1.Skin Graft and Chronic Osteomyelitis

Authors presented the gratifying result of treatment of chronic osteomyelitis with overlying skin defect and/or poor skin condition, which developed the wide skin necrosis after saucerization and resulted the second skin graft necessary. Five cases were treated by primary extensive saucerization of tibia which left a cavity with bleeding walls. The exposed bony surface with bleeding walls was covered by delayed free skin graft when the good granulation covered the bony surface. Through this experience, delayed free skin graft is justified the best procedure in these cases to cover the post-surgical raw bone surface and to have primary healing of the bone infection. A method of hemicylindrical pedunculated skin graft to obliterate the deeply excavated surgical bony defect is introduced.

2.Double Incision Technique for Carpal Tunnel Release.

A preliminary study was conducted to evaluate the immediate postoperative recovery and socioeconomic efficacy with the surgical release of Carpal tunnel syndrome by double incision technique. Sixty cases of carpal tunnel syndrome were operated by one surgeon between 1993 and 1995 and 44 cases in 39 patients were available for this study. Surgery was done under axillary block and patients were evaiuated preoperatively, 3 weeks and 6 weeks each postoperatively. The evaluation was focused on the immediate recovery in terms of subjective symptoms of pain and paresthesia, local tenderness, strength of grip power and pinch power and postoperative disability. 1l was noted that surgical release of carpal tunnel syndrome utilizing double incision technique was as effective as seen in the endoscopic release in terms of quicker recovery of pain, local tenderness and strength. It was also noted that this double incision technique was as safe and economically beneficial as observed in the conventional open procedure.

3.Clinical Experience Of Osteochondroma

Osteochondroma is the most common of primary benign bone tumors. It is characterized by protruding mass from metaphysic of long bone with semitranslucent cartilaginous cap on its surface. This lesion, which has its own growth plate, usually stops growing at skeletal maturity. The multiple variant of osteochondroma, namely the osteochondromatosis, may show various deformities around the joints in its course. In addition, sarcomatous change is sometimes developed. One hundred and twenty one patients were diagnosed as osteochondroma or osteochondromatosis from Jan. 1981 to Dec. 1991, and followed-up for 3.4 years(6months-8years). There were 74 cases of solitary osteochondroma(male : female=1.47 : 1), and 47 cases of osteochondromatosis (male : female=1.61:1). The age at initial visit was averaged 19.5 years(2 years 8 months-58 years). In our experience, common sites were distal femur, proximal tibia, proximal humerus in solitary osteochondroma, proximal tibia, distal femur, distal tibia in osteochondromatosis. Mass with or without pain was the most common chief complaint. Familial histories were noted in 38% of osteochondromatosis. The deformity, which needed correction, were found in 10 among 119 cases ; corrective osteotomy in 7 cases, lengthening with Ilizarov apparatus in 3 cases. The complications such as limitation of motion, sensory disturbance, recurrence, persistent deformity after corrective surgery, epiphyseal plate injury, superficial infection, were noted in 28%. During the follow-up period, there were one case of malignant transformation, and recurrence was identified in 7 cases and three of them were reoperated. Interestingly, progressive deformity was noted in 3 cases postoperatively. And, if the tumor produces no symptom and no functional impairment, we recommend minimizing the indication of surgical intervention. In particular, special attention must be taken in growing children in order to avoid inadvertent complications such as progressive deformity.

4.Penetrating Atherosclerotic Ulcer of the Descending Thoracic Aorta in a Patient with Heterozygote Familial Hypercholesterolemia.

The penetrating atherosclerotic ulcer of the aorta resulting from the atherosclerosis of the aortic wall can clinically mimic type III aortic dissection, since both diseases produce the ulceration and dissection of aortic wall. However, their imaging features and pathophsiologies are distinctly different from each other. Familial hypercholesterolemia(FH) menifests overt hyperlipidemia that can results in premature atherosclerosis of the aorta as well as the coronary artery. We report a clinically and radiologically evident case of perntrating atherosclerotic ulcer of the descending thoracic aorta which was developed in a 36-year-oldd heterozygote FH male.

5.Changes of ERG Parameters in Diabetic Retinopathy.

The purpose of this study is to evaluate the parameters of electroretinography[ERG]in eyes with diabetic retinopathy. The ERG responses were recorded in 46 normal subjects and 72 diabetic patients.We obtained the values of 12parameters from 5 basic responses and compared the changes of ERG parameters with the stages of diabetic retinopathy. The amplitude of oscillatory potentials decreased progressively from normal to severe nonproliferative diabetic retinopathy[NPDR].The b-wave implicit time of cone response was delayed in eyes with moderate NPDR to high risk proliferative diabetic retinopathy[PDR]. The b-wave amplitude of rod response was reduced in eyes with mild to moderate NPDR.The a-wave amplitude of maximal combined response significantly decreased between severe NPDR and early PDR.

7.Chiari Pelvic Osteotomy in Children and Adolescent.

Twenty five patients (twenty seven hips) who had Chiari osteotomy at Kang Nam St. Marys Hospital between 1980 and 1995 were reviewed to evaluate the factors in the operative technique that contribute to successful outcome and assess the clinical results in various conditions. The length of follow-up ranged from one to fourteen years and the age of at operation ranged from four to twentythree years. Eighteen patients had developmental dysplasia of the hip: four, septic hip: three had another disorders, Prior to the Chiari osteotomy, fourteen hips had an femoral osteotomy and four, trochanteric arthroplasty. We used to the standard osteotomy as described by Chiari with certain modification. A pneumatic saw and osteotome are used instead of Gigli saw. This technique is simple procedure to make the correct level and angle. Bone graft was not performed in all cases even the osteotomy was displaced more than 50 percent of the iliac width. The overall results were 12 excellent, eight good, five fair, and two poor. In eleven patients, the osteotomy had to be displaced more than 50 percent to provide good coverage of the femoral head. Their results were good or excellent. A good result will be obtained if enough attention is paid to displacing the osteotomy. The osteotomy using the pneumatic saw provides accurate level and direction of osteotomy and it is an simple procedure also.

Although many reports have been published about Legg-Calve-Perthes disease, developmental dysplasia of hip, it is difficult to gain a satisfactory results in treatment. We experienced 9 year-old girl with ipsilateral neglected DDH combined with contralateral LCPD, and treated with Klisic operation for DDH and femoral valgus osteotomy for hinged abduction of LCPD. We gained satisfactory results and report this rare case.
