Dysarthric patients often present velopharyngeal incompetence (VPI), characterized nasalization articulation for hypernasality,which seriously impaired their communication. Research of evaluation of VPI is mainly about cleft palate and postoperative, few about the dysarthria. Some approaches, such as physiologic approach to rehabilitation, have been used to correct hypernasality, and prosthesis,such as palatal lift prosthesis (PLP) and nasal speaking valve (NSV), are also proved effectively. PLP has been widely used for hypernasality oversea, but fewer in China.
4.Vowel's Nasalance and Contact Quotient in Children with Hearing Loss

Objective To investigate the vowel's nasalance and contact quotient (CQ) in children with hearing loss. Methods 57 subjects were divided to 3 groups: profound hearing loss children with hearing aids (HA, n=19), profound hearing loss children with cochlear implant (CI, n=19) and children without hearing loss (n=19). With the help of NV and electroglottography (EGG), the subjects' nasalance and CQ were recorded when they said /a/, /æ/, /i/, /o/, /u/. Results There was no significant differences among the 3 groups on nasalance (P>0.05). There was significant difference among 5 vowels on nasalance, ranked as /u/, /o/, /æ/, /i/, and /a/ (P<0.001). There was significant difference among 3 groups on CQ, ranked as HA, normal, and CI group (P<0.001). There was significant difference on CQ among 5 vowels,ranked as /i/, /u/, /æ/, /o/, and /a/ (P<0.001). Conclusion The voice and resonance should be assessed before speech therapy, which the speech therapy starts from the most correct vowel for children with profound hearing loss.

6.Perceived Confidence in Practice of Core Basic Nursing Skills of New Graduate Nurses.

PURPOSE: This study was to measure and identify the differences of perceived confidence in practice of core basic nursing skills performed by new graduate nurses in Korea. METHODS: The tool used in this study was a questionnaire for measuring the confidence in 20 items of core basic nursing skills which was structured based on Korean Accreditation Board of Nursing Education tool. 231 new graduate nurses participated in this study. The reliability of this questionnaire had Cronbach's alpha .918. RESULTS: Subjects who experienced simulation education and standard patient education were 86.6% and 35.9%, respectively. Item enema intervention, tracheostomy care, and blood transfusion showed low practice confidence level. These items showed significant differences on whether the subjects experienced simulation and clinical practicum or not. CONCLUSION: Developing and managing clinical education program under deep cooperation between practicum agency and clinical instructor are needed. Simulation practicum will complement insufficient core basic nursing skills between newly graduated nurses before they allocate at the clinical department.

7.Acute Compartment Syndrome Induced by Rhabdomyolysis Due to Antipsychotic Drug Overuse

A 49-year-old male was found unconscious at his accommodation and visited the emergency room. He was on antipsychotic and antidepressant drugs (vortioxetine hydrobromide, mirtazapine, sertraline hydrochloride, quetiapine, and alprazolam) for schizophrenia and major depression. At the time of discovery there were signs of overdose of the drugs around the patient. A physical examination revealed, pain, pallor, and edema in the left buttocks and lateral thigh. Active ankle movements below the left ankle were not possible and sensations in the tibia and peroneal nerves were lost. The pressure in the buttock compartment was measured at 42 mmHg. Magnetic resonance imaging revealed edema and high intensity signals in the left hip muscles and surrounding soft tissue. An emergency fasciotomy was performed and partial restoration of the lower extremity sensation and muscle strength were achieved after 24 hours.
8.Preliminary Results of Uncemented Metal on Metal Total Hip Replacement Arthroplasty.

Particulate wear debris(polyethylene, polymethylmethacrylate and metal particles, or combination of these) can initiate aseptic loosening. To eliminate polyethylene debris and reduce the total amount of wear, metal on metal bearings(Metasul) which were made of a wrought Co-28Cr-6Mo-0.2C alloy, were introduced. From May 1994 total hip replacements using a Metasul metal on metal articulation were performed by the authors. 34 patients with 40 hips with minimum follow-ups during 24 months were analysed in this study. The mean follow up period was 31 months(ranging from 24 to 47 months). We used Armor cups and standard cups as acetabular comporients and SL stems, CLS stems and cone stems as femoral components. Preoperative average Harris hip score of 48 points became postoperatively 93(from minimum 81 to maximum 100). And there were slight and occasional thigh pains in 2 cases(5%). There were no revisions for the early aseptic loosening of femoral stems or stem failures. Radiologic evaluation which revealed mild calcar atrophy was developed in 33 cases(82.5%), revealing cortical hypertrophy in 10 cases(25%). Osteolysis was not noted around the prosthetic stem nor around the cup. Radiolucent lines were found in 9 cases of femoral components(22.5%) and in 1 case of acetabular component(2.5%) and subsidence of more than 3mm in 1 case of femoral components(2.5%) were also found. There was 1 case of definitive loosening of acetabular component(2.5%). These also showed that endosteal bone formation adjacent to femoral prosthesis was developed in 22 cases(55%) but no distal pedestal and stem shift occurred. The short-term but satisfactory results of the contemporary metal on metal articulation investigated in this study were encouraging and warrant continued study.
