1.Cholesteatoma of the Renal Pelvis: A case report.

Cholesteatoma in the kidney and ureter are accumulations of waxy, gray flakes of keratin materials, secondary to squamous metaplasia of the transitional epithelium. Herein, we describe a case of cholesteatoma in the renal pelvis of a 69-year-old woman, and give a brief review of the literature. In the upper pole of the left kidney was a 6 cm cystic lesion filled with a thick, flaky, grayish, comified material. Microscopically, the cystic area showed calyceal and pelvic structures being replaced by keratinizing stratified squamous epithelium. The surrounding renal parenchyma was atrophic with features of chronic pyelonephritis.

2.Two Cases of Metastatic Carcinoma of Breast.

We report two cases of rare, and clinically atypical metastatic breast carcinoma; zosterifo pattern, and cicatrirical pattern. A 51-year-old female had zosteriform eruption in the right chest and arm, which was diagnosed as an inflammatory, and telangiectatic type breast carcinoma I. skin biopsy. Another patient, a 71-year-old female,. had a retraction of nipple and breast, al erythematous nodular plaques like a surgical scar with distant metastasis to the counterlateral inguinal lymph node. A skin biopsy disclosed nodular type lymphatic dissemination of brea.,: carcinoma. In both patients, cutaneous lesions were the first signs of breast cancer with a gra-r prognosis.

3.Nevus Lipomatosus Cutaneus Superficialis: Report of a Case.

Nevus lipomatosus cutaneous superficialis(NLCB) is a rare condition consisting of ectopic fat tissue in the dermis and having the predilection for the pelvic girdle, especially gluteal region. The patient was a 20 year-old female. About 8 years prior to the first visit she had noticed the rice-sized papules on her lower back, which had been gradually increased in size and coalesced to become two, cerebriform, skin colored to yellowish tumor masses of soft consistency, distributed on both sides of the midline of lower back, and measuring respectively 3.5X7.Qcm and 7.0x11.5cm in size. Jihe biopsy specimen taken from the tumor mass revealed the ectopic presence of fat lobules occupying half of the reticular dermis. The fat lobules consisted of almost mature fat cells with the presence of dilated capillaries in their centers.

6.Age - Related Changes of Glycosaminoglycan in Senescence - Accelerated Mouse ( SAM ) Skin.

BACKGROUND: The various manifestations of intrinsic cutaneous aging may reflect the age-related changes of dermal ground aubstances, the important components of the dermis. OBJECTIVE: This study was undertaken to observe whether there is any age-relsted change, quantitatively and qualitatively, in dermal glycosaminoglycan(GAG) METHODS: The Senescence-A.ccelerated-Mice(SAM) strain "Prone" to develop senescence(P/8), aged 4 days(P-Y a group, n=5) and 4 months old (P-0 group, n=5) were used, with SAM strain Resistance to senescence (R/1), aged 4 montha old (R-0 group, n=5). The whole skin of SAM was incubated in 0.1 sodium phosphate buffer(NaPB), 2M guanidine-HC1, and 4M guanidine-HCl, sequencially, for the extractiop of GAG. The amount of GAG was measured by using alcian blue and by methylene blue metachrometic assay. Uronic acid(UA) was estimate l employing carbazole reaction. Extracted skin protein profiles were analyed by SDS-PAGE. RESULTS: 1. The total contents of GAG per wet weight of skin, as measured uairq, alcian blue, was highter in R-0 group than R-Y and P-Y group, which is statistically significant. 2. The methylene blue metaehromatic assay yielded highter absorbance values in 2M guanidine-HCl extract than NaPB ext:racts. 3. The total contents of UA decreased with aging in R strains, buttriking increase was noted in the P strain. 4. On SDS-PAGE, the protein profiles of NaPB extracts showed similarity to serum protein. 125 kDa protein band was noted only in guanidine-HCl extracts. 37.40kDa protein bands were appeared in 2M, 4M guanicline-HCl extract of R-0 group. But. there was no significant difference in both strains. CONCLUSION: These findings suggest that macromolecules, such as GAG, is one of the target molecules of the cutaneous eging process, and these change muy be related to the age-related changes of dermal water content.

9.A Case of Generalized Dermal Melanocytosis.

Dermal melanocytosis includes Mongolian spot, nevi of Ota and Ito, dermal melanocytic harnartoma, and blue nevus. Nevi of Ota may be respectively associated with Mongolian spot, blue nevi and unilateral or bilateral nevi of Ito. We present a case, a 24 year-old female, who had bilaterally brownish or slatebluish pigmented patches on the face, sclera, neck, anterior chest, deltoid, supraclavicular, scapular, back and sacrococcygeal area. And also she had a bluish, bean-sized nodule on her left mid-back. It is likely that this case may be considered as an extremely extensive case of pig:m.entary anomaly, associating aberrant and persistent mongolian spots, extensive nevus of Ito, bilateral nevus of Ota and blue nevus. To our knowledges it seems that such case has not previously been reported in Korean literature.

10.Structural Alterations of Retinal Detachment After Intravitreal Triamcinolone Injection in Rabbit Eyes.

PURPOSE: To investigate the effect of localized retinal detachment on both the detached and attached regions, and to determine the effect of triamcinolone on Muller cell gliosis. METHODS: Pars plana vitrectomy was performed in both eyes of 12 pigmented rabbits. A dome shaped retinal detachment was made by injecting sodium hyaluronate into the subretinal space. Triamcinolone (5 mg) was applied intravitreally to one eye (12 eyes). The detached retinal area and the neighboring attached region were studied by light and electron microscopy 3, 7, and 28 days after surgery. Tissues were prepared in 5 um sections for hematoxylin-eosin staining and immunohistochemistry with antibody to glial fibrillary acidic protein (GFAP). RESULTS: In addition to the well-known degeneration of photoreceptor cells in the detached retina, an incomplete but severe loss of ganglion cell axons occurs in both the detached and the attached regions. The total retinal thickness gradually decreased in the detached areas, while the thickness of the inner retinal layers remained virtually unchanged over several weeks. Gliotic alterations were apparent in both the detached and non-detached retinal areas, and intravitreal triamcinolone did not alter these gliotic alterations of Muller cells. CONCLUSIONS: It is noteworthy that progressive retinal destruction also occurs in the attached retina after local detachment. This may account for visual impairment in strikingly large areas of the visual field, even after retinal reattachment.
