4.Utility of Computed Tomography in Diagnosis of Small Bowel Perforation after Blunt Abdominal Trauma.

BACKGROUNDS: The high mortality and morbidity rates associated with traumatic rupture of the hollow viscera have been attributed to the clinical difficulty in establishing an early diagnosis. The accuracy of CT in the detection of injuries of the solid visceral organ is well established, but the value of CT in diagnosing small bowel perforation artier blunt abdominal trauma is controversal. This study was conducted to ascertain CT findings of small bowel perforation result from blunt abdominal trauma. METHODS: A retrospective analysis of preoperative abdominal CT findings in 40 patients was performed. Small bowel perforation was confirmed by surgery. Precontrast and postcontrast enhanced CT images were obtained in all patients. RESULTS: Diagnostic findings of small bowel perforation including intraperitoneal or retroperitoneal free air, discontinuity of the bowel wall and extravasation of oral or IV contrast materials were observed in 34 cases(85%). The most common fading of small bowel perforation was intraperitoneal or retroperitoneal free fluid collection(90%), followed by segmental bowel wall thickening(83%), intraperitoneal or retroperitoneal free air(80%) and focal mesenteric fat infiltration(70%). CONCLUSIONS : The CT scan is sensitive and effective modality for evaluation of small bowel perforation after blunt abdominal truauma, because of high detectability of diagnostic antral suggestive CT findings.

Mesenchymal Chondrcsarcoma is a rare malignant tumor originally described by Lichtenstein and Bernstein in 1959. Since the original description, several other reports of this tumor have been published, bringing the total numbers of cases in the literature to about Sixty-five. We report a case of Mesenchymal Chondrosarcoma in an 11-year-old boy who complained of a palpable mass in the region of the left iliac crest.

9.Total Hip Arthroplasty in Hemophilic Arthritis: A Case Report

Total hip arthroplasty was performed on a 32 year old man with hemophilia A. A series of complications were encountered; namely, femoral shaft fracture during arthroplasty, hemolytic anemia, Factor VII inhibition, pseudotumor formation and wound dehiscence and sepsis as a result of shortage of cryoprecipitate supply, and general debilitation and septicemia from fhe use of immunosuppressive agents, finally leading to death 67 days after operation. Initially, 399 bottles of cryoprecipitate (each containing approximately 100 units of Factor VII) were anticipated. In practice, however, a total of 660 bottles and packs had been consumed on the patient when the supply was exhausted. Total hip arthroplasty can be carried out on hemophilia patients, provided that the planning be impeccable and the supply of antihemophilic factor virtually unlimited.
