1.An Epidemiologic Investigation of Typhoid Fever Outbreak in 0hchun Middle and High Schools Located in Young-il Gun Kyungpook Province.

An epidemiologic investigation was conducted to identify the source of infection and the mode of transmission of typhoid fever outbreak occurred in Ohchun middle and high schools located in Young il Gun, Kyungpook Province from May 21 to Jun 4, 1994. Four out of 13 hospitalized students in three general hospitals in Pohang City were culture positive typhoid fever (Salmonella typhi Group D). Review of the in and outpatient logbooks of the three general hospitals in Pohang City, county health center and local clinics in the vicinity of the Ohchun middle and high schools suggested that the outbreak was confined to the schools. Drinking water source of the schools was two underground water pumps that had no treatment facilities; one in northern end and the other in southern end of the main school building. However, the southern water pump was closed on April 28, 1994 because a recent test for the water revealed E-coli suggesting a possible fecal contamination. Two hundred twenty students stated that they had experienced symptoms related to typhoid fever since April l, 1994 in a self administered questionaire survey of all of the 2, 226 students. Personal interviews with these 220 students were performed to confirm the clinical history together with a rectal swab culture for typhoid fever and Widal test. A total of 39 patients were detected; 15 confirmed cases (culture-positive) and 24 suspected cases (symptom positive with equal to or greater than 1:160 titer of Salmonella O-antibody or 1:320 titer of H-antibody titer in Widal test). The epidemic curve showed a unimodal curve with a peak on the second week(17 May, 1994) and tailed down to the 5th week (22 28 May, 1994). The overall incidence rate of typhoid fever was 1.8 per 100 students; 1.5 (1.6 for male, 1.5 for female) in the middle school and 2. 4 (6. 2 for male, 0. 4 for female) in the high school. When contrasted with the students who did not drink water at all in the school, the relative risks of incidence rate of typhoid fever for the students who drank the underground water of the school, both underground water and water brought from home, or water brought from home only were 55.5, 19.0, and 3.0 respectively. Three classes which had especially high incidence rates of typhoid fever were located in the southern part of the main school building, where the contaminated underground water pump is located. These findings suggested that the source of infection was the contaminated underground water of the school. Both of the two underground water pumps were closed permanently and the public running water was supplied to the schools.

2.Experimental Study for Rabbit Corneal Epithelial Toxicity of MMC.

We studied the biological effects of Mitomycin C(MMC) in cellular metabolic activity and cytotoxicity on the corneal epithelial cell in rabbit that is clinically used according to MMC concentrations and exposure time. The concentration of MMC were 0.01%, 0.02%, 0.04%, 0.06%, 0.1%, and exposure time were 5, 10, and 30 minutes. MTT assay (3-I4,5-Dimethy-lthiazol-2-ylI-2,5-diphenyltetrazolium bromide) and transmission electromicroscopic examination were performed to access the metabolic activity of corneal cell, toxicity of corneal epithelial cell, and cellular morphology. In calorimetric assay, the higher the concentration of MMC and longer the duration of exposure time, the cellular metabolic activity was decreased. Also the toxicity of corneal epithelial cell was increased in a concentration and time dependent manner. The metabolic activity of corneal cell were inhibited by 50% at least over MMC 0.04% for 30 minutes exposure time. The LD50 at 5 minutes exposure was MMC 0.057%. In histologic finding, the higher the concentration of MMC, the change of cellular structure such as loss of microvilli, enlargement of rough endoplasmic reticulum or mitochondria, damage of nuclear membrane were more appeared. To obtain the efficacy of MMC for inhibition of cellular proliferation, we think that further experimental and clinical studies should be need to assess the safe concentration and exposure time of MMC to the corneal toxicity.

3.Safety of intravenous thrombolysis in embolic stroke by infective endocarditis

Ischemic stroke is a serious neurological complication of infective endocarditis. Intravenous tissue
plasminogen activator (t-PA), which has only been approved for treatment of hyperacute stroke, has
been excluded as an ischemic stroke treatment due to infective endocarditis according to current expert
consensus guidelines. Here, we describe a case of a hyperacute stroke patient treated with intravenous
t-PA, who was later diagnosed with infective endocarditis.

5.Surgical treatment of Giant Cell Tumor

Giant cell tumor is a predominantly benign condition but often the tumor is locally aggressive and tends to have high rate of recurrence and it can evolve into a malignant tumor. Thus the lesions have a practically difficult therapeutic problem for the orthopaedic surgeon and any method of treatment has not been satisfied. Between March 1984 and March 1993, clinical observation was carried out on 22 cases of giant cell tumor of bone to analyse the recurrence rate and functional evaluation of the joint according to the treatment methods, pathological grade and cortical destruction. The mean follow-up time was 75. 2 months. The recurrence rate of the intralesional excision group was 54.5% and that of the wide excision group was 9%, There was no relationship between the pathologic grade and recurrence rate. There was no recurrent case in the grade I cortical destruction but almost all of recurrence was occurred in the grade I or II cortical destruction. The functional result of the giant cell tumor occurred around the knee joint was that the mean ratings in the intralesional excision group were 79% and in the wide excision group were 51%. In conclusion, to achieve better joint function and less recurrence rate, intralesional or marginal excision with heat using bone cement or with chemical cauterization using phenol and alcohol can be used for cases of radiological grade I or II, and wide excision for grade III.

