1.Effect of Pantethine in the Treatment of Hyperlipidemia.

After pantethine in doses of 540 mg per day was administered for 8 weeks in 20 patients with hyperlipidemia, the serum levels of total cholesterol, triglyceride and HDL-cholesterol were compared with the pretreatment level. The serum level of total cholesterol decreased to 240+/-52 mg%[mean+/-standard deviation] after 4 weeks and 223+/-41 mg% after 8 weeks from the pretreatment level of 293+/-38 mg%(p<0.01). The serum triglyceride level also decreased to 254+/-109 mg% after 4 weeks and 239+/-114 mg% after 8 weeks from the pretreatment level of 375+/-126 mg%(p<0.01). We report a case of coronary arteriovenous fistula in a female adult confirmed by aortography. The fistulous communication was between right coronary artery and right ventricle. This is the first case report of adult patient with coronary arteriovenous fistula in Korea.

3.Post Traumatic Chronic Lymphedema: A case report.

4.Variant Origin of Obturator Artery: A Branch of Inferior Epigastric Artery from External Iliac Artery.

The obturator artery normally originates from the internal iliac artery. However, variation in the origin of obturator artery has been reported in many countries. Since no such case has been reported in Korea, we examined variations in the origin of obturator artery in cadavers donated to the medical school at the Chungbuk National University. Thirty-six pelvic halves from 18 cadaveric subjects (13 males and 5 females) were studied in this study. Normal origin of the obturator artery from the internal iliac artery was observed in 88.9% (16/18) of cadavers or in 91.7% (33/36) of pelvic halves. A variation in the origin of obturator artery was observed in 11.1% (2/18) of cadavers or in 8.3% (3/36) of pelvic halves. All of the variant obturator arteries originated from external iliac arteries as branches of inferior epigastric arteries. Bilateral presence of variant obturator arteries was observed in 5.6%(1/18) of cadavers. The obturator artery arose from inferior epigastric artery at a distance of 1 to 2.4 cm from origin point of inferior epigastric artery, and then the obturator artery ran inferiorly and medially with the inferior epigastric artery running superiorly and laterally. Presence of variant obturator artery would be important to clinical fields with interest to pelvic anatomy, such as radiology and surgery.

6.Synergistic Effect of Dermatophagoides farinae and Lipopolysaccharides in Human Middle ear Epithelial Cells.

PURPOSE: Although the concept of "one airway, one disease," which includes the middle ear space as part of the united airway is well recognized, the role of allergens in otitis media with effusion (OME) is not clearly understood. We aimed to investigate the effect of the interaction between Dermatophagoides farinae (Der f) and lipopolysaccharide (LPS) on the induction of epithelial inflammatory response in vitro. METHODS: Primary human middle ear epithelial cells were exposed to Der f, LPS, or both in different sequences, and the magnitude of the immunologic responses was compared. The mRNA expressiona of mucin (MUC) 4, 5AC, 5B, 8, GM-CSF, TNF-α, TLR4, and MD-2 were evaluated using real-time PCR. MUC levels before and after siRNA-mediated knockout of TLR4 and MD-2 were assessed. Lastly, the involved cell signaling pathway was evaluated. RESULTS: The expressiona of cytokines, and the MUC 4, 5AC, 5B, and 8 genes were augmented by pretreatment with Der f followed by LPS; however, reverse treatment or combined treatment did not induce the same magnitude of response. Increased MUC expression was decreased by TLR4 knockdown, but not by MD-2 knockdown. The signal intensity of MUC 8 was higher in MD-2 over-expressed cells than in those exposed to LPS only. The translocation of nuclear factor-κB was observed in cells pretreated with Der f followed by LPS. CONCLUSIONS: When Der f treatment preceded LPS exposure, Der f and LPS acted synergistically in the induction of pro-inflammatory cytokines and the MUC gene, suggesting an important role in the development of OME in patients with concealed allergy airway sensitization.

7.A Pilot Study on Environmental Factors Contributing to Childhood Home Slip-Down Injuries.

PURPOSE: The purpose of this study was to investigate environmental factors contributing to childhood home slip-down injuries. METHODS: Among a total of 2,812 injured children in our Customer Injury Surveillance System (CISS), we performed a prospective study on 262 children with home slip-down injuries who visited the pediatric emergency department of Asan Medical Center between March 2008 and February 2009. We made a frequency analysis on parameters such as activities just before the accident, the presence of any obstacles or lubricant materials, specific home place in the home where the injuries occurred, flooring materials on which the slipdown happened, additional objects hit after slip down, the site and kind of injury, the duration of therapy, and the disposition. RESULTS: Walking was the most common activity just before the injury. Because rooms and bathrooms were most common places in the home for slip down injuries, laminated papers/ vinyl floor coverings and tiles were the most common flooring materials used in the places where the injuries occured. Most commonly, no obstacles caused the children to slip down, but the furniture, stairs, doorsills, wetness, or soapy fluid followed after that. Over half of the children who slipped (58%) also collided with other than the floor itself after the slipdown, most common objects hit were the edges of the furniture, and doorsills, followed by stairways. The head and neck were the most commonly injured sites, and a laceration was the most common kind of injury. Most children needed less than 1 week of therapy, only 4 children (1.53%) admitted. There were no mortalities. CONCLUSION: The environmental factors contributing to slip-down injuries were the bathroom, laminated papers/vinyl floors, the furniture, stairs, doorsills, and wetness or soapy fluid. Especially, the furniture, stairs, and doorsills can be both primary obstacles and secondary collision objects. For the safety of our children, we must consider these factors on housing, when decorating or remodeling our house.

