1.The Relationship between Work Posture and Cervical Spine Abnormality among Cervicobrachial Disorder Workers in a Microwave-oven Assembling Factory.

This study conducted to evaluate the association of work posture with cervical spine abnormality toward 106 microwave-oven assemblers. A self-administered questionnaire was used to obtain a general characteristics and a work history of workers. The posture of neck and shoulder joint of workers were measured with video tape recording and the simple X-ray of cervical spine was checked-up to observe the X-ray abnormality including degenerative change, curvature abnormality, disc space narrowing or disc canal narrowing. The result were as follows. 1. The prevalence of degenerative change among cervicobrachial disorder workers was 44.3%(47 persons), curvature abnormality was 43.4%(46 persons), disc space narrowing was 21.7%(23 persons), and disc canal narrowing was 21.7%(23 persons). 2. The prevalence of degenerative change at cervical spine was increased with the degree of neck flexion(flexed versus neutral; odds ratio (OR)=2.7), the total work posture of neck(mild or severe awkward versus neutral; OR=1.2, 3.4). 3. The prevalence of degenerative change at cervical spine was increased with the degree of the right shoulder flexion, the heaviness of the materials carried by the right hand and the degree of awkwardness at the total work posture of right shoulder(p<0.05). 4. There was no evidence of association between curvature abnormality, disc space narrowing, disc canal narrowing and work posture. In conclusion the awkward work posture was related with degenerative change of cervical spine among microwave oven assembling workers and the further study in these field must be made to prove the association objectively.

2.Studies on anti-oral cancer activities of medicinal plant extracts.

Treatment of oral cancers with chemotherapeutic agents are evaluated as an effective method for remission to reduce cancer proliferation nowadays. But, minimization of side-effects such as bone marrow suppression, gastrointestinal toxicity and renal damage is another problem to be solved. Thus, a possible approach to develop a clinically applicable chemotherapeutic agents is to screen anticancer activity among traditional medicinal plants which have been used for thousands of years with very low side-effects in orient. In this study we focused on screening anti-oral cancer activities among 14 traditional medicinal plant extracts that revealed anticancer activities on other solid tumors. The results were as follow: 1. Methanol extract of Lepidium apetalum showed the highest anti-oral cancer activity against A253 cells. At concentration of 4 microgram/ml, the cell viability was 48% under our experimental condition. IC50 value obtained was 4 microgram/ml. 2. Methanol extract of Coptis japonica and Solanum nigrum were effective on KB cells. Cell viability observed were 62% and 67% at concentration of 4 microgram/ml, and IC50 values were 12 microgram/ml and 10 microgram/ml respectively. 3. When the methanol extract of Lonicera caerule was combined with 2 microgram/ml of cisplatin, the anticancer activity was synergistically increased. One hundred microgram/ml of Lonicera caerule showed 92% (alone) or 59% (combined with cisplatin) cell viabilities. IC50 value of Lonicera caerule extract against KB cells was reduced from 301 microgram/ml to 126 microgram/ml when combined with 2 microgram/ml of cisplatin. 4. Medicinal plant extracts effective on both A253 and KB cells were Coptis japonica, Lepidium apetalum, Solanum nigrum, Caesalpiniae Lignum, Curcuma aromatica.

3.A Case of Phthiriasis Palpebrarum in Children.

The majority of the crab louse is found in pubic hairs, but some of them will colonize axillary hairs, eyelashes, eyebrows and occasionally scalp hairs. In children crab louse may colonize the eyelashes and scalp hairs. The authors found a crab louse, Phthirus pubis and their nits on the eyelashes and scalp hiars of 25-months-old female who complained of foreign body and itching sensation of her right upper lid. Treatment is accomplished by careful mechanical removal of the louse and nits followed by the epilation and application of antibiotic eyedrops or ointments. We report a case of Phthiriasis palpebrarum in children.

4.Analysis of the Effect of Lumboperitoneal Shunt and the Prognostic Factors in Communicating Hydrocephalus.

