2.Effect of biscover on the marginal microleakage of composite resin restoration.

The purpose of this study was to compare the effect on marginal leakage of a resin surface sealant (Biscover) applied before or after polymerization of composite resin to unsealed composite restorations. Thirty Class V cavities with the occlusal margin in enamel and cervical margin in dentin or cementum were prepared on the buccal surfaces of sound extracted molars and restored with a microfilled light-cured composite resin (Micronew). Restorations were randomly assigned into one of three equal groups (n = 10): a control group - no surface sealing, group 1 - applied Biscover after polymerization of the composite resin, and group 2 - applied Biscover before polymerization of the composite resin. Specimens were thermocycled, immersed in a 2% methylene blue solution for 4 hours, sectioned longitudinally, and analyzed for leakage at the occlusal and gingival margins. The results of this study were as follows; 1. In sealed group, group 2 showed higher microleakage than group 1 at both occlusal and gingival margins, but there was no significant difference between two groups (p > 0.05). 2. Unsealed control group showed a little higher microleakage than sealed group at occlusal margins, and a little higher or similar microleakage than sealed group at gingival margins (p > 0.05). 3. Control group and group 2 showed significantly less microleakage at the occlusal margins, but group 1 showed no significantly difference between microleakage at the occlusal and gingival margins.

3.A Case of Esophageal Obstruction Complicated in a Patient with Chronic Granulomatous Disease: Esophageal obstruction in Chronic Granulomatous Disease.

Chronic granulomatous disease (CGD) is an inherited immunodeficient disease characterized by recurrent infections and granuloma formation. Granulomatous obstruction of esophagus is one of the rare complications of CGD. The use of steroids and antimicrobials for esophageal obstruction by granuloma in CGD patients has been controversial due to the possibility of concomitant inapparent infection. We report a case of esophageal obstruction in an 8-year-old CGD patient showing the poor response to antibiotics therapy. However, dramatic improvement of symptoms and radiologic findings of esophageal obstruction were achieved after steroid therapy. One month after discontinuation of steroid, esophageal obstruction recurred and the patient was re-treated with steroid. After that time, he experienced one more recurrence of esophageal obstruction. This symptom subsided after antibiotics therapy without steroid and he has been followed up to the present without further relapse.

4.A Clinical Review of the Pyeloplasty.

A total of 33 patients underwent repair of congenital ureteropelvic junction obstruction during a 10-year period. A majority of patients (85%) presented with flank pain. Hydronephrosis was demonstrated in all patients. A majority of the patients (94%) underwent dismembered pyeloplasty with intubation. In 9 cases, pyelolithotomy was performed at the time of pyeloplasty. One hypertensive patient became normotensive postoperatively. Seven patients underwent secondary procedures after a previous failed pyeloplasty, including 3 cases of dismembered pyeloplasty, 3 cases of ileal ureteral replacement and a case of nephrectomy. The clinical success rate was 79% with one operation.

6.5 Cases of Pressure Alopecia after Tympanomastoidectomy.

Pressure alopecia can occur after prolonged pressure on the scalp with the head fixed in one position. We have repor ted 5 cases of pressure alopecia, occurring over the contralateral temporoparietal 1 area after tympanomastoidectomy due to chronic ot.itis media. Total times of operations were from 4 hours to 7 hours. Four of the 5 patients experienced scalp pain during the early postoperative period and one of them showed large bulla formation. Loss of hairs followed between the 14th and 36th postoperative day, and new hairs began to grow 2 or 3 months after the operation.

7.Appropriateness Ratings in Cataract Surgery.

This study aimed to evaluate the appropriateness of cataract surgery and identify the characteristics of patients and surgeons, clinical and functional outcomes, and surgical methods associated with appropriate cataract surgery. For this purpose, Korean cases of cataract surgery were rated as either 'necessity', 'appropriate', 'uncertain' or 'inappropriate', based on RAND/UCLA Ratings. For this assessment, the cases of 222 patients who underwent cataract surgery, on either one or both eyes, were studied. The surgeries were performed by 20 ophthalmologists practicing at one of fourteen medical institutions (university hospitals and general hospitals). Patients were interviewed and clinical data collected. The Doctors were questioned with self-entered questionnaire forms. The medical records were also examined to gain an understanding of the surgical process. The ratings were as follows: 30.6% (68 patients) of surgeries belonged to the bracket "necessity", 46.4% (103 patients) to "appropriate", 15.3% (34 patients) to "uncertain" and 7.7% (17 patients) to "inappropriate". In this study, "necessity" and "appropriate" were defined as "appropriate" (77.0%, 171 patients), and "uncertain" and "inappropriate" as "inappropriate" (23.0%, 51 patients). The low preoperative Snellen visual acuity and visual function, advanced age and male patients were associated with appropriate surgery. It is concluded that appropriate surgery was related to the clinical and functional outcomes (visual acuity and visual function) and patient characteristics (age and male).

8.A Mycological Study on Dermatophytoses in Seoul During the Peroid 1985 - 1991.

BACKGROUND: Dermatophytoses show marked differences in their incidences, distributions, and causative organisms by age, sex, occupations, environmental conditions and geographic distributions. OBJECTIVE: We evaluated the recent distributions of dermatophytes and compared them with the previous reports in Seoul. METHODS: We evaluated the incidences of dermatophytes according to the patient's age and sex, isolated month, and the infection sites with 387 strains of dermatophytes isolated from the various forms of dermatophytoses during the period from August 1985 to July 1991 in Seoul. RESULTS: T. rubrum was the most prevalent dermatophyte in all age groups except the first decade. The incidence of M. canis was high in age groups below 20 years old relatively high incidences of T. mentagrophytes and T. interdigitale in age groups over 30 years old. Dermatophytes were frequently isolated in summer season, but the incidence of M. canis was relatively high in October and December. T. rubrum was the most prevalent dermatophyte from all forms of dermatophytoses except tinea capitis, but the incidence of T. rubrum was relatively low in tinea corporis. The incidence of T. mentagrophytes was relatively high in tinea unguium and tinea manus, and M. canis was isolated only in tinea corporis and tinea capitis. From the cases showing more than two forms of dermatophytoses. T. rubrum was isolated for the most part. CONCLUSION: This study suggested that there was a rise in the cases of t. rubrum infections in Seoul and in recent years the prevalence of M. canis infections has markedly increased but those of E. floccosum and t. mentagrophytes have decreased.

10.A Case of Congenital Leukemia Cutis.

Congenital leukemia is a rare disease developing within the first 4 to 6 weeks of life, whose natural history is quite different from that of childhood and adult leukemia. Leukemia cutis occurs in 25% to 30% of infants with congenital leukemia and is more frequently associated with acute myeloid leukemia than acute lymphocytic leukemia. We report a case of congenital leukemia cutis in which a walnut-sized, slightly erythematous, dome-shaped tumor that had developed at left frontal area since birth was confirmed by skin biopsy. A review and analysis of Korean literature pertaining specially to leukemia cutis in congenital leukemia is presented along with the report of an additional case.
