1.Ki-1 Positive T-Cell Lymphoma of Bone in a Child.

Ki-1 monoclonal antibody is a well known marker for Reed-Sternberg cells in Hodgkin's disease, but also occasionally reacts with activated lymphoid cells of either benign or malignant nature. Recently, Ki-1 antibody positive Non-Hodgkin's lymphoma, usually of large cell and/or polymorphous type, has been reported in the lymph nodes, skin, soft tissue, and stomach, but not in the bone. We report a case of multifocal primary bone lymphoma in a seven-year old body involving the left shoulder and right frontal bone, which proved to be a large cell, polymorphous lymphoma, helper T-cell type expressing Ki-1 antigen.

2.Primary T-cell Lymphoma of the Thyroid Associated with Hashimoto's Thyroiditis, Histologically Mimicking MALT-Lymphoma.

Most of thyroid lymphomas are B-lineage, and T-cell lymphomas are rare. Here, we report a case of primary thyroid T-cell lymphoma associated with Hashimoto's thyroiditis. A 48-yr-old woman presented with incidentally found neck mass. Histologically, the resected right lobe of the thyroid was replaced by monomorphic small atypical lymphoid cells with lymphoepithelial lesion-like change, most of which were immunoreactive for CD3, CD8, betaF-1, and TIA-1. Peripheral T-cell lymphoma, unspecified, was finally diagnosed after molecular study for TCR-gamma gene rearrangement. This is the second case of cytotoxic T-cell lymphoma reported in the thyroid gland so far. Unique association between thyroid follicles and neoplastic lymphocytes may be characteristic feature of this type of T-cell lymphoma.

4.Arthroscopic Findings in Acute Shoulder Dislocation associated with a Fracture of Greater Tuberosity of the Humerus.

PURPOSE: To evaluate arthroscopic findings of intraarticular lesions in patients with initial dislocation of the shoulder associated with a fracture of greater tuberosity and differences in lesions according to age. MATERIALS AND METHODS: The subjects were 13 patients who had received an arthroscopic evaluation from February 1997 to August 1998. Using 40 years of age after which rotator cuff tear frequently occurs at the time of traumatic dislocation as a base, we classified these cases into two groups: Group A (6 cases) under 40 years of age and group B (7 cases) over 40 years of age. As for arthroscopic findings, we investigated 8 items, including SLAP lesion, anterior labral tear above the mid-glenoid notch, capsular tear, Bankart lesion, Hill-Sachs lesion, glenoid cartilage fracture, rotator interval and cuff tear. RESULTS: Bankart lesion was observed in 50% of group A, but group B showed no Bankart lesion, but capsular tear without labral lesion was present in 71%, and rotator interval and cuff tear were observed in 29%. CONCLUSION: Many pathological findings were found with arthroscopy, and important pathological differences were observed between the two groups.

5.Adverse Effects of Air Pollution on Pulmonary Diseases

Environmental exposure to air pollution is known to have adverse effects on various organs. Air pollution has greater effects on the pulmonary system as the lungs are directly exposed to contaminants in the air. Here, we review the associations of air pollution with the development, morbidity, and mortality of pulmonary diseases. Short- and long-term exposure to air pollution have been shown to increase mortality risk even at concentrations below the current national guidelines. Ambient air pollution has been shown to be associated with lung cancer. Particularly long-term exposure to particulate matter with a diameter <2.5 μm (PM2.5) has been reported to be associated with lung cancer even at low concentrations. In addition, exposure to air pollution has been shown to increase the incidence risk of chronic obstructive pulmonary disease (COPD) and has been correlated with exacerbation and mortality of COPD. Air pollution has also been linked to exacerbation, mortality, and development of asthma. Exposure to nitrogen dioxide (NO2) has been demonstrated to be related to increased mortality in patients with idiopathic pulmonary fibrosis. Additionally, air pollution increases the incidence of infectious diseases, such as pneumonia, bronchitis, and tuberculosis. Furthermore, emerging evidence supports a link between air pollution and coronavirus disease 2019 transmission, susceptibility, severity and mortality. In conclusion, the stringency of air quality guidelines should be increased and further therapeutic trials are required in patients at high risk of adverse health effects of air pollution.

Failure of normal embryogenesis may result in various anomalies of the inferior vena cava that can bereli-ably detected by computed tomography. Agenesis of the intrahepatic segment of the inferior vena cava withazygos continuation is a rare anomaly due to a complex developmental process of the inferior vena cava. Theauthors report a case in which this anomaly was diagnosed on computed tomography and confirmed by inferi-or venacavography.

7.A case of placenta previa percreta with bladder invasion.

Placenta percreta with bladder invasion is an extremely rare complication causing life-threatening hemorrhage, up to 17 liters.1 Most reported cases have been diagnosed after 30 weeks of pregnancy and few have presented before 20 weeks.2 Only 20 cases of placenta percreta with invasion of the bladder have been reported during last decade. In this condition, chorionic villi attach directly to the myometrium without intervening decidua. The exact etiology of impaired decidualization is unknown; however, a previous endometrial insult, such as previous cesarian section or endometrial curettage may be a contributing factor. To avoid massive bleeding, uterine body incision, hypogastric artery ligation before hysterectomy and uterine cervical removal have been attempted.1 Chorionic villi may attach to the myometrium (accreta), invade the myometrium(increta), or in its severe form (percreta) the trophoblastic tissue penetrates the whole myometrium and invades adjacent structures. We experienced a case of placenta previa percreta with bladder invasion and review the literatures.

Kikuchi's disease (KD), histiocytic necrotizing lymphadenitis, is a rare self-limited lymphadenopathy, which usually affects young women. KD has been reported to precede, coexist with or follow the diagnosis of other entities, such as systemic lupus erythematosus (SLE), adult-onset Still's disease, Hashimoto's disease, and viral infections. In a few cases, KD is associated with cutaneous lupus erythematosus (CLE), without systemic involvement. Herein, we report the first Korean case of KD associated with lupus erythematous profundus in a 9-year-old boy.

10.A Case of Hydrogen Peroxidese Induced Proctitis..

Hydrogen peroxide is commonly used for cleansing superficial wounds, because of its antimicrobial action attributed to oxidizing capacity. The release of oxygen doping oxidation results in effervescence, which promotes a weak mechanical means of removing tissue debris. In the past, hydrogen peroxide was used for the management of meconium ileum and for fecal impaction because liberated oxygen tend to break up impaction and initiate peristaltic reflex. However, the stronger solution has a caustic action. The potential dangers of hydrogen peroxide enema were not so well known until 1950s when Pumphery recognized deleterious effects from such enema. During 1980s, hydrogen peroxide was used for disinfection of endoscope and several authors reported peroxide-induced colitis. We report a case of hydrogen peroxide induced chemical colitis in a 47-year-old man, who complained bloody stools and tenesmus after introducing hydrogen peroxide soaked gauss into the rectum to relieve pruritus am. Colonoscopy demonstrated diffuse erythema, edema and friability of rectal mucosa upto 6-7 cm from anal verge. The proximal margin of the lesion was well demarcated from normal appearing mucosa. Mucosal biopsy revealed histologic findings compatible with ischemic changes. His symptoms improved rapidly with conservative treatment.