The effect of lumboperitoneal(LP) shunt and the prognostic factors of 40 cases of communicating hydrocephalus confirmed by brain computerized tomography(CT) and/or magnetic resonance imaging(MRI) and radioisotope cisternography(RI) were analyzed. Possible prognostic factors such as disease entity, CT or MRI findings, and the type of radioisotope cisternography were compared to improvement of clinical status after LP shunt. The etiology of communicating hydrocephalus in the analyzed 40 cases included trauma in 13 cases(32.5%), subarachnoid hemorrhage(SAH) in 12 cases(30%), intracerebral hemorrhage(ICH) in 10 cases(25%), and idiopathic in 5 cases(12.5%). An overall clinical improvement after LP shunt was seen in 22 cases(55%). However, in 17 cases of SAH and idiopathic group, improvement after LP shunt was seen in 13 cases(75%), indicating that LP shunt is more effective in SAH/idiopathic group than trauma/ICH group(p<0.05). Many findings of brain CT/MRI such as Evan's index, periventricular low density, 3rd ventricular width, obliteration of cerebral sulci, rounding of frontal horn, and cortical atrophy were analyzed as prognostic factors. Except for the absence of cortical atrophy(p<0.05), none of the factors were related to the patient's outcome. The type of abnormal RI cisternography findings(Typ I, II, III) also did not show any relationship with the effectiveness of LP shunt. These findings suggest that SAH/idiopathic group were more favorable candidates for LP shunt than trauma/ICH group, and that there are no reliable brain CT/MRI findings indicating a good prognosis after LP shunt except for the absence of cortical atrophy. RI cisternography findings are also not a reliable diagnostic tool in evaluating the indication of LP shunt in communicating hydrocephalus.

5.Clinical Analysis of Electrical Burn Patients.

BACKGROUND: Good documentation of electrical injuries at the time of presentation is very important to emergency management, so this study was designed to investigate the clinical characteristics and the outcomes of patients with electrical injury. METHODS: A review of 75 cases of electrical injuries admitted to our hospital via the emergency department over a 4 year period from 1996 to 1999 was conducted. RESULTS: There were 49 patients with high-voltage injuries and 19 patients with low-voltage injuries. All but 4 patients were males, with a mean age of 29.5 years. The most common type of injury was 14.5% TBSA in the high-voltage group and 2.5% in the low-voltage group. Forty-nine(72.1%) of the injuries were work related. The number of patient with compartment syndrome was 19, and fasciotomies were performed in all but one patient. Myoglobinuria was noted in 22 patients, but no patient developed acute renal failure due to myoglobinuria. In the high-voltage group, 10 limb amputations were performed. Complication were observed in 12 patients. The most common complication was neurological injury. The average length of hospital stay was 50.7 days in the high-voltage group and 13.8 days in the low-voltage group. The overall mortality rate was 4.3%. CONCLUSION: Prevention of electrical injuries is very important. Education and compliance with safety measures, as well as basic knowledge and precaution in dealing with electricity, are essential to avoid these injuries.

6.Initiation of Herpes Zoster Treatment and Postherpetic Neuralgia.

BACKGROUND: Herpes zoster is a significant and troublesome disease. The pain of acute herpes zoster may be severe, but is usually transitory. Some patients, especially the elderly at particular risk, go on to develop neuralgia. Unfortunately, neuralgia is often severe and refractory to most forms of treatment. The purpose of this study was to estimate improved rates of neuralgia according to associated factors. METHODS: We observed the improved rates of neuralgia in 123 patients who had herpes zoster with severe pain and treated with antiviral therapy after admission. Also, we compared them by age group, dermatomal distribution, and initiating day of antiviral therapy. RESULTS: At 4 weeks of treatment, the improved rates of neuralgia according to age younger group was high and older group was low. The improved rates of neuralgia according to the starting day of treatment was high 100% in 1 day, 76.5% in 2, 65.0% in 3days, and 18.2% in 7 days after skin eruption. CONCLUSION: In herpes zoster with severe pain, age and initiation of antiviral therapy are significant predictors of neuralgia.

7.Posterior Instrumentation of Thoracolumbar Fracture.

The thoracolumbar spine remains the most common site of vertebral column injuries. Surgical stabilization using posterior instrumentation for thoracolumbar injuries offers several advantages such as nearly anatomical reduction of fractures, protection of neurologic function and most important early ambulation of the patient, and so major benefits of early fixation are decreased hospital stay, early rehabilitation, and prevention of deformity and pain. The treatment of fracture-dislocation of the thoracolumbar spine has been progressively improved over the past decades and recently a lot of posterior instrumentation has been introduced to improve fixation of the involved vertebrae three dimensionally and short segmental fixation as possible.

8.Diagnostic Value of the Pediatric Appendicitis Score in Pediatric Appendicitis.

