2.A Study of Immunological Function in Workers Occupationally Exposed to Toluene.

To assess the immunological function of toluene exposed group, the proportions of T lymphocyte, B lymphocyte, CD 4 cell, CD 8 cell, the ratio of CD4 to CD8 (CD4/CD8) in peripheral blood were measured on twenty-one toluene exposed workers and twelve healthy workers who did not have previous history of toluene exposure. In addition, to evaluate the present status of toluene exposure, urinary hippuric acid concenturations were measured in exposed group. The mean concenturation of urinary hippuric acid was 2.84 g/creatinine g in exposed group. The proportions of T lymphocyte, B lymphocyte, CD8 cell and CD4/CD8 of exposed group were slightly lower than non-exposed group except the proportion of CD4 cell which was similar in both groups. But these differences were not statistically different in both groups. The proportions of T lymphocyte and CD4 cell were significantly correlated with the length of duration in exposed group (P<0.05)

3.MRI of the diffuse axonal injury.

CT has facilitated early recognition and treatment of focal brain injuries in patients with head trauma. However. CT shows relatively low sensitivity in identifying nonhemorrhagic contusion and injuries of white matter. MR is known to be superior to CT in detection of which matter injuries, such as diffuse axonal injury. MR imaging in 14 cases of diffuse axonal injury on 2.0T was studied. The corpus callosum, especially the body portion, was the most commonly involved site. The lesions ranged from 5 to 20 mm in size with ovoid to elliptical shape. T2WI was the most sensitive pulse sequence in detecting lesions such as white matter degeneration, hemorrhagic and nonhemorrhagic contusion. The lesions were nonspecific as high and low signal intensities on T2WI and T1WI respectively. CT showed white matter abnormality in only 1 case of 14 cases. We propose MR imaging as the primary imaging procedure for the detection of diffuse axonal injury because of its multiplanar capabilities and higher sensitivity.

4.In Vivo Delineation of Regional Myocardial Perfusion in Open-Chest Dog by Hydrogen Peroxide Myocardial Contrast Echocardiography.

Intravascular injection of hydrogen peroxide produces oxygen microbubbles suitable for echocardiographic contrast enhancement. To evaluate the effect of a method of myocardial contrast 2-D-echocardiographic delineation of myocardium during acute coronary occlusion, injection of a fresh mixture of 2ml of 0.2% H2O2 and 1ml of heparinized dog blood into aortic root were made in 12 poenchest dogs 10 minutes after occlusion of left anterior descending coronary artery distal to the first diagonal branch and left ventricular short axis 2-D echocardiographic images at the midpapillary muscle level were obtained. On injection of H2O2 blood mixture normally perfused myocardium was enhanced in echodensity but the area of malperfusion did not change in echodensity. The borderlines between the area of normal perfusion and malperfusion was well delineated. The malperfused area measured at mid papillary muscle level by planimetry area method was 29.7+/-6.0% and 32.6+/-6.7% by endocardial circumferential length method. There was a linear correlation between planimetric estimate of area of malperfusion by H2O2 contrast echocardiography and visual determination of regional wall motion abnormality by 2-D echocardiography(r=0.93, P<0.001). There was no change in heart rate before, during and after H2O2 injection. Infection of H2O2 blood mixture caused bradycardia(8.3%), second degree A-V block(16.6%) and ventricular fibrillation(8.3%). H2O2 clearance was achieved in 3-10 minutes. These findigs suggest that H2O2 enhanced myocardial contrast ehocargiography using 2ml of 0.2% H2O2 and 1ml of blood muxture is an accurate, reproducible, real-time in vivo method of quantifying the extent of myocardial perfusion defect during acute coronary occlusion in dog.

5.Diagnostic significance of computed tomography in gastric cancer

Gastric cancer is the most common gastrointestinal malignancy in Korea. Identification and evaluation ofgastric mass lesions and regional-distant metastases by abdominal CT scan are important for the treatment planningand prognostic implications of gastric cancer patients. Author reviewed CT scan are important for the treatemntplaning and prognostic implications of gastric cancer patients. Author reviewed CT scan of 61 cases of pathologyproven gastric cancer, retrospectively, for recent 20 months from July 1983 to Feb. 1985 at department ofradiology, Korea University, Hae Wha Hospital. The results were as follows: 1. There were 50 cases of advanced adenocarcinoma, 8 cases of early gastric cancer, 2 cases of leiomyosarcoma, and 1 case of lymphoma in total 61cases. 2. The sex ratio of male to female was 2:1. Age distribution was from 24 to 75 year old and peak incidencewas in 6th decade. 3. The most frequent site of involvement with gastric cancer was gastric antrum in 51%. 4. 48of the 50 patients with advanced gastric adenocarcinoma(96%) had a wall thickness greater than 1 cm, and all of 8cases of early gastric cancer had a wall thickness less than 1 cm. Regional lymph node tumor infiltration wasfound in 100% of gastric wall thickness greater than 2.0cm, in 64% of cases of 1.5 to 2.0cm, in 50% of cases of1.9 to 1.5cm, and 12.5% of cases of less than 1.0cm. 5. In a comparison of enlargement of reginal lymph node by CTscan to tumor infiltration of regional lymph node by histology, senitivity was 52%, specificity was 87%, and reliability was 66%. 6. The structures involved by distant metastases of these cases were the retroperitoneallymph node in 15, liver in 8, and pancrease in 3. 7. The diagnostic accuracy of CT staging was considered about68% by correlation of the surgical and histological findings. 8. The CT scan is one of the accurate and simpletool for evaluation of size, shape, extent, as well as distant metastases in the cases of gastric malignancies.

