1.Pelvic Arteriovenous Malformation: A Case Report.

Arteriovenous mafformation(AVM) of female pelvic organ is a rare disease of unknown cause. The authors' report a case of pelvic AVM which was incidentally found during US examination of the patient with choriocarcinoma after chemotherapy. The real-time sonography revealed several cystic lesions around the uterus with adjacent dilated tortuous vessels. The color Doppler sonography depicted abundant blood flow mixed with red and blue colors within the cystic lesions and rapid turbulent systolic and diastolic flows. CT showed well-enhancing round vascular lesions with elongated vessels in the pelvis, and MRI depicted signal-void cystic lesions on both T1 and T2 weighted images with small portions of high intensity within the lesions on T2 weighted image. The angiography revealed pelvic AVM fed by tortuous uterine and vaginal arteries with a dilated draining vein

4.Assessement of MR signal intensity of cranium and cervical spine bone marrow.

The components of bore marrow change dramatically during lifetime. To evaluate the bone marrow of cranium and upper cervical spine, the authors retrospectively evaluated 300 examinations of cranium and the second cervical bone in patients without known bone marrow abnormality. T1-weighted images were used to analyze the changes of lone marrow signal intensity according to the age and sex. The signal intensity of bone marrow of cranium increased most rapidly from birth to age of 10 years. Between 11 and 20 years of age, gradual increase of signal intensity was noted. There was minimal augment of signal intensity after age of 20 years. The examination of signal intensity of bone marrow of the cranium revealed slightly higher score in male than in female. The synchondrosis of the second cervical vertebra was visible in 97%. These results may be useful in the detection of abnormal bone marrow signal of cranium and upper cervical spine.

5.Lobar Agenesis of the Liver'Imaging Findings.

PURPOSE: Congenital Iobar agenesis of the liver is a rare anomaly. We report five cases (three cases of right Iobar agenesis and two cases of left Iobar agenesis) and discuss the radiologic findings of this congenital anomaly. MATERIALS AND METHODS: Between July, 1992 and February, 1993, three cases of right Iobar agenesis and two cases of left Iobar agenesis of the liver were diagnosed by means of computed tomography(CT) and/or sonography. MR imging was performed in two patients, cholangiography in two, and digital subtraction angiography in one. RESULTS: The main findings of right Iobar agenesis of the liver were nonvisualization of the right portal vein and absence of liver tissue to the right of gallbladder. The findings of left Iobar agenesis were nonvisualization of left portal vein, absence of liver tissue to the left of the gallbladder, and absence of ligamentum teres. The ancillary finding of the Iobar agenesis was visualization of less than three hepatic veins. CONCLUSION: It is important to consider Iobar agenesis of the liver in differential diagnosis when imaging studies reveal abnormal portal vein branches, unusual position of gallblader, absence of ligamentum teres, and visualization of less than three hepatic veins.

6.Clinical and cholangiographic evaluation of bile duct carcinoma

40 cases of bile duct carcinoma gathered over a 6-year period at Kosin Medical College were reviewed and theirclinical and cholangiographic findings were as follows: 1. There were 29 males and 11 females (the ratio of men towomen, 2.6:1) ranging from 37 to 74 years of age. The majority (70% of cases) were in 4th & 5th decades. 2.Clinical symptoms and signs: jaundice in 95%, RUQ or epigastric pain in 75%, pruritus in 52.5%, dark urine in 35%,weight loss in 32.5%, fever and chills in 22.5%, clay colored stool in 12.5%, and palpable mass in 12.5%. 3.Lab,findings: elevated serum total bilirubin(above 20.0mg% in 45%, 10.0-19.0mg% in 22.5%, 5.0-9.9mg% in 20%,1.3-4,9mg% in 5%), elevated alkaline phosphatase in 95%. Clonorchiasis were noted in 17.5%. 4. Histologic findingswere adenocarcinoma in most cases. 5. The location of bile duct carcinoma were common hepatic duct in 35%, commonbile duct in 32.5%, porta hepatis in 12.5%, junction with cystic duct in 10% and diffuse form in 10%. 6. In 33cases, PTC or post-operative cholangiographic examination were done. And the most frequent findings weredilatation of the proximal bile duct and abrupt narrowing or complete obstruction of distal lumen. In 27cases(82%), complete obstruction of bile duct was noted. Attempts were made to analyze the type of obstruction:Constricted type in 39%, Nipple type in 18%, round or flat type (smooth or slightly irregular) in 15%, andserratd type in 9%. Incomplete obstruction were noted in 6 cases(18%). Among them, abrupt narrowing of lumen wasnoted in 9% and diffuse narrowing in 9%. 7. ERCP was done in 7 cases. Findings were constricted type in 42.6%,constricted and slightly irregular type in 14.3%, downward convexity in 14.3%, diffuse irregular narrowing in14.3% and intraluminal filling defect in 14.3%.

