1.Development of an e-Learning Program about Medication for New Nurses.

PURPOSE: The purpose of this study was to develop an e-Learning program about medication for nurses to enhance nurses' medication performance ability and to analyze learners' responses after studying with this program. METHOD: For the development of the e-Learning program, the NBISD(Network Based Instructional Systems Design) model, suggested by Jung(1999) was applied as a basic model and the instruction design theory of Gagne & Briggs(1979) and ARCS theory of Keller(1983) were applied. After the operation of this program for one month to 34 new nurses, learners' responses were analyzed. RESULT: Learners' knowledge of medication was greatly improved after this program. In addition learners' satisfaction with the overall education program, help in field applicability, ease of screen shift and exploration, and tutor activities were high and the contents were regarded suitable for e-Learning. Many things were advantageous such as easy accessibility, easy understandability with pictures and flash animation, practical cases and feedback from a tutor. Provision of a supplementary handout and improvement of a tight time schedule were pointed out as things to be improved. CONCLUSION: This e-Learning program can be used effectively for medication education for registered nurses, student nurses, and new nurses.

2.Development and Application of Case-based Video Education for Oncology Nursing based on Clinical Reasoning

Purpose:
The purposes of this study were to develop a case-based video education program for oncology nursing based on clinical reasoning and to identify the clinical reasoning competence and satisfaction with the education of students in an oncology advanced practice nursing program after applying the video education.
Methods:
This study was a single group before and after designed experiment for identifying the clinical reasoning competence and satisfaction with education after a case-based video education program for oncology nursing based on clinical reasoning. Eight experts developed a case-based video education program based on clinical reasoning. Fifty-seven students from an oncology advanced practice nursing program participated in this education format.The clinical reasoning competence and satisfaction with education were measured with online self-reporting questionnaires.
Results:
The overall education satisfaction was 4.21±0.67 points (out of 5). The clinical reasoning competence increased from 55.25±6.54 points to 61.18±7.13 points (out of 75) after completing the education program and there was a statistically significant difference (t=-6.52, p<.001).
Conclusion
The case-based video education for oncology nursing based on clinical reasoning was effective in improving the students’ clinical reasoning competence. Various education programs should be developed and applied to improve the clinical reasoning competence of students in an oncology advanced practice nursing program.

4.Repair of Hypospadias Using Bladder Mucosal Graft.

Since Memmalaar first reported the use of bladder mucosa graft technique in penoscrotal hypospadias in 1947, this technique has been used with reasonable success for the reconstruction of new urethra for the posterior hypospadias and hypospadias cripples. The a bladder mucosa is abundantly available, sterile and has good elasticity. This mucosa does not produce hair, does not excoriate, and does not desquamate from the exposure of the urin. Two new urethrae in patients with posterior hypospadias, aged 11 and 21, were successfully reconstructed with bladder mucosal graft technique. A minor meatal stricture developed in one patient. However, the over-all results of this technique appear to be satisfactory.

5.Factors Influencing Falls in Inpatients.

PURPOSE: This study was done to identify factors influencing falls in inpatients. A comprehensive analysis of individual, disease, and environment related factors and an assessment of being in a risk factor group were included in the data. METHOD: The medical records of 325 inpatients were reviewed. Data were collected from January to July 2003. Frequencies, t-test and chi-square-test were used to analyze the data and the SPSS program was used. RESULTS: Individual-related factors for inpatients falls were age, drinking and weakness. Disease-related factors for inpatients falls were diagnostic department, admission from ER or OPD, admission by wheelchair or orther method and activity status. Environment-related factors for inpatient falls were re-education and side-rails. Assessment of the risk factor group related factors were age, history of falls, body balance, depression, communication, medication, chronic disease, urinary condition and total score for risk factors. CONCLUSION: Experienceing a fall among Korean inpatients was associated not only with individual factors, but also with disease and environmental factors. The findings of this study suggest that broad intervention programs should be provided to prevent inpatient falls.

6.A Case of Gastric Leiomyoblastma.

