5.The Effects of Beta-blocker and Acetazolamide on Ascorbic acid Concentration in Aqueous Humor.

Aqueous humor resembles the serum in composition; its concentration of ascorbic acid, however, measuring to be 15 to 69 times higher (than that of serum) has been presumed to be one of the possible predisposing factors of developing glaucoma. The inhibitor of aqueous humor production, one of the current therapeutic measures of glaucoma has been reported to increase its concentration of ascorbic acid. The effects of Beta-blocker and Acetazolamide on the ascorbic acid concentration are evaluated in this study and further, 50 eyes of 50 cataract patients and 20 eyes of 20 glaucoma patients as the objects, the ascorbic acid concentration in aqueous humor is compared between these two groups under the same pre-set conditions of using the inhibitors. Beta-blocker and acetazolamide elevated the concentration of ascorbic acid by 7.5% and 10.7%, respectively and the combined administration of both resulted in a significant increase of 20.4% (p<0.05). The cataract and glaucoma patients under both medications showed concentrations of 284.7 microgram/ml and 267.3 microgram/ml, each, though, such a slight difference of figure implies no statistical significance. The ascorbic acid concentration in serum and its ratio of composition in aqueous humor and serum does not differ between the two groups. It's concentration in aqueous humor, however, showed a considerable individual variation which was especially remarkable in glaucoma patients. As the conclusion of above evaluations, either ascorbic acid has almost no effect on the pathogenesis of glaucoma or has a similar degree of influence on the pathogenesis of cataract and glaucoma with different mechanisms.

7.Pathologic Analysis of 39 Cases of Epilepsy Surgery.

Pharmacologic therapy is still the primary management for epilpsy; however, surgical treatment is a reasonable therapeutic option for patients suffering from medically intractable seizures, especially temporal lobe epilepsy having a documented unilateral epileptogenic area. Thirty nine patients with pharmaco-resistant complex partial seizures underwent anterior temporal lobectomy and hippocampectomy in 38 cases and frontal cortisectomy in one case. On pathological examination, hippocampal sclerosis was a predominent pathologic finding and was identified in 18 cases. Other non-neoplastic lesions consisted of 5 cases of vascular lesions(2 cavernous angiomas, 2 arteriovenous malformations and 1 angiomatosis), 3 cases of fibrous nodule, 2 cases of cicatrical changes of cerebral cortex, and 1 case of parasitic infection. Neoplasms including two cases of oligodendroglioma and one case of anaplastic astrocytoma were also noted. In seven cases, there was no detectable lesion on gross and microscopic examination. On post-operative follow-up, seizures were completely terminated in most cases(31 cases, 79%). The rest of the patients also displayed marked alleviation of symptoms. The seizures tended to recur more aften among the patients with neoplasm or no pathologically detectable lesion. In order to detect any minute pathological lesion, thorough gross and microcsopic examinations are considered to be essential.
8.Serous Cystadenoma of the Pancreas.

Pathologists and others have been calling the serous cystadenoma of the pancreas "microcystic cystadenoma", following Compagno and Oertel's proposal in l978 because it usually consists of innumerable small cysts (less than 2cm). However, unilocular or predominantly macrocystic types of serous cystadenoma have been occasionally reported. Therefore, they present a difficulty in precise preoperative and intraoperative diagnosis for their simi1ar gross appearance to other cystic lesions of the pancreas. We discovered two cases of macrocystic and unilocular serous cystadenomas of the pancreas which were lined by cuboidal to flattened epithelial cells. They contained PAS positive and D-PAS negative intracytoplasmic glycogen granules. The unilocular, macrocystic and microcystic patterns may represent a morphologic spectrum of a pancreatic neoplasm. So we propose to use the term "serous cystadenoma" rather than microcystic cystadenoma.

9.The Effects of LASIK-Related Negative Pressure on the Microcirulation of Optic Nerve and Retina.

It is known that intraocular pressure becomes elevated over 65 mmHg when using a suction ring or fixation ring during LASIK. We intended to find out if transient elevation of IOP alters the microcirculation of the optic disc and retina. 10 patients were enrolled in this study. LASIK was performed on one eye using a suction ring and fixation ring. The other eye was used as a control. Heidelberg Retina Flowmetry[HRF] was performed preoperatively, postoperatively 1 day, 1 week, 3 weeks, and 3 months, respectively and analysis was done at the nasal and temporal neuroretinal rim, peripapillary retina, and optic cup. There was a statistically significant increase of flow at all sites on postoperative 1 day. Furthermore, increased flow was continued until postoperative 3 weeks at the temporal neuroretinal rim. However, the flow was normalized to the preoperative level by 3 months in all measured areas. No differences of flow were found when compared to the control eyes in each site.

10.Breast Feeding and Lower Respiratory Tract Illness in the First Year of Life.

Breast feeding is one of many factors that have been associated with the development of lower respiratory tract illnesses. To assess the relation between breast feeding and subsequent experience of lower respiratory tract illness, we performed this study, retrospectively, on 366 infants who had been admitted to the pediatric ward, St. Francisco General Hospital, or had been brought to the well baby clinic from November, 1992 to March, 1993. The results were as foolows: 1) Among 366 infants, the number of all first illnesses observed at each interval were 259 cases (70.8%), of which 198 (54.1%) were associated with wheezing illnesses and 61 (16.7%) with non-wheezing lower respiratory illnesses. 2) Duration of breast feeding regardless of-whether other foods were given, was categorized as 3 grups, i. e., none or for less than a month (group), one to 4 months (group B), and 4 or more months (group C). Incidences of wheezing illnesses in the first 4 months of life were lower in group B(10.1%) and group C (8.4%) than group A (27.3%), respectively (p<0.01). Wheezing illnesses beyond 4 months of life and non-wheezing illnesses in all intervals seemed to be unrelated to the duration of breast feeding. 3) Considering a variety of other fisk factors, including sharing a room, parental childhood reapiratory trouble, sex and maternal education, children who received minimal breast feeding (group A) had the greater risk of early wheezing illnesses in the simultaneous exposure to sharing a room than group B and C (p<0.005). Our results suggest that decreased breast feeding is associated with a higher incidence of wheezing illnesses in the first 4 months of life, particularly in combination with other risk factors, such as sharing a room.
