4.Cervical Range of Motion in Korean Adults.

OBJECTIVE: The purpose of this study was to determine the normal value of cervical motion in Korean adults. METHOD: Five hundred healthy subjects, 250 men and 250 women between third decades and seventh decades completed this study. The Cervical Range of Motion (CROM) instrument was used to measure six cervical motions and horizontal head motion. RESULTS: The elderly group had significantly less motion than the younger group for all six motions and horizontal head motions (p<0.05). Women had greater range of cervical motions than men in all cervical motions (p<0.05). CONCLUSION: The method using the CROM instrument seems to be useful for primary and follow up test of cervical disease.

5.Analysis of Radiological Change Following Reduction of Congenital Dislocation of the Hip

Development of acetabulum and epiphysis of the femoral head following reduction in seventy-four patients with ninty-one congenitally dislocated hips were assessed radiologically. Acetabular index, CE angle of Wiberg, neck-shaft angle of the femur and overgrowth of the pelvis and femur were measured according to the various treatment. To evaluate results, the method of treatment was divided into four groups; closed reduction, open reduction, open reduction combined with pelvic osteotomy and modified Klisic operation (open reduction, pelvic osteotomy, femoral osteotomy and so on). Each treatment was compared with three subgroups according to ages; first subgroup was belong to patients under one year of age, second from one year to three years, third over three years. This paper agrees to Harris (1976) and Lindstrom (1979) about improvement of acetabular index and CE angle of Wiberg that showed a rapid rate of repair in the first 6 months and thereafter slowly progressed to normal range of degree. Overgrowth of pelvis and femur was also noticed in cases of hips treated pelvic and/or femoral osteotomy, moreover even in cases of open reduction only. The goal of this preliminary investigation may be supposed to propose the idea which method of treatment to the first visiting patient will approach the most satisfactory final result.

6.A Study of Ni-resistant bacteria isolated from gingival crevicular fluid on the patients wearing Ni-Cr alloy prosthesis (In terms of molecular biological aspects).

As a material of metal-ceramic prosthesis, nickel as a form of Ni-Cr alloy has been used for many dental prostheses in many cases. However, several problems in use of the alloy have been revealed (ex ; tissue stimulation, skin allergy, hypersensitivity, cytotoxicity and carcinogenecity). Little is known about nickel with respect to the relationship between Ni-prosthesis and gaining of Ni-resistance in oral microorganisms. The present study was undertaken to check wheather use of Ni-prosthesis leads to occurrence of Ni-resistant microorganisms. So this study may suggest the possible relationships between the oral microorganisms and nickel-resistance in oral enviroment. Bacteria were isolated from the gingival crevicular fluid on the patients wearing Ni-Cr prosthesis. The isolated bacteria were tested for their Ni-resistance in nickel containing media at different concentration from 3mM to 110mM. E. coli HB101 was used as control. The Ni-resistant bacteria were isolated and biochemically identified. The Ni-resistant bacteria were tested several biochemical, molecular-biological tests. Performed tests were ; measuring the growth curve, antibiotic test, growth ability test in liquid media, isolation of the chromosome and plasmid, digestion of DNA by restriction enzyme, electrophoresis of chromosome and plasmid DNA, identification of Ni-resistant genes by the DNA hybridization. The results were as follows: 1) The bacteria isolated from gingival crevicular fluid on the patients wearing Ni-Cr alloy prosthesis showed nickel-resistance. 2) The isolated microorganisms grew at nickel containing media of high concentrations (60mM-110mM). 3) Based on the biochemical tests, the isolated microorganisms were identified as Enterococcus faecalis(13 cases), Klebsiella pneumoniae(1 case) and Enterobacter gergoviae(1 case). 4) Enterococcus faecalis expressed not only nickel resistance but also the multi-drug resistanceto several antibiotics ; chloramphenicol, kanamicin, streptomycin, lincomycin, clindamycin. However all strain showed the sensitivity against the tetracycline. 5) DNA hybridization result suggest that there is no homology between the previousely known gene of nickel resistance in Klebsiella pneumoniae and chromosomal DNA Enterococcus faecalis.

7.A Case of Sclerosing Panniculitis.

Sclerosing panniculitis refers to the skin induration and hyperpigmentation of the legs that often occurs in patients who have venous insufficiency. Sclerosing panniculitis is similar to those described under the designations hypodermitis sclerodermiformis or lipodermatosclerosis. Histologically the septa of the subcutis become thickening by inflammation and fibrosis. Adipocytes coalesce to form small and large pseudocysts. Some of these are lined by refractile, amphophilic crenulated membranes. This alteration was referred to as lipomembranous change. We report a case of sclerosing panniculitis in a 40-year-old female patient presented as induration and depression of the skin and subcutaneous tissues of the lower third of the leg.

9.Laparoscopic Treatment of Symptomatic Nonparasitic Liver Cysts.

Nonparasitic liver cysts are usually asymptomatic and require treatment when they are symptomatic. Hence, many treatment methods, such as percutaneous aspiration, aspiration followed by injection of sclerosing agents into the cyst, excision of the liver cyst, and hepatic resection have been suggested. Recently, the laparoscopic cholecystectomy has become popular, and this method has been challenged as a treatment for symptomatic nonparasitic liver cysts. To evaluate the feasibility of using and the disadvantages of laparoscopic treatment, we reviewed the medical records of 10 patients with a symptomatic nonparasitic liver cyst who were treated by laparoscopic unroofing at the Department of Surgery, Chungbuk National University Hospital. All patients were diagnosed by USG and abdominal CT. All patients had vague abominal discomfort, abdominal distension, or indigestion. The sizes of the liver cysts varied from 7 cm to 20 cm, and half of them were located in the right lobe, the other half in the left lobe. In 5 patients, the cysts were multiple. The mean operative time was 99.5 minutes, and mean hospital stay was 8 days. The follow up period was from 3 to 37 months. Two patients required a reoperation because of cyst infection and a rapidly growing cyst. Remaining cysts were identified in 4 among 8 patients, but they did not have any symptoms. In conclusion, laparoscopic unroofing is feasible as a first choice for treatment of a symptomatic liver cyst. However, in liver cyst that are located at the dome of right side or in a thickened wall, incomplete unroofing and residual cysts can be anticipated. In this case, we suggest that open cyst excision or unroofing may be better than laparoscopic unroofing.
