1.The Elderlys Notion of Death-on an Island in Chonbuk Province.

Death is the critical moment of life. All elderly are faced with death. Therefore, the elderly's notion of death must be include in holistic care for the elderly. The purpose of this study was to understand the elderly's notion of death. The research questions were "What is the elderly's notion of death?", and "What is the embedded meaning of the elderly's notion of death?". This study was conducted using an ethnographic approch. The fieldwork for this study was done from July 1996 to July 1998 on an island in Chonbuk province. Data were collected by indepth interviews and participant observation. The result of this study is as follows The elderly's notion of death was founded on 'Being a ancestor god', 'Evaluation of one's own life', 'Be sujected to the evil(BuJung : the taboo of uncleanness)' 'Being a ancestor god' means the notion of death leading to eternal life. It means a relationship to this world, So, it is required for all descendants, especialy the son. Also, they thought that the Spirit is greater importance mote thait the body, and that the spirit is embedded in bone. And so, the spirit is delivered to descendants through a propitious site for grave(MoungDang). It is thought toa change in the fortune of the descendants(Balbok). 'Evaluation of ons's own life' means that life was evaluated personally and socially throughout the dy ing process. This means reflection on an individual's life and interpersonal relationship's, and appearance social aspects of the individual death. 'BuJung(be subjected to the evil)' emerged as fear about death, and pursuit of the cause of death, Most of the causes were attributed to supernatural entities. It contributed to the inspiration of a cornmunity identity. The elderly's notion of death was the present world oriented and actualized. This study contributed to the nurse's understanding of the Korean elderly's notion of death. Implications for nursing practice, education, and research were discussed. Further research at other settings and using other research methods were recommended.

3.A case of aberrant right subclavian artery associated with innominate artery compression syndrome.

An aberrant right subclavian artery, the most common congenital anomaly of the aortc arch, is rarely symptomatic during the infancy, if an anomalous origin of a right common carotid artery is also associateda varient of innomiate artery compression syndrome. We experienced a case of an aberrent right subclavian artery associated with an anomalous origin of the right common carotid artery in a female newborn, who showed severe respiratory distress soon after birth. The diagnosis was confirmed by aortogram and operative findings. the ligature and section of the aberrent right subclavian artery resulted in improvement of respiratory distress. A brief review of the related literature is also presented.

5.Histopathologic Features and Immunophenotype of 19 Primary Cutaneous Lymphomas.

The diagnosis of primary cutaneous lymphoma is based on a combination of clinical, histological, immunophenotypic and genetic criteria. Nineteen cases of primary cutaneous lymphomas were studied for clinicopathologic, immunophenotypic, and genetic features. Seventeen (89%) cases were T cell origin and two cases (11%) were B cell origin. CD30-positive cutaneous lymphoproliferative disorder was the most frequent subtype, occupying 42% (8 cases) of the cases. CD8 was positive in 5 cases consisting of 3 cutaneous T cell lymphomas and 2 anaplastic large cell lymphomas. CD4 was positive in 2 cases of mycosis fungoides and 3 cases of lymphomatoid papulosis. Six (67%) of 9 cases of cutaneous T cell lymphoma were positive for TIA-1. Ten (83%) out of 12 cases showed clonal rearrangements of TCR gamma genes, however, one T/NK cell lymphoma and one anaplastic large cell lymphoma did not. EBV association was detected only in T/NK cell lymphomas among 10 cases examined. In conclusion, our study showed higher proportion of CD30-positive lymphoproliferative disorders and less frequent mycosis fungoides in Korea compared to the incidences in Western countries. Our immunostaining results suggested that mycosis fungoides and lymphomatoid papulosis are CD4-positive T cell origin, however, the remaining primary cutaneous T cell lymphoma is predominantly CD8-positive cytotoxic T cell origin.

6.Beckwith-Wiedemann sydrome with left adrenal cortical neoplasm .

Beckwith-Wiedemann sydrome is a multisystemic pattern of congenital anomalies with overgrowth. This syndrome is first described independently by Beckwith in 1963 and by Wiedemann in 1964. There is wide spectrum of clinical manifestations, including prenatal or postnatal overgrowth, neonatal hypoglycemia, macroglossia, visceromegaly, omphalocele, hemihypertrophy and a predisposition for embryonal tumors, most frequently Wilms' tumor. We experienced a case of Beckwith-Wiedemann syndrome who developed left adrenal cortical neoplasm of indeterminate malignant potential.

