3.Thalidomide Therapy on A Case of Prurigo Nodularis.

Prurigo nodularis is a troublesome chronic dermatosis that showed unsatisfactory response to conventional therapies. Since thalidomide has been applied to the treatment of prurigo nodularis, although the action mechanism is still uncertain, many dermatologists have confirmed its effectiveness. We treated a 54-year-old male patient who had prurigo nodularis affecting the whole body for 10 years with 100 to 300mg of thalidomide daily as the sole therapy for 4 months. The skin lesions were flattened leaving postinflammatory hyperpigmentation and the pruritus also sub-sided. Two years after stopping thalidomide, no recurrence was observed.

6.A case of Bart's syndrome.

Barts syndrome was desrribed by Bart(Bart et al, 1966) with hereditary congenital localized absence of skin from the lovrer legs associated with blistering of the sun and mouth and nail abnormalities. A newborn girl presented with a congenital localized absence of skn over both shins & dorsa of both feet which were covered with the thin, translucent, brown red glistening membranes. A number of blisters were seen on the back, thighs upper extremities, and both 2rd toe nails were absent. Electron microscopic examination of defected skin revealed dermo-epidermal . paration with focal disruption of basal lamina.

7.Clinical Nurses' HPV-related Knowledge and Perception of Cancer Causes: HPV Vaccinated vs. Not Vaccinated.

PURPOSE: The purpose of this study was to compare knowledge level of those clinical nurses who received HPV vaccine and those who did not and their perception of the relatedness of HPV vaccine to causes of cervical cancer. METHODS: A total of 249 clinical nurses were surveyed from June to July, 2009. The questionnaire originally developed by Kim & Ahn (2007) examined HPV-related knowledge originally and the tool for perception of the causes of cervical cancer was originally developed by Kim (1993). The total number of subjects equaled to: vaccination group of 52 (20.9%) and non-vaccination group of 197 (79.1%). RESULTS: Vaccination group showed significantly higher score of both knowledge of HPV vaccination and the perception of the cause of cervical cancer in comparison to the nonvaccination group at (p<.05). Among 4 subscales of the perception of causes of cervical cancer, destiny and constitution subscale scores were significantly different between the two groups at (p<.05). CONCLUSION: Clinical nurses need to constantly update with current knowledge of HPV and be prepared with currently changing cancer prevention strategies, especially in cervical cancer.

8.Effect of Posterior Chamber Intraocular Lens Implantation on Unilateral Pediatric Cataract.

The posterior chamber intraocular lens(PC-IOL) implantation is the most effective method for adult cataract. In children, however, its use and effectiveness are still being debatd. To investigate the effect of PC-IOL implantation in unilateral pediatric cataracts, we analysed the results of visual acuities and refractive powers, the presence of complications, and the differences according to the types of posterior capsulectomy in 16 eyes of 16 children who underwent PC-IOL implantation and amblyopia treatment for unilateral pediatric cataracts. Postoperatively, nine eyes(56.3%) showed an improvement of visual acuities more than two lines on Han`s visual acuity chart. Average difference between target which was adjusted to the fellow eye and postoperative refractive power is -0.68diopters after at least 8 months follow-up. Four eyes in which had an intact capsule after surgery or were underwent posterior capsulectomy developed after-cataract, but not in eyes underwent posterior capsulectomy and anterior vitrectomy simultaneously. In unilateral pediatric cataracts, PC-IOL implantation, posterior capsulectomy, and anterior vitrectomy may give a good result for visual rehabilitation but the follow-up is necessary to evaluate the long-term safety.

10.Ambulatory Blood Pressure Monitoring and Blood Pressure Load in Obese Children.

BACKGROUND AND OBJECTIVES: This study was aimed at evaluating the significance of blood pressure (BP) load in ambulatory blood pressure monitoring (ABPM) in obese children and adolescents. SUBJECTS AND METHODS: ABPM was conducted for 60 selected patients who had visited Sunlin Hospital between January 2008 and August 2008. Patients were classified into 3 groups; an obese group whose body mass index (BMI) was > the 95th percentile, an overweight group whose BMI was > the 85th percentile but less than the 95th percentile, and a normal group whose BMI was below the 85th percentile. Overall mean BP, day and night BP and BP load were measured by ABPM. RESULTS: Of the 60 patients, twenty-seven children belonged to the obese group, 9 and 24 to the overweight and the normal group, respectively. Among the three groups, the overall average systolic and diastolic BP, daytime diastolic BP, and systolic BP loads in daytime and nighttime were statistically different. Comparing the obese group with the normal group, systolic BP loads in daytime and nighttime in the obese group were significantly higher than those in the normal group. Also, the obese group had more patients whose BP loads were over 25% greater than the normal group while the difference in the number of patients with overall hypertension was not significantly different. CONCLUSION: Assessment of children's BP through assessment of BP load is a more detailed and precise tool than assessment through mean BP using ABPM and BP can be better controlled using measurement of BP load.