PURPOSE: This study was performed to evaluate the pediatric appendicitis score (PAS) and to propose diagnostic criteria for acute appendicitis in children. METHODS: The medical records of 194 patients who underwent appendectomies for clinically suspected acute appendicitis and the preoperative evaluations of those patients in our emergency room were, respectively, reviewed. Each of 8 clinical variables, hopping tendereness in the right lower quadrant (RLQ), anorexia, pyrexia, nausea/vomiting, RLQ tenderness, leukocytosis, neutrophilia, and migration of pain, was assigned a score of 1 or 2 to obtain a total of 10. The PAS, ultrasound (US), and combination of the PAS and US were evaluated for sensitivity, specificity, predictive value, and accuracy. RESULTS: Negative appendectomies were performed in 11.3% (22 of 194 patients) of the cases. A PAS > or = 6 was compatible with the diagnosis of appendicitis. However, in cases with a PAS< or =5, US was necessary for the diagnosis of appendicitis. Analysis of the data for the PAS and for combined PAS and US method revealed, respectively sensitivities of 73.8% and 94.6%, specificities of 86.4% and 70.6%, positive predictive values of 97.7% and 96.6%, negative predictive values of 29.7% and 60.0%, and accuracies of 75.3% and 92.1%. CONCLUSION: The PAS is a simple and good diagnostic test for assessing an acute abdomen and diagnosing acute appendicitis in children. However, a combination of the PAS and US is more accurate than the PAS alone in diagnosing acute appendicitis.

9.The Controlled Study between The Group Used Highly purified FSH and The Group Used FSH/hMG in Controlled Ovarian Hyperstimulation for In Vitro Fertilization and Embryo Transfer.

OBJECTIVE: It is known that normal follicular growth and pregnancy are possible using Follicular stimulating hormon(FSH) alone during controlled ovarian hyperstimulation(COH) for IVF-ET. The purpose of this study was to compare and analize the results of COH for IVF-ET between a group using only FSH-HP and a group using FSH/hMG METHOD: 111 cycle of the total 280 COH for IVF-ET cycles belonged to the FSH-HP group, and the FSH/hMG group accounted for remaining 169 cycles. The amount of gonadotropin administered, the duration of hormonal therapy, the number of embryos transfered, the endometrial thickness, and the pregnancy rate were compared between the two groups and statistically analized using the t-test. p values less than 0.05 deemed statistically significant. RESULT: There were no statistical differences on the amount of gonadotropin administered, the duration of hormone therapy, number of oocytes retrieved, nor the endometrial thickness between the two groups. However,the serum E2 level was significantly lower, and the number of embryos of high quality(GradeIandII) significantly higher in the FSH-HP group. In pregnancy rate, FSH-HP group shows higher than FSH/hMG group(38.8% VS 34.2%). but the difference was not large enough to be considered statistically significant. CONCLUSION: The results of this study show that compared to using FSH/hMG, treatment with FSH-HP alone allowed the retrieval of more embryos of high quality, and although not statistically significant, achieved a higher pregnancy rate, thus indicating that it can be used as an effective method for COH.

10.Detection of the Transition Zone and Adhesions in the Diagnosis of Adhesive Small-bowel Obstruction: the Added Value of Ultrasonography (US) in Comparison with Only CT Imaging.

PURPOSE: We aimed to assess retrospectively the benefit of the use of ultrasonography (US) in comparison with the use of only CT imaging for the detection of the transition zone and adhesions to determine a diagnosis of adhesive small-bowel obstruction (SBO). MATERIALS AND METHODS: Thirty-five patients underwent an additional US examination after CT imaging to determine a diagnosis of SBO. All of the patients were surgically confirmed as having adhesive SBO. The CT images were interpreted for the location of the transition zone, the location and shape of adhesions and for other SBO findings. All of the additional US scans were performed with reference to the CT findings. The standard of reference for the diagnosis was the surgical findings. The diagnostic accuracy and mean confidence score of the transition zone location and the detection rate of adhesions were evaluated for both CT imaging alone and for CT imaging with additional US. RESULTS: The diagnostic accuracy to locate the transition zone was significantly increased with the use of additional US with CT imaging (94.6%, 33/35) as compared to 65.7% (23/35) with the use of only CT imaging (p = 0.01). The mean confidence score was significantly increased (by 0.95) with the use of an additional US examination (p < 0.01). The detection rate for adhesions was 20% (7/35) with the use of only CT imaging and the detection rate was 68.6% (24/35) with the use of an additional US examination. CONCLUSION: The use of a US examination in addition to CT imaging can increase the accuracy and confidence to locate the transition zone and can increase the rate to detect adhesions in patients with adhesive SBO. An additional US examination may be especially helpful when the CT findings are equivocal.
