1.Three Cases of Familial Benign Chronic Pemphigus.

Familial Renign Chronic Femphigus(FRCP) is rare, persistent and vesicobullous genodermatoai. Three casee of FRCP with typical clinical and histapathological findings are reported. The first case of FBCP was 27-year-oId man who had recurrent episodes of grou vesicles and rust with miId pruritue at the posterior neck, in both axillae and in inguinal area for 10 years. The second case was 49-year-old woman with skin lesions of recurrent persistent. vesicles, pustules on the erythematous skin, crust and lichenification on the flexural area of extrimeties, axillae, ingumal and neck area for 10 yeara. The third case was 49-year-old male patient who had recurrent pruritic grouped vesicles with crust on the neck and axillary area. In all three patients, skin eruptions usually began and or aggravated in the spring or summer and tend to heal spontaneously in the autumn and winter and showed positive Nikolsky sign. The only one case of the reported three casea had familiaI history of skin lesions similar to FRCP. Histopat,holag;ic finding of biopsy specimens from the vesicular eruption showed auprabasal vesiculation with acantholysis, lacunae and villi formation in all 3 cases.

3.Three Cases of Darier's Disease Occuring in One Family.

Darier's disease is a congenital dyskeratosis and is as an autosomal dominant reait but a case with a family history is relatively rare and only 2 such cases have been reported in Korea. We present typical Darier's disease occuring in a mother and her 2 daughters. A 45 year-old female has shown muitiple, confluent, dirty verrucous keratotic papules on the seborrheic area and whitish papules on the sotf palate since the age of 7 year-ole. Histologically there, are corps ronds, grains and suprabssal lacunae in the epidermis.

5.Biochemical Analysis on Postmortem Specimens.

Because the postmortem changes rapidly distort values of many constituents in the body fluids, the determination of postmortem chemical changes and relation among body fluids is very important in medicolegal community. A laboratory result is not useful if appropriate data for comparison are lacking. The aim of this paper is to determine the reference ranges and the postmortem changes for chemical constituents in postmortem specimen. We examined 17 clinical laboratory parameters in cadaveric blood and vitreous humor obtained from 51 autopsy cases. The mean, standard error, standard deviation and confidence interval of all measured constituents were shown in Table 2 & Figure 1. The postmortem blood level of amylase, phosporus, LDH, ALT, AST and CPK are very higher than those of antemortem blood. The postmortem vitreous level of total bilirubin, triglyceride, GGT, alkaline phosphatase, albumin, total protein and cholesterol are very low and is not reliable. The ratio of vitreous/blood in calcium, BUN, creatine and uric acid are 0.74, 0.68, 0.29 and 0.23 in decreasing orders(Table 3,4). For the medical examiner or pathologist, routine chemical examinations of the vitreous humor can provide evidence of abnormality not apparent in gross or microscopic examination of the tissue. A more extensive study will be required, using a larger number of subjects, to establish the full range of variability.

7.A Case of Pseudo-Bartter Syndrome.

Pseudo-Bartter syndrome is a rare medical disease of the kidney characterized by normal blood pressure, hypokalemic metabolic alkalosis, hyperreninemia and hyperaldosteronism with drug history of diuretics. We report US, CT and MRI findings of a patient with clinically proved pseudo-Bartter syndrome. The patient was a 37-year-old woman with a history of long term ingestion of the diuretics(furosemide) for 20 years. Renal US revealed hyperechoic renal medulla at both kidneys. The resistive index(RI), calculated from the duplex doppler waveform is 0.61. Unenhanced CT revealed faint high attenuation along the medulla. Tl-weighted MRI revealed indistinct corticomedullary differentiation.

8.CT Appearances of Appendiceal Diseases.

Cross-sectional imaging plays an increasing role in the diagnosis of appendiceal diseases due to known highnegative appendectomy rate. 9.A Case of Granuloma Faciale.

A case of 38 year old woman had noted 3 lesions with asymptomatic, reddish brown colored, coin sized, slightly elevated, confluent patches on the right postauri-cular for about 20 years. The infiltrate in the patches felt moderately firm to the touch. the lesions were suspected to be chronic dermatitis group, erythema multiforme or granuloma annulare clinically, but the histopathologic features showed the typical findings of granuloma faciale. Literature was briefly reviewed for the discussion and comparision of clinical features and histopathological findings of author's case with the 2 cases previously reported in Korea.

10.Clinical and Hypotensive Experiences with Diltiazem Hydrochloride(Herben(R))in Essential Hypertension.

30 patients with essential hypertension were treated with diltiazem (Herben(R))significantly decreased both systolic and diastolic blood pressuer and heart rate. Mean decrease in systolic & diastolic blood pressuer were 38.0+/-4,5mmHg and 16.3+/-2.3 mmHg respectively and mean drop in heart rate was 17+/-2 beats/mm. The side effects of diltiazem (Herben(R)) were mild headache, fatigue and pruritus, but those were insignificant. Diliazem may be of benefit to hypertensive patients, and particularly useful when hypertension occurs in association with coronary artery disease.
