5.The Establishment of Hospice and Palliative Care System from the Cancer Patients and Families' Point of View.

BACKGROUND: Many terminal cancer patients and families are affected with physical, emotional, and social problems. Many people claim that a type of medical services is needed to manage them such as hospice palliative care. There have not been many studies of cancer patients and families with respect to their opinions and attitudes on hospice palliative care for terminal cancer patients, although their views on it is important. METHODS: We surveyed 687 in-patients, out-patients and their families with cancer in 8 hospitals. The self-administered questionnaires included the following; 1) socio-demographic and clinical variables; 2) opinions on hospice palliative care; 3) attitudes on ethical issues associated with hospice palliative care; 4) factors associated with withholding futile care at the end-of-life. The data were analyzed with x2-test, Mantel-Haenszel x2-test, and multiple logistic regression. RESULTS: Almost 90% of the subjects agreed to the need of obtaining hospice palliative care with health care insurances and reaching a social consensus on the contents of its programs. Five hundred and seventy six (83.8%) subjects agreed to the need of using advanced directives. Two hundred and eighty five (72.2%) cancer patients and 200 (68.5%) families agreed to the need of withholding futile care at the end-of-life and of people accepting their cancer diagnosis with insight and living in the metropolis as they were more likely to do. In the multivariate analysis, the attitudes on withholding futile care at the end-of-life was significantly different only by insight of cancer diagnosis (OR; 1.09-3.15). CONCLUSION: This study showed that hospice palliative care should be established through social consensus on the issues related to ethics and insurances, and that cancer patients and families must have a right to choose such services with informed decision-making.

7.Surgical Treatment of Multiple Rice Bodies in Chronic Subacromial and Subdeltoid Bursitis: A Case Report.

Multiple rice body formation is a complication of chronic bursitis frequently associated with seronegative rheumatoid arthritis or tuberculosis. It resembles synovial chondromatosis on imaging and clinically. We report on a pathologically diagnosed multiple rice body formation in subacromial and subdeltoid bursitis in a 44-year-old man who was treated by surgical removal and bursectomy. At 16 months after the removal, range of motion of affected shoulder was normal. No evidence of recurrence of rice body in plain X-ray and ultrasonography. Multiple rice body formed in chronic subacromial and subdeltoid bursitis could be treated with surgical removal and bursectomy successfully.

8.Treatment of Urethral Obstruction after Wallstent Implantation in Traumatic Urethral Stricture.

Endoscopic intraluminal implantation of a self expandable stent has been increasing in patients with recurrent urethral strictures because of its simple and relative safe technique. There has been a few reports concerning luminal obstruction after implantation of Wallstent. We have experienced 4 cases of urethral obstruction after variable period of Wallstent implantation in the treatment of traumatic urethral stricture. Herein the methods for treatment of urethral obstruction are discussed. The patients were managed with two different types of treatment. The first one was transurethral resection of granulation tissue inside Wallstent. The other is reinsertion of Wallstent inside the first one after transluminal resection of granulation tissue in patients with urethral obstruction occurring within 5 months after Wallstent implantation. Retrograde urethrography and urethroscopy after 3 months have demonstrated patency of urethra and epithelial covering of the implant. Our experience shows that in all 4 patients with traumatic urethral stricture have developed variable degrees of urethral obstruction after a variable period of wallstent implantation. Therefore, we consider that our method is one of the alternatives to treat urethral obstruction after wallstent implantation in patients with traumatic urethral stricture.

9.Comparision in the yield of fetal nucleated red blood cell between the first-and second-trimester using double density gradient centrifugation.

OBJECTIVE: The aim of our study was to make a practical comparative evaluation of the first and second trimesters in order to determine the period during which a higher yield of fetal nucleated red blood cells (FNRBCs) can be obtained. METHODS: NRBCs were isolated from maternal blood during the first and second trimesters of pregnancy using double Percoll gradients with different osmolarities. Magnetic activated cell sorting was performed with Kleihauer-Betke stain. We isolated fetal NRBCs from 10 mL of samples of maternal blood and determined fetal sex and fetal aneuploidy by fluorescence in situ hybridization (FISH). RESULTS: The average number of NRBCs was 9.85 in samples obtained during the first trimester and 14.88 in samples obtained during the second trimester (P=0.07). The average number of NRBCs with Y chromosome signals was 5.73 in the first trimester and 8.22 in second trimester (P=0.56). However, the percentage of NRBCs with Y chromosome signals in the first trimester (70.6%) was significantly higher than in the second trimester (59.8%) (P=0.049). We diagnosed the blood samples from 7 pregnant women having fetal aneuploidy using this method and the number of NRBCs was 18.4. CONCLUSION: The method using Percoll osmolarity and a double density gradient system may be a very useful method for separation of NRBCs in the first trimester of pregnancy and also in the second trimester.

10.Immunohistochemical Analysis for Basal Activation of NF-κB in Acral Lentiginous Melanoma.

BACKGROUND: Spontaneous basal activation that might be related to survival mechanism of tumor cells by allowing them to escape from apoptosis has been proven in some tumor cells, but it has not been evaluated in malignant melanoma tissue. OBJECTIVE: The purpose of this study was to evaluate basal activation of NF-κB using immunohistochemical analysis and demonstrate its clinical significance in cutaneous malignant melanoma tissue. MATERIALS AND METHODS: Twenty-five cases of acral lentiginous melanoma(ALM) from 20 patients were selected. Immunohistochemical analysis was performed to detect nuclear localization of classic NF-κB heterodimer, p50 and p65 in the formalin-fixed, paraffin-embedded tissues. RESULTS: In about 50% of cases, nuclear expression of NF-κB heterdimer, p50 and p65 was detected, 12(48%) and 13(52%) of 25 cases of ALM, respectively. However, the nuclear ex-pressing of p50 and p65 was not significant for tumor thickness or level of invasion in ALM. CONCLUSION: We demonstrated basal NF-κB activation in malignant melanoma, but we need to research further to demonstrate its clinical significance.