8.Peripheral T Cell Lymphoma Associated with Hemophagocytic Histiocytosis Mimicking Malignant Histiocytosis.

BACKGROUND: Peripheral T cell lymphoma (PTCL), a prevalent form of non Hodgkin lymphomas in East Asia, can manifest fever, hepatomegaly, lymphadenopathy, pancytopenia and hemophagocytic histiocytosis (HPH). Similar clinicopathologic findings are also frequently encountered in reactive hemophagocytic syndrome (HPS) and malignant histiocytosis (MH) , thus diagnoses could be confused among them. With recent advancement of immunohistochemlal techniques, diagnostic accuracies have been improved and most cases of MH could have been reclassified as PTCL. In this study, we intended to delineate the lineage of atypical malignant cells in bone marrow of subjects which were previously diagnosed as MH or HPS with immunohlstochemical analysis and characterize clinlcophathologic findings of PTCL associated with HPH in the bone marrow. METHODS: Five cases dignosed as HPS, 3 as MH, 3 as presumed MH, and 7 as PTCL on bone marrow examination were enrolled in this study. We performed immunohistochemical stain for CD45, CD3, CD43, CD2O and CD68, then revised the diagnoses and summarized the clinical and morphologic features of PTCL associated with HPH. RESULTS: Eleven out of 18 cases were confirmed as PTCL which were previously diagnosed as MH(1), presumed MH(3) and PTCL(7). Eight cases of 11 PTCL showed HPH mimicking MH with infiltration of the atypical malignant cells, even if the proportion of atypical malignant cells was small on bone marrow aspirates. They manifested fever and hepatomegaly but didn't have lymphadenopathy at the early stage of disease. Subtypes of PTCL with HPH were PTCL, unspecifed (3), angioimmunoblastic T cell lymphoma (1) and undetermined (4). They showed poorer outcome in 3-month survival rate (25%) than in those with PTCL without HPH(100%). CONCLUSION: These results suggest that PTCL associated with HPH should be excluded from MH by immunohistochemical analysis. Considering that prognosis of PTCL with HPH is very poor, accurate and rapid diagnosis is needed for prompt treatment.

10.A Study on Stroke Patients' ADL, Depression, Self-Efficacy and Quality of Life.

This study was a descriptive research in search of a nursing intervention scheme to improve stroke patients' quality of life by understanding the relationship of stroke patients' ADL. depression, self-efficacy and quality of life with various variables and identifying factors that affect their quality of life. Each subject was interviewed one to one based on a structured questionnaire. With regard to research tools, Modified Barthel Index (MBI: Fortinsky et al., 1981), which was translated by Choi, Hye-sook (996), was used in measuring ADL, Randloff's (1977) tool. which was translated by Choi, Soon-hee, was used in measuring depression, and the tool developed by Sherer et al. (1982), which is to measures self-efficacy under general conditions not limited to specific conditions, and modified by O, Bok-ja (1994) was used in measuring self-efficacy. The quality of life was measured using the scale of satisfaction of life developed by Diener et al.(1985). The results of this study were as follows: 1. The means of ADL of the subjects was 79.5+/-1.9, depression 26.8+/-0.4. self-efficacy 47.1+/-5.7, and the quality of life 12.3+/-.9. 2. The subjects' quality of life showed a statistically significant difference according to gender (t=7.9, p= .006), satisfaction with income (F= 5.8, p = .004), the burden of medical fee (F=3.7, p= .028) and the period of disease (F=2.8. p= .042). 3. With regard to relationship among ADL, depression, self-efficacy and the quality of life, ADL was in a relatively low positive correlation (r= .293, p= .003) with and the quality of life, depression in a high negative correlation (r=- .634. p= .000) with the quality of life. and self-efficacy in a positive correlation with the quality of life (r= .388. p= .000). 4. Factors that made a significant influence on the quality of life were depression (8=- .309. p= .001) and satisfaction with income (8=-2.611. p= .001). Based on these results, this study made following suggestions: 1. It is necessary to run rehabilitation programs to improve stroke patients' ADL, depression and self-efficacy. 2. It is necessary to perform research of monitoring stroke patients' quality of life in various areas using measuring tools.
