1.CT Findings of Focal Organizing Pneumonia: Correlation with Pathologic Findings.

PURPOSE: To evaluate the CT findings of focal organizing pneumonia and to correlate them with pathologic findings to help differentiating from lung cancer. MATERIALS AND METHODS: We evaluated radiologic and pathologic findings of five patients with solitary pulmonary nodule which were confirmed as focal organizing pneumonia pathologically. RESULTS: On CT scan, focal organizing pneumonia had irregular margin contacting the pleura in all five cases. The shape of the nodules were spherical to wedge or elliptical and the size from 3.5cm to 5.5cm (average 4.2cm) in largest diameter. On postcontrast CT scan, all nodules showed enhancement and four cases showed central low density components. Two nodules contained air within the nodule. In four cases, pleural changes such as effusion and/or focal thickening were noted. No lymphadenopathy was found in all cases. Pathologically, the enhancing portion on CT showed findings of organizing pneumonia such as granulation tissue with fibroblast proliferation in alveolar space and interstitial thickening. The central low density areas on CT were due to ischemic necrosis, abscess and exudate, transudate and infiltration of foamy histiocyte. CONCLUSION: The possibility of focal organizing pneumonia should be considered when peripherally located solitary pulmonary nodule had enhancing component with no combined lymphadenopathy on CT scan.

2.Computed tomography of lacrimal fossa tumors

The lacrimal fossa can be invovled by a wide specturm of orbital pathology. The correct diagnosis is importantto avoid unnecessary procedure and to do appropriate management. 14 patients with mass lesions in the lacrimalfossa were evaluated with CT and clinical findings. The results were as follows: 1. Final diagnosis of 14 caseswith lacrimal fossa tumors was pleomorphic adenoma in 3 cases, adenoid cystic carcinoma in 1 case, pseudotumor incases, lymphoma in 2 cases, neurofibroma in 1 case, chloroma in 1 case and metastatic adenocarcinoma in 1 case. 2.The duration of symptoms of pleumorphic adenoma was more than 1 year and characteristic CT findings were globularmasses with pressure erosion of the adjacent bone. Patient wih adenoid cystic carcinoma had a short history ofsymptoms. CT showed a fusiform mass but intracranial extension with frank destruction of sphenoid bone. 3.Patients wtih pseudotumor and lymphoma had symptoms for less than 1 year. The CT findings were ill-definedinfiltrative patterns with scleral thickening and the differential diagnosis of them was difficult. 4. The marginsof neurofibroma and chloroma were well defined while that of the metastatic adenocarcinoma was ill-defined. 5. Thedegree and the extent of the contrast enhancement gave no benefit in the differential diagnosis of each diseaseentities and even of the benign and malignant lesions.

4.The Treatment of Tibial Shaft Fractures by Ender's Nailing

Between July 1982 and July 1985, we treated thirty-one fractures of the tibial shaft by intramedullar ary Ender's nailing at the Department of orthopedic Surgery of Pusan Paik Hospital. Reductions of the fractures were achieved by closed method in 25 cases and open method in 6 cases. The results were as follows. 1. The average age was 34. 5 years and the most common cause of injury was traffic accident(67.7%) 2. Nineteen cases (61%) were closed fracture and 12 cases (39%) open. Seventeen cases (55%) were simple fracture and 14 cases (45%) comminuted. The most common level of the fracture was middle third of the tibial shaft. 3. Twenty cases (64.5%) had associated injuries and average interval from injury to operation was 11. 8 days. 4. The average healing time was 16.7 weeks and the time prolonged in comminuted fractures than in simple fractures. 5. Complications were one case of valgus deformity (11°), one case of delayed union with pseudoane urysm of anterior tibial artery on fracture site and one case of infected nonunion in Grade III open comminuted fracture. 6. Ender nails are relative ease of nailing, allow early weight bearing and decrease complication. Ender nailing is considered to be a good method in treating tibial shaft fracture especially for management of multiple injured patient and when other procedures have failed.

5.Reconstruction of Posterior Cruciate Ligament Using Patella Tendon Graft

It is generally accepted that posterior cruciate ligament is the prime stabilizer and perhaps the most importent ligament in the knee. We agreed that primary repair or reconstruction of ruptured posterior cruciate ligament is essential. Recently Clancy reported good results to correct knee instability due to rupture of the posterior cruciate ligament with a procedure that employes a free graft of one-third of the patella tendon with its tibial and patella attachments. The free patella tendon graft has been shown to revascularize readly, develop resonably high tensile strengh, allow bone to bone healing, have excellent static stability and not to deprive the knee of a significant stabilizer. So, we analized the 10 cases of the posterior cruciate ligament injuries who were admitted and treated reconstruction using free patella tendon graft in Busan Paik Hospital between May, 1984 to Dec, 1986 and obtained the following results. 1. In 8 cases, obtained satisfactory results. In 2 cases, obtained unsatisfactory results due to combined other compartment injuries, but improved posterior stability than preoperative status. 2. Reconstruction of posterior crucitae ligament using free patella tendon graft was excellent procedure for regaining of static stability of the knee.