6.The Assessment of Acquired Dyschromatopsia among Organic-Solvents Exposed Workers.

We investigated the occurrence of color vision loss in 70 organic solvent mixtures exposed workers and in 47 controls. Color Vision was assessed with a color arrangement test designed to identify the defective color sense, the Han Double 15-Hue Test. The results of the test were no significant difference between exposed workers and controls in the proportion of subjects who committed one or two errors. Quantitative analysis, using color confusion index(CCI), showed no signifiant difference between exposed workers and controls. A significant linear correlation was present between age and CCI in both exposed workers(CCi=0.0056age + 0.94; r=0.23; p<0.05) and controls(CCI=0.0066age + 0.86; r=0.33; p<0.05). Qualitative analysis of the patterns on the hue circle showed that the prevalence of acquired dyschromatopsia was 21% in both and no significant difference. Multiple regression analyses showed that age was significantly related to color vision loss. These results did not provide evidence of a relationship between organic solvents exposure and incidence of color vision loss. In field studies for monitor the people at risk of the acquired color vision loss involving low-dose organic solvents exposed workers, both quantitative and qualitative information should be considered.

7.Antianginal Effect of Fenalcomine Hydrochloride.

Antianginal and untoward effects of fenalcomine hydrochloride were studied in 24 cases of angina pectoris. Following discontinuation of all medications for 2 weeks except for liberal use of sublingual nitroglycerin for the relief of anginal attack, fenalcomine, 150mg a day in 3 divided doses, was given for 8 to 16 weeks. In 20 cases, routine blood counts including platelets, serum electrolytes and cholesterol as well as blood sugar level were checked before and at the completion of the medication. hepatic and renal functions were also studied. The effect of fenalcomine on severity, frequency and duration of anginal attack was excellent to good in 17 cases(70.9%), and was fair in 5 cases(20.8%). In the remaining 2 cases(8.3%), who responded poorly had angina pectoris for more than 3 years. It appeared that fenalcomine was similarly effective in those with or without associated hypertension or diabetes mellitus, or history of previous myocardial infarction. The comparison of the results of post-treatment laboratory tests with pre-treatment data revealed no significant changes. Furthermore, no untoward clinical reactions attributable to the drug was noted in all cases. These facts suggest that fenalcomine is a reliable and well tolerated antianginal agent which can be used singly or in combination with other agents.

9.The Newer Assessment Tools for Functionings and the Personal and Social Performance Scale in Patients with Schizophrenia.

Personal and Social Performance scale (PSP) is the new measurement scale of patient functioning. PSP is simple and useful questionnaire for psychiatrist and medical personnel, especially for rehabilitation center workers. Compared with GAF (Global Assessment of functioning Scale), PSP has relatively good covariance with social and occupational and psychological symptoms. In clinical practice, PSP can be adopted for treatment planning and the evaluation of treatment effect, especially for the patients with schizophrenia.

10.Factors Affecting Invasive Management after Unplanned Extubation in an Intensive Care Unit.

BACKGROUND: Unplanned extubation (UE) of patients requiring mechanical ventilation in an intensive care unit (ICU) is associated with poor outcomes for patients and organizations. This study was conducted to assess the clinical features of patients who experienced UE and to determine the risk factors affecting reintubation after UE in an ICU. METHODS: Among all adult patients admitted to the ICU in our institution who required mechanical ventilation between January 2011 and December 2013, those in whom UE was noted were included in the study. Data were categorized according to noninvasive or invasive management after UE. RESULTS: The rate of UE was 0.78% (the number of UEs per 100 days of mechanical ventilation). The incidence of self-extubation was 97.2%, while extubation was accidental in the remaining patients. Two cases of cardiac arrest combined with respiratory arrest after UE were noted. Of the 214 incidents, 54.7% required invasive management after UE. Long duration of mechanical ventilation (odds ratio [OR] 1.52; 95% confidence interval [CI] 1.32-1.75; p = 0.000) and high ICU mortality (OR 4.39; 95% CI 1.33-14.50; p = 0.015) showed the most significant association with invasive management after UE. In multivariate analysis, younger age (OR 0.96; 95% CI 0.93-0.99; p = 0.005), medical patients (OR 4.36; 95% CI 1.95-9.75; p = 0.000), use of sedative medication (OR 4.95; 95% CI 1.97-12.41; p = 0.001), large amount of secretion (OR 2.66; 95% CI 1.01-7.02; p = 0.049), and low PaO2/FiO2 ratio (OR 0.99; 95% CI 0.98-0.99; p = 0.000) were independent risk factors of invasive management after UE. CONCLUSIONS: To prevent unfavorable clinical outcomes, close attention and proper ventilatory support are required for patients with risk factors who require invasive management after UE.
