5.Neuroscience of Eating Behavior: Focusing on Intake.

In mordern society, nutritional and appetite disorders occur in epidemic proportions and are serious health harzards. Obesity and diabetes affect over 30% of American population, while eating disorders, such as anorexia nervosa and bulimia nervosa occur in a growing number of adolescences and young adults. The changes in various sociocultural aspects with the introduction of Westernized culture have had the effect of increasing the risk of same problems in Korea. Disorderd eating patterns are a primary symptom of numerous psychiatric disorders and loss of appetite and cachexia, during illness or in the elderly, preclude proper medical treatment for restoring good health or preserving life. Increased understanding of the systems of the body and brain, related to energy and nutrient balance, may help us to treatment and ultimately prevent these common disorders. In this review, the author highlights the psychobiological mechanisms or factors which are associated with eating behavior, especially in the view of intake psychobiology. This review would be concentrated on 1) the theoretical concepts and theories of eating behavior ; 2) the psychobiological determinants of food intake ; and 3) the psychobiological control of eating behavior.

6.The Study on the Control of Renin Secretion.

It is still not well documented what the basic roles for the regulation of renin secretion from the kidney take place. Since the early study on the renal ischemia for the production of hypertension was introduced, the renin-angiotensin system has been regarded as the possible pathogenetic mechanism for the renovascular hypertension. The renin-angiotensin system, however, could be activated by various stimuli, such as, the changes of intrarenal perfusion pressure, the load or concentriation of sodium at the sites of macular densa, the changes of the sympathetic nervous activity and the changes of potassium balance. To investigate the renin-angiotensin system and the influence of sympathetic nervous system on the regulation of renin secretion, the renovascular hypertension was induced in the dogs by constriction of unilateral renal artery, and the plasma renin activity was measured. The sodium load at the sites of macula densa was attained by furosemide, and then the activity of sympathetic nervous system was depressed by reserpine. The plasma renin activity was assayed by the method of Helmer and Cohn. By this bio-assay method, the plasma renin activity equivalent to 1 nanogram angiotensin-II can be measurable and the prepared plasma was found to have still vasopressor activity. The results observed in this experimental work are summarized as follows. 1. The blood pressure reached maximum on the 3rd postoperative day, and declined gradually to the level of preoperative day on the day of from the 6th to 12th day following constriction of renal artery. 3. The plasma renin activity was found to be well correlated with the increase of blood pressure, and then declined to its preoperative level with the reduction of blood pressure. It appears, therefore, that the renin-angiotensin system plays an important role in the pathogenesis of renovascular hypertension. 4. The urine flow rate in normal and reserpinized dogs was the same before administration of furosemide, but its rate was significantly increased in both groups after furosemide. The urine flow rate of reserpinized dogs, however, was significantly lower than that of the normal dogs during the first 5 minutes. 5. The urinary sodium excretion in normal and reserpinized dogs was significantly increased in both groups after administration of furosemide and there were no difference between the two groups. 6. The plasma renin activity of renal venous blood was significantly higher than that of femoral arterial blood in both groups before and after administration of furosemide. 7. The plasma renin activity of reserpinized dogs was significantly lower than that of normal dogs before administration of furosemide. After furosemide, however, the plasma renin activity was significantly increased in 30 minutes in both groups. This increase of plasma renin activity was less prominent in resepinized dogs than in normal. This results would suggest that the intrarenal mechanism regulating renin secretion also requires an intact sympathetic nervous system.

8.The Comparison of Aphakic Refraction Before and After Cataract Surgery.

We reviewed the records of 101 eyes who underwent extracapsular cataract extraction without intraocular lens implantation at Kosin Medical Center from Feb. 1987 to Mar. 1990. Of these, 47 eyes that showed postoperative corrected visual acuity of 0.5 more and more than 2 months' follow-up periods, were recruited for this study. To detect the changes of preoperative predicted aphakic refractions and postoperative observed aphakic refractions. we studied the changes of pre- and postoperative axial lengths, keratometric measurements, and observed aphakic refractions at postoperative 2 months were compared with predicted aphakic refractions, using linear regression formula. The results were as follows: 1. Distribution of age were in the range of 24 to 79 years(mean, 59.4 years), and seventh decade(31.8%) was most common. 2. Postoperative keratometric measurements was decreased in power of mean +/- SD; 0.04 +/- 0.14 diopters(p>0.05). 3. Postoperative axial length was increased by 0.20 +/- 0.09mm(p<0.05). 4. The difference between pre- and postoperative refractive power was mean +/- SD; 0.58 +/- 0.16 diopters(p<0.05).

10.Treatment of Comminuted - Intraarticular Fracture of Distal Radius using External Fixator: Comparative Study of Treatment by External Fixation with or Without bone Graft.

Distal radial fractures are recognized as very complex injuries with a variable prognosis that depends upon the fracture type and the treatment method. The purpose of this study is to compare radiological and clinical result between external fixation and external fixation with primary bone graft in intraarticular comminuted fracture of distal radius. From January 1993 to December 1995, 32 patients with comminuted intraarticular fractures of the distal radius were treated using a external fixation. 15 cases were treated with ligamentotaxis without bone graft and 17 cases were treated with additional primary bone graft. In last follow-up examination, the mean loss of radial length, radial inclination and volar tilt were less in with bone graft group than without bone graft group. According to Demerit point system Excellent to good results were obtained in 88.2% in cases of external fixation and bone graft group, 66.6% in cases of only extemal fixation group. External fixation combined with primary bone graft is more useful method for the reconstruction and treatment of comminuted intraarticular fractures of the distal radius than closed reduction and external fixation because cancellous bone graft not only provides early mechanical stability but also helps to reduce and supports the depressed intraarticular fragments. Furthermore, this method not only prevents late collapse of fracture but also enhance fracture healing.