7.Flexor Tendon Graft

Flexor tendon grafting is a well-proved procedure for the restoration of tendon defects in the hand. The results are not entirely predictable, it offers a good expectation of successful outcome. Authors have performed the tendon graft in thirteen cases of flexor tendon injury of the hand during the period from September '79 to January '82 and evaluated the results. The followings were the results obtained. 1. Male was eleven, female was two. Age distribution is from 4 to 38 years. 2. Injured fingers were thumb 6, index 2, middle finger 4 and little finger 1 cases. The site of the primary injury was on area of Bunnell's Zone II in all cases. 3. Time interval from tendon injury to flexor tendon graft was 6 weeks to 19 months (average 5.6 months). 4. The results of tendon grafts rated as excellent 2 cases and good 3 cases in preoperative Good group; excellent 1, good 3 and fair 1 in “Scar” group; fair 1 and 1 poor in “Joint” group; fair 1 in “Salvage” group. In the thumbs, the results were better than in the fingers. 5. Degrading the postoperative results were flexion contracture in 3 cases, lack of flexion ROM in 2 cases and bowstringing in 2 cases. 6. Pinch power of the finger where tendon graft was performed was 52% of that of the opposite healthy finger. 7. As complication, bowstringing was observed in 2 cases and recurvatum deformity of the PIP joint in one case.

8.Surgical Treatment In Pott's Paraplegia

Pott's paraplegia has been frequently serious complication of tuberculous spondylitis, although antituberculous drugs have improved the prognosis and healing of the disease. There are much controvercy on treatement of Pott's paraplegia, which comprises immobilization with or without anterolateral antituberculous drugs, immobilization and early posterior arthrodesis, anterolatear decompression and radical anterior decompression with anterior spinal fusion. The method of anterior decompression and anterior interbody fusion which was introduced by Hdgson since 1956 has good reults. The author analyzed 27 consecutive cases of Pott's parsplegia operated at orthopedic departement, In Je Paik Hospital from May 1980 to August 1988. The results were as follows, 1. Number of cases in child was nine(33%), both sexes were similar. 2. Regions of spine involved were most common at thoracic spine(62%). Number of affected vertebrae was averaged 3.3 vertebrae. 3. There were nineteen early(70%) and eight late(30%) paraplegia. 4. There were two complete and twenty five incomplete paraplegia. 5. In operative finding, inflammatory products and sequestrated material was extrinsic factor in early in paraplegia. 6. The recovery of the paraplegia occur in twenty cases(74%) 7. The recovery of paraplegia after treatment was far better in type of early onset, type of incomplete paraplegia less than one year duration. 8. The radical anterior decompression and anterior interbody fusion was considered as recommendable method for remove of all pathologic foci, recovery of paraplegia and healing the tuberculosis with stability.

9.Clinical Study of Borderline Malignant Tumor of Ovary.

Borderline malignant ovarian tumor are characterized as neoplasrns exhibiting cellular priliferative changes greater than those seen in the benign form, but not showing destnictive invasion of the ovarian strama. We reviewed 14 cases of borderline ovarian tumor who had been diagnosed and treated at Kyung Pook university hospital from 1987 to 1992. We nnalysed the clinical features, treatment and prognosis which were compared to 24 cases of the epithelial ovarian cancer at the same period The most common histological thpe of boderline tumnr was mucinous type (1 l cases, 79%), the next one was serous type (3 cases, 21%). In FIGO stage of Borderline tumor, 11 caseS weve stage I a, 1 with stage II c, 1 with stage gb and 1 with stage lV, and the FIGO stage of 24 cases of ovarian cancer was 8 cases with stage I a, 1 with stage I b, 2 with stage I c, 1 with II c, 2 with stage III a, 1 with stage III b, 7 with stage III c and 2 with stage 1V. The mean age of borderline and malignant tumor was 37.1 and 47.6 years.. 3 cases were being pregnant on the operatian. In the 14 cases of borderline turnor, 7 cases were managed by conservative operation, but 1 case recurred 4 years later so extirpative operation and pastoperative chemotherapy were given. The other 4 cases were managed by extirpative operation but postoperative chemotherapy was given in only 1 patient. Follow up ranged from 7 to 72 months. 12 cases were alive. 2 cases died, one was stage III b, the other was atage IV. This atudy suggeat, that it is neceasary to follow up carefully even the early stage of botderline tumor and manage as oyarian carcinama in the advanced stage.