A 48 year-old man was admitted with the chief complaint of epigastric pain for 10 days. With upper gastrointetinal series and gastrofiberscopy, there noted a round hemispherical submucosal tumar at lesser curvature side of stomach. Because of continuous epigastric pain and large submucosal mass with mucosal ulceration the patient undewent operation. After subtotal gastrectomy, we found the round well-demarcated tumor with mucosal ulceratian and hemarrhage, which located at muscular layer. Microscopically, dominent cells of tumor are round or polygonal epithelioid cells, which cytoplasm was lightly eosinophilic and was tightly coadenced about nucleus so that the bulk of the cell appeared clear. In the immunohistochemistry with desmin, the tumor cells are positive. Histological diagnosis was leiomyoblastoma of stomach. In 1960 Martin et al. described six cases of unusual round cell myogenic tumor of stomach. In 1962, Stout report a series of 69 similar cases and he proposed the term leiomyoblastoma'. Until recent, leiomyoblastoma are rare and largely misdiagnosed as leiomyoma or leiomyosarcoma. A careful histological study is recommended in case of smooth muscle cell tumor of stomach

7.A Case of Pneumatosis Cystoides Intestinalis after Erlotinib-containing Chemotherapy for Pancreatic Cancer.

Pneumatosis cystoides intestinalis (PCI) is an uncommon condition that is characterized by the presence of gas within the bowel wall. We experienced a case of PCI after erlotinib-containing chemotherapy for advanced pancreatic cancer. A 66-year-old woman was admitted to the hospital with intermittent epigastric pain, and she diagnosed with advanced pancreatic cancer accompanied by hepatic metastasis. Combination chemotherapy of gemcitabine and erlotinib was started. PCI occurred about 14 months after start of the chemotherapy but she did not complain of abdominal pain and physical examination showed no evidence of peritoneal irritation sign. Laboratory findings showed no abnormalities. Following conservative treatment for 3 days, no specific symptoms were observed, and abdominal X-ray examination showed reduced abnormal air deposition. Erlotinib-containing chemotherapy was continued and PCI had not recurred.

8.Barium Peritonitis due to Inadvertent Vaginal Insertion rather than a Colonic Insertion: 1 Case Report.

Inadvertent vaginal insertion of barium sulfate is an uncommon complication of a barium enema examination. In the few reported cases, venous embolizaton of barium occurred and this usually resulted in death. We present here a case of vaginal insertion of the enema catheter in a young woman, resulting in barium in the uterus, fallopian tubes and abdominal cavity, but not in the veins. After an emergency laparoscopic operation, the patient had been doing well for 6 months without evidence of complication.

9.Successful Removal of a Large Common Bile Duct Stone by Using Direct Peroral Cholangioscopy and Laser Lithotripsy in a Patient with Severe Kyphosis.

A 75-year-old woman with hypertension presented with acute suppurative cholangitis. Chest radiography revealed severe kyphosis. Abdominal computed tomography revealed a large stone impacted in the common bile duct (CBD). The patient underwent emergent endoscopic retrograde cholangiopancreatography, and cholangiography revealed a large stone (7×3 cm) in the CBD that could not be captured using a large basket. We could not use the percutaneous approach for stone fragmentation by using a cholangioscope because of severe degenerative kyphosis. Finally, we performed holmium laser lithotripsy under peroral cholangioscopy by using an ultraslim endoscope, and the large stone in the CBD was successfully fragmented and removed without complications.

10.Adenosquamous Carcinoma of the Pancreas Literature Review.

Adenosquamous carcinoma of the pancreas is a rare tumor that has a less than favorable prognosis because of its aggressive biological behavior. A 47-year-old man was admitted with complaints of right upper quadrant pain of the abdomen. Abdominal computed tomography (CT) demonstrated a low density solid mass lesion in the uncinate process of the pancreas. Dynamic CT, fine needle aspiration by endoscopic ultrasound and the cytological examination by endoscopic retrograde cholangiopancreatography were performed for making the preoperative diagnosis, but the results were a disappointment. Pylorus preserving pancreatoduodenectomy was performed. The resection specimen was composed of squamous cell carcinoma components mixed with adenocarcinoma. The patient stopped further treatment after his 1st chemotherapy cycle because of his poor condition, which due to multiple metastases to the liver and lymph nodes. We report here on a case of adenosquamous carcinoma of the pancreas and we review the relevant Korean literature.