6.Measurement of BMD ( Bone Mineral Density ) and Hip Axis Length for Predicting Hip Fracture.

Osteoporosis is a disease characterized by excessive bone loss or osteopenia particulary in the axial skeleton at the site of fracture, such as the spine and proximal femur. Since the strength of both spine and femur is directly proportional to the bone mass, this osteoporosis always increases the risk of fracture. In this study, to evaluate whether a simple measurement of femoral geometry and BMD value are related with hip fracture, we obtained DEXA Scan (Lunar Expert-XL) of hip by retrospective study. DEXA scan was measured on 70 control people and 17 hip fracture patients aged 50 or older. The result is I. The mean Ward BMD value of hip fracture group is significantly lower than control group (Hip fracture group: 0.52g/cm2, Control group: 0.67g/cm2 P=0.0001) 2. The mean L-spine BMD value of hip fracture group is significantly lower than control group (Hip fracture group: 0.81g/cm, Control group: 0.97g/cm2 P=0.0002) 3. The mean femur axis length of hip fracture group is longer than control group (Hip fracture group: 6.77g/cm2, Control group: 6.57g/cm2 P=0.006) As a conclusion, the measurement of BMD and hip axis length in DEXA scan is an effective method for screening the hip fracture risk patient and BMD value of femur, hip axis length and L- spine BMD value are strongly associated with hip fracture.

7.Clinical Observation on the Effect of Parenteral Reserpine.

Parenteral reserpine was given intramuscularly to 32 hospitalized hypertensive patients: 10 hypertensive patients without renal insufficiency, 3 hypertensive patients with heart failure, 10 hypertensive patients of malignant phase or with uremia, and 9 hypertensive patients with cerebrovascular accident. Follwoings were the result. 1. In the majority of patients, the effective dose of reserpine was 2 to 3 mg. 2. Reserpine given intramuscularly lowered blood pressure in 2 to 4 hours, had its maximum effect in 3 to 6 hours and had a duration of 3 to more than 24 hours (average 9 hours). 3. When effective dose of reserpine was given, blood pressure was lowered significantly (more than 30mmHg in mean blood pressure) in 18 patients (81.7%) of 22 hypertensive patients without renal insufficiency, and in 4 patients (40%) of 10 hypertensive patients with renal insufficiency. 4. Major side effect was drowsiness which was more evident in the patients with renal insufficiency. 5. Reserpine administered parenterally is an effective and safe agent for the treatment of hypertensive emergencies on a short term basis especially in the patient without renal insufficiency.

9.Posterior reversible encephalopathy syndrome following rapid correction of anemia

A 49-year-old woman with anemia who developed headache and seizure after blood transfusion was
diagnosed with posterior reversible encephalopathy syndrome (PRES). Magnetic resonance imaging
showed typical PRES findings including lesions in bilateral parieto-occipital subcortical white matter
and overlying cortex. Only a few cases of PRES after transfusion have been reported and this case is
unique in that there was a latent period between infusion and development of PRES. We postulate that
rapid change of hemoglobin level may disrupt cerebral autoregulation and result in delayed PRES. We
suggest that neurological symptoms after blood transfusion should be appropriately investigated.
10.The Role of Preoperative Indirect Ophthalmoscopy Under General Anesthesia for Diagnosis of Torsional Strabismus.

The purpose of this study is to evaluate the status of fundus torsion in 48 patients with unilateral inferior oblique overaction(IOOA) and the results were compared with that in 182 patients who showed normal inferior oblique function by an indirect ophthalmoscope under general anesthesia before strabismic surgery. In 48 eyes with unilateral IOOA, 35 eyes(72.9%) showed extorsion in the same eye. We could also find extorsion in 9 normal fellow eyes(18.7%). Among 182 horizontal strabismic patients(364 eyes) with normal inferior oblique function, there were 23 eyes(6.3%) of extorsion and 1 eye (0.3%) of intorsion. We compared the degree of extorsion with that of IOOA in 48 patients with unilateral IOOA: a mild extorsion were observed in 2 of 10 eyes with IOOA+1. A mild and moderate extorsion were observed in 13(72.2%) and 3 (16.7%) of 18 eyes with IOOA+2, respectively. A mild and moderate extorsion were observed in 9(45%) and 8(40%) of 20 eyes with IOOA+3 respectively. These findings showed that the greater IOOA produces the greater extorsion(p<0.01). On our results the fundus indirect ophthamoscopy before strabismic surgery seems to be helpful in the diagnosis of cyclotorsion.