8.The Correlation of Grade Point Average of Medical School and the Score of Korean Medical Licensing Examination.

PURPOSE: This study analyzed the correlation between grade point average (GPA) of medical school and the score of Korean Medical Licensing Examination (KMLE). METHODS: This study based on the results of 67th KMLE applicants who graduated from a college of medicine in 2003. We also gathered data of these applicants from the college of medicine: gender, age, type of entrance, GPA of basic medicine, clinical medicine, clinical clerkships and final test scores. We analyzed whether there was discrimination between achievement of KMLE passed and that of KMLE failed, which of variables affected the results of KMLE. RESULTS: 173 applicants passed KMLE among 189. There were significant correlations between basic medicine, clinical medicine, final test score and the score of KMLE (respective p-value; < 0.0001). There were also significant differences of GPA between KMLE passed applicants and failed. Final test scores were the most correlated with those of KMLE. If the GPA of 2nd grade was below 2.5 and the GPA of 3rd year grade was below 2.3, they was a high-risk group for failing KMLE (sensitivity 100%, specificity 90%). CONCLUSION: There were significant correlations between the GPA of medical school and the score of KMLE, and significant differences between KMLE passed applicants and failed. A high-risk group of failing KMLE was the students that the GPA of 2nd grade was below 2.5 and the GPA of 3rd grade was below 2.3.

9.A Case of Conjunctival and Corneal Calcification in a Child on Peritoneal Dialysis.

Calcification in patients with endstage renal disease on renal replacement therapy can occur in extraskeletal area such as conjunctiva and cornea. Conjunctival and corneal calcification (CCC) has mostly has been reported in adults with endstage renal disease on hemodialysis. CCC seems to be associated with the duration of renal replacement therapy, and high CaxP value. We report a 10-year-old girl who was on peritoneal dialysis for 31 months and presented with CCC on both eyes. Her corneal calcification was resolved after the epithelial debridement and ethylenediaminetetraacetic acid(EDTA) soaking therapy.

10.A Comparison of Efficacy and Safety of Magnesium sulfate and Ritodrine Hydrochloride in the Management of Preterm Labor.

OBJECTIVES: The purpose of this study was to evaluate the efficacy and safety of magnesium sulfate and ritodrine hydrochloride in the management of preterm labor. METHODS: This study was undertaken to assess the clinical efficacy & safety of magnesium sulfate and ritodrine hydrochloride on 120 patients admitted with preterm labor from Jul. 1, 1996 to Dec. 31, 1998. RESULTS: 1. The incidence of preterm labor was showed 6.0~7.7% of total number of delivery. 2. The risk factors of the preterm labor were premature rupture of membrane, severe preeclampsia, previous preterm delivery, twin pregnancy, placenta previa, fetal anomaly, incompetent cervix, placental abruption, and uterine myoma in order, but 28.6% of preterm labor had no apparent risk factors. 3. The days gained in uterus was not statistically different between the magnesium sulfate group and the ritodrine group (p<0.05) but longer in the two groups than the control group. And the delivery time was also not statistically different between the magnesium sulfate group and the ritodrine group (p<0.01) but significantly increased in the two groups than the control group. 4. The rate of complete and incomplete success was similar as 12.5% and 35% (total success rate 47.5%) in the magnesium sulfate group, 7.5% and 45% (total success rate 52.5%) in the ritodrine group but only 2.5% and 27.5% (total success rate 30%) was showed in the control group. 5. The side effects were much more in the ritodrine group than the magnesium sulfate group. The patients requiring second-line therapy were similar in the both groups but the main cause was uncontrolled uterine contraction in the magnesium sulfate group, and intolerable side effects in the ritodrine group. 6. The pulse rate was not statistically different in magnesium sulfate group but markedly increased in ritodrine group (p<0.05). After managements of preterm labor, the serum potassium and ionized calcium level was significantly decreased in the two groups (p<0.05, p<0.01). CONCLUSION: There was not significantly different efficacy and safety of magnesium sulfate and ritodrine hydrochloride in the management of preterm labor.