7.Endobronchial IVletastasis of Parosteal Osteosarcoma: A Case Report.

Endobronchial metastasis from extrapulmonary carcinoma, both clinically and radiologically similar in appearance to a primary lung cancer, is rare. We present a case of endobronchial metastasis from parosteal osteosarcoma. The first abnormality noted on the chest radiography was tramline calcification with branching pattern along the right upper lobe bronchus and intermediate bronchus. This lesion progressed into a solid calcified nodule which increased in size. Another lesion with same pattern was also observed in the left lower lung zone.

8.Head and Neck Tumors and Neck Node Metastasis: Comparison of Ga-67 Scan and CT Findings.

PURPOSE: To assess relative diagnostic value of Ga-67 planar, Ga-67 SPECT, and CT images for detection of head and neck tumors and cervical lymph node metastasis. MATERIALS AND METHODS: Thirty eight patients of pathologically proven head and neck tumors including squamous cell carcinomas(n=32), malignant lymphomas(n=3), undifferentiated carcinomas(n=2), adenocarcinomas(n=l) were enrolled in this study. Ga-67 planar and SPECT images were obtained with intravenous injection of 5mCi of Ga-67 citrate. On the basis of 30 and 20 mm in the greatest diameter of cervical lymph nodes, we compared lesion detectability of Ga-67 planar, SPECT, and CT. RESULTS: Thirty eight cases of head and neck tumors were detected in 29 cases(76.3%) with Ga-67 planar image, 37 cases(97.3%) with Ga-67 SPECT, and 32 cases(84.2%) with CT. 25 of 32 squamous cell carcinomas were positive with Ga-67 planar image and all of 32 cases with Ga-67 SPECT. Both of two undifferentiated carcinomas were positive with Ga-67 planar and SPECT images. Two of three malignant lymphomas were positive with Ga-67 planar image and all of three with Ga-67 SPECT. Eight of nine tumors were negative with Ga-67 planar image and those were less than 30ram. One case of adenocarcinoma was negative with Ga-67 planar and SPECT images. Seven of nine lymph nodes greater than 30mm were positive with Ga-67 planar image and all of nine with Ga-67 SPECT. On the basis of 20mm in greatest diameter, 16 of 21 lymph nodes greater than 20 mm were positive with Ga-67 planar and SPECT images. CONCLUSION: CT providing better resolution than Ga-67 scan permitted analysis of size and location of metastatic cervical nodes, however primary tumors of oral cavity, vocal cord, and nasopharynx were often not detected on CT when metallic and motion artifacts were present, where Ga-67 SPECT was useful. Ga-67 SPECT enabled better anatomical localization than Ga-67 planar image and was useful in detection of lymph nodes greater than 30 mm.

9.The Diagnostic Value of MRI for the Differentiation of Benign and Malignant Soft Tissue Masses.

PURPOSE: To evaluate the diagnostic value of magnetic resonance(MR) imaging in the differentiation of benign and malignant soft tissue masses. MATERIALS AND METHODS: MR examination of 47 patients with pathologically proven soft tissue masses were reviewed. This series included 19 malignant and 28 benign masses of upper and lower extremities, body, and neck. Lesions were evaluated for their margin, signal intensity and homogeneity, pattern and degree of enhancement, involvement of the adjacent tissue, and degree of surrounding edema. RESULTS: Sixteen of benign lesions(57.1%) had well-defined and smooth margin. The tumor margin was irregular in nine lesions(47.4%), however, smooth margin was noted in five cases(26.3%) of malignant lesions. On T2-weighted images, signal inhomogeneity was noted in 84% of malignant lesions and 32% of benign lesions. On Gd-DTPA enhancement study, all malignant lesions(n=18) showed enhancement and 89% showed heterogeneously enhancing pattern. Neurovascular involvement was observed more often in malignancy(n=7) compared to benign lesion(n=1). Direct invasion of the adjacent bone was noted only in malignant lesions(n=3). Surrounding edema was revealed in two benign and 11 malignant lesions. CONCLUSION: MR imaging appears useful for differentiating malignant and benign lesions by identifying tumor margin, signal homogeneity of tumor, pattern and degree of enhancement, involvement of adjacent tissue, and surrounding edema. The nature of the lesion can be determined in the majority of cases although no unique criterion to differentiate benignancy and malignancy is identified.
