator cuff, and reverse shoulder arthroplasty has been proven to provide favorable outcomes in some patients. RA is often complicated with osteoporosis and bony deformity; therefore, close attention is necessary to prevent fractures during shoulder arthroplasty.

4.Bile duct necrosis:Complication of transcatheter hepatic arterial embolization.

Bile duct necrosis and liver abscess are rare complications after transcatheter hepatic arterial embolization (THAE) of hepatocellular carcinoma. Authors report bile duct necrosis and liver abscess occurred in 2 cases as a complication of THAE of hepatocellular carcinoma. In these two patients, lipiodol emulsion mixed with adriamycin and mitomycin was used more than three times as chemoembolic materials.

6.Endoscopic retrograde cholangiographic findings in choledocholithiasis

Although ultrasonography replaced many invasive studies in biliary tract diseases, direct cholangiography does still play an important role in the diagnosis and management of choledocholithiasis. Endoscopoic retrograde cholangiography (ERC) is regarded as the best method in evaluation of exact extent of the disease and its frequent complication, cholangitis. Authors analysed 56 cases of choledocolithiasis diagnosed by ERC and compared these with ERC in 18 cases of normal, 22 cases of cholecystitis, 15 cases of clonorchiasis and 9 cases of parenchymal diseases of liver. The results are as follows; 1. ERC findings of choledocholithiasis are filling defects by stoneor stones, dilation of common hepatic as well as common bile ducts and findings of cholangitis. 2. ERC findings of cholangitis are dilatation of larger intrahepatic biliary radicles and acute peripheral tapering, decrease of arborization, increased or right angle branching pattern, straightening and rigidity as well as irregular narrowing of intrahepatic biliary trees. This findings are observed in majority of choledocholithiasis. 3. Over9mm in diameter at intraprancreatic portion of common bile duct was regarded as abnormal, with 95% sensitivity,85% specificity and 91% diagnostic accuracy by decision matrix analysis. 4. In the presence of dilatation of CBD and findings of cholangitis in ERC, one should consider choledocholithiasis in spite of absence of stone defect.

The purpose of this study is to investigate the effect of the ball-cone contact interface variation on the ceramic ball stress which is the dominant factor of the design of the moldular artificial hip joint. A contact stress analysis problem is formulated using the optimization principle and finite element analysis technique on the ball-cone system of three dimensional axi-symmetrical model, Numerical procedure is employed to get the solution for varying angles of cones and angular mismatches between the ball and cone interfaces. There is a fair amount of friction in the interface and included in the analysis model.

8.Superselective Embolization of Renal Angiomyolipoma.

PURPOSE: Authors retrospectively evaluated the efficacy of superselective renal arterial emboliation in 5 patients with symptomatic renal angiomyolipoma. MATERIALS AND METHODS: Five Patients with renal angiomyolipoma who had been treated with superselective renal arterial embolization were analysed retrospectively. Two patients who had multiple bilateral lesions were associated with tuberous sclerosis. The mbolic materials used were absolute alcohol in 6 sessions, absolute alcohol mixed with lipiodol in one session, and polyvinyl alcohol in one session. RESULTS: We could observe devascularization of the tumor on the post-embolization arteriograms. The symptoms such as flank pain were improved in all patients during the follow-up period of 3 months to 3years. CONCLUSION: Superselective renal arterial embolization is an effective and safe procedure in the management of renal angiomyolipomas.

9.Adenocarcinoma in the duodenal bulb and proximal descending duodenum: UGI, US and CT findings.

The findings of upper gastrointestinal series(UGI), computed tomography (CT), and sonography(US) of ten duodenal adenocarcinoma confirmed by surgery (n=7) and endoscopic biopsy (n=3) were retrospectively analyzed. We performed US in all cases, CT and UGI in 8 out of 10 cases. UGI showed 4 cases of ulcerating type, 3 of stenotic type, and one failed to visualize duodenum due to previous gastrojejunostomy. UGI was more accurate in depicting the mucosal changes such as ulcer, however, it played a limited role in the evaluation of extraluminal extension. US showed 4 cases of hypocchoic wall thickening, 3 of hypoechoic extraluminal mass, and one of polypoid intraluminal mass but 2 were not detected on US. US accurately detected hepatic and pancreatic metastasis, however, it played a limited role in the evaluation of direct colonic invasion. CT showed was eccentric wall thickening in 4 cases, large extraluminal mass in 2, and low density intraluminal mass in but one was missed on CT. CT was the most accurate modality to determine the extent of the lesion and adjacent or distant metastasis but it could be misdiagnosed as submucosal tumor if the extraluminal component is large. UGI, CT and US can be used complementary for accurate diagnosis of a duodenal cancer. US may be used as a screening tool for detecting duodenal cancer.

10.Percutaneous drainage of pancreatic abscess and pseudocyst.

Percutaneous drainage of 4 pancreatic abscesses and 5 pancreatic pseudocysts was performed under the guidance of ultrasonography or fluoroscopy. Clinical inprovement following the percutaneous drainage enabled surgical procedure in 2 of 4 pationts with pancreatic abscesses and obviated surgery in the other 2. In patients with pancreatic pseudocysts, recurrence have nat been observed for the last 3 years. Percutaneous drainage is a safe and effective procedure and could be the method of choice in the management of pancreatic pseudocyst that is symptomatic and enlarging. Percutaneous drainage could play a significant role in the management of the patients with pancreatic abscess.
