5.Neovascularization of the Optic Disc Associated with Acute Ophthalmic Artery Obstruction.

PURPOSE: It has been reported that neovascularization of the optic disc (NVD) occurs in ocular ischemic syndrome, sickle cell hemoglobinopathy, ocular sarcoidosis, Eales' disease, and central retinal artery occlusion. However, NVD in acute ophthalmic artery occlusion had not been reported. Therefore, we report one unusual case of NVD in acute ophthalmic artery obstruction. METHODS: A 56 year-old man with complaint of abrupt decrease of visual acuity and periorbital discomfort underwent medical history taking, periorbital examination, fundus examination, and fluorescein angiography. He was followed up for 5 months RESULTS: Fundus examination showed segmentation of blood flow and diffuse retinal whitish opacity on the posterior pole around the center of the macula, without any cherry red spots in the perifoveal area. Fluorescein angiography showed a marked filling delay in the nasal retinal and choroidal circulations with a definite watershed zone involving the peripapillary choroid and macula. The retinal arteries filling and arteriovenous transit time were delayed. There was no light-perception vision and low intraocular pressure. Diabetes was found on systemic examination. Five weeks later, NVD and choroidal atrophy around the optic disc were detected. Panretinal photocoagulation was performed for the regression of NVD. NVD was regressed, but recurred.

8.A Case of Neurofibromatosis with Bilateral Pheochromocytoma.

Pheochromocytoma is originated from chromaffin cell of sympathetic nervous system and associated with other disease, such as neurofibromatosis, duodenal carcinoid, medullary thyroid cancer and parathyroid adenoma. Especially, pheochromocytoma is developed more than 50% in neurofibromatosis associated with hypertension. In such cases, several clinical features documented as more frequent bilateral phochromocytoma, more associated with other neuroendocrine tumors and thus more poor prognosis. We can observe the sustained hypertension despite of surgical resection of tumors in pheochromocytoma cases. One of the possible reason of post operative sustained hypertension is the pheochromocytoma originated from minor organ of Zukerkandl that was not resected during operation. Untreated or delayed treated cases with pheochromocytoma were often expired by complication of hypertension such as cerebrovascular hemorrhage, myocardial infarction, etc. Thus, in neurofibromatosis with hypertension, screening of pheochromocytorna is very important for the early detection of tumor and more favorable prognosis. Recently, We experienced a case of neurofibromatosis associated with bilateral pheochromocy-toma expired by cerebral hemorrhage during operation, so we report the case with literature review.

9.Effect of application methods of a self-etching primer adhesive system on enamel bond strength.

The purpose of this study was to evaluate the effect of passive or active application of primer and coat times of bond on the shear bond strength when a self-etching primer adhesive (Clearfil SE Bond) was applied to enamel surface. Crowns of sixteen human molars were selected. Buccal and lingual enamels of crowns were partially exposed and slabs of 1.2 mm thick were made. They were divided into one of four equal groups (n = 8). Group 1: passive application of Primer and 1 coat of Bond, Group 2: active application of Primer and 1 coat of Bond, Group 3: passive application of Primer and 2 coats of Bond, Group 4: active application of Primer and 2 coats of Bond. Clearfil AP-X was bonded to enamel suface of each group using Tygon tubes. The bonded specimens were subjected to microshear bond strength (uSBS) testing with a crosshead speed of 1 mm/min. The results of this study were as follows; 1. The uSBS of Group 1 was the lowest among groups and the uSBS of Group 4 was the highest. 2. There was not statistically significant interaction between enamel uSBS by application method of Primer and coat time of Bond (p > 0.05). 3. There was not statistically significant difference between enamel uSBS by passive and active application of Primer (p > 0.05). 4. There was statistically significant difference between enamel uSBS by one- and two-coat of Bond (p < 0.05).

10.Relation between Coronary Artery Cross Sectional Area and Left Ventricular Wall Mass.

The coronary artery cross sectional area (CSA) is proportional to LV mass. We have measured the cross sectional area of the left and right coronary arteries in patients with ischemic heart disease to see whether it is related to the change in the LV mass. The following results were obtained ; 1) There were no significant difference in mean CSA of coronary arteries and LV mass determined by echocardiography and cineangiography between control and ischemic heart disease. 2) There were significantly increased ratio of left ventricular mass by cineangiogram to CSA of left anterior descending coronary artery in patients with myocardial infarction as compared with control group. 3) A linear relation between LV mass by cineangiogram and CSA of left coronary artery was noted in control group (r=0.53, P<0.05) and ischemic heart disease group (r=0.51, P<0.05). 4) A linear relation between LV mass determined by echocardiography and CSA of left coronary artery was noted in control group (r=0.55, P<0.05).
