1.Maffucci's Sundrome ( Dyschondroplasia with Hemangiomata ).

A case of Maffucci's syndrome, which was considered to be a first case reported in Korean literature, was presented with review of literatures. The patient, 17 years old Korean girl, had muliple cutaneous cavernous type hemangioma on the right foot and ankle area with an enchondroma in the third toe which showed slight shortening of length and abnormality on bone X-ray study. Authors presented this case with the results of histopathological examination of the skin and bones and right side femoral angiography for establishment of the diagnosis.

2.Hyperlipoproteinimia Type III ( Broad Beta disease, Floating beta lipoproteins ) with Review of Literatures of Primary Hyperlippoproteinemias.

A stout and obese fourty five year-old Korean man was affected with hyperlipoproteinemia type III. His skin lesion showed yellowish papular eruptive xanthomas with erythematous halo located chiefly on the trunk and extensor aspects of the extremities including both palms. In addition to the skin lesion he showed pea-sized cervical lymphnode swellings, slightly tessellated fundi and, on ECG, premature ventricular contraction. Otherwise no specific findings were seen. The laboratory examinations showed increases of both serum cholesterol and triglycerides, increased fasting blood sugar level and abnormal glucose tolerance curve with turbid color of fasting blood serum which had been kept standing at 4C for 24 hours. On the paper electrophoresis "broad beta band" blending with the pre-beta area was noted. The authors discussed laboratory characteristics, clinical manifestations and differential diagnosis of primary hyperlipoproteinemia on each type with review of the literatures of the disease.

3.Twenty Six Cases of Exfoliative Dermatitis: It's Clinical, Etiological, Laboratory Findings and Courses of Treatment, with Review of Literatures.

Twenty six cases of Exfoliative dermatitis were found during the years from July of 1967 to June of 1972 at the department of dermatology of Chonnarn University Hospital, and the cases of the disease were studied for ciinical signs and symptoms, etiological factors, laboratory findings and responses to the treatment. The incidence of the disease to total number of skin patients in the years: 0. 2%, mean age distribution: 41. 9 years old, and the most cornmon etiological factors: drugs such as 4. 4-diaminodiphenylsulfone (dapsone, DDS), mercurial preparations, herbs and arsenicals in order, were found. The distinct laboratory findings of the disease revealed leukocytosis, eosinophilia and hypoproteinemia. However, the results of liver function tests (SGOT, SGPT, CCF, TTT, Alk. phosphatase) were found to be generally within normal limits in cases of hypoproteinernia. The clinical symptoms of the disease appeared to be lymphadenopathy, coated or avitaminosis type glossitis or stomtitis with characteristic skin lesions of the disease. In the treatment of the disease, the authors emphasized careful treatment to correct fluid and electrolyte balance and hypoproteinemia with adequate topical skin management. The authors also had discussions of the etiological factors comparing with other literatures, clinical signs and symptoms, therapeutical achievement and problems, and reviewed the literatures of the disease.

4.Further Studies of Adenosine Deaminase in the Human Skin (II).

Adenosine deaminase (adenosine aminohydrolase, E.C.3. 5. 4. 4; ADA), which catalyzes the deamination of adenosine to yield inosine and ammonia, was characterized in the human penile foreskin. ADA was found to be present in both layers of the skin with slightly higher activity in the epidermis(Epidermis; (7. 2+2. 3) * 10-4 unit/mg protein, Dermis;(5. 7 +1. 9) *10-4 unit/mg protein). The enzyme exhibited a broad pH optimum from 6. 5 to 8. 0 in both layers of the skin, and was heat-labile, being completely inactivated by heat treatment at 70C-75 C for 10 minutes. In contrast to the ADA of other tissue, the enzyme was inhibited by 2 mM of Cu2+, Fe2+ and Co2+ at pH7. 0 in both layers of the skin. The inhibitory effect of Cu2+ on the enzyme was stronger than other metal ions, and the enzyme was completely inactivated by 20 mM of Cu2+ in both layers. The Cu2+ inhibited enzyme activities which were recovered by adding EDTA. From the above results, it is suggested that the enzyme in both layers of the skin are consisted of same types of isozymes.

5.The Effect of the Intravenous Esmolol on Blood Pressure and Heart Rate during Electroconvulsive Therapy.

BACKGROUND: Electroconvulsive therapy (ECT) that works by electrically inducing grand mal seizure is an effective therapy for patients with major psychosis and affective disorders. But ECT may produce intense stimulation of the central nervous system resulting in hypertension and tachycardia. Such an acute hyperdynamic state may be undesirable because of possible cardiovascular complications. We compared the ability of different bolus doses of esmolol to blunt the hemodynamic effects of ECT. METHODS: Twenty ASA physical status 1~2 patients were enrolled in a crossover design study to determine the effects of two standard esmolol bolus doses (0.5 mg/kg and 1.0 mg/kg) on the hemodynamic response and seizure duration during ECT. In each patients receiving esmolol or placebo, arterial pressure, heart rate, seizure duration and peripheral oxygen saturation (SpO2) were recorded. RESULTS: The seizure duration with placebo was 43 +/- 9 sec, esmolol 0.5 mg/kg bolus dose was 39 +/- 14 sec and esmolol 1.0 mg/kg bolus dose was 39 +/- 12 sec, but it was not significant. Compared with esmolol 0.5 mg/kg bolus dose, esmolol 1.0 mg/kg bolus dose decreased blood pressure and heart rate during ECT more effectively. CONCLUSIONS: Esmolol 1.0 mg/kg bolus dose was considered to be the better dose in blunting the hyperdynamic response during ECT without shortening of seizure duration.

6.The Study on the Ultraviolet-B Blocking Effect of Sunscreens in the Epidermal Langerhans Cells of Hairless Mice.

BACKGROUND: Sunscreens have been used widely to prevent the photosensitive skin diseases, skin cancer, and skin aging. However, no sunscreen blocks all kinds of effects caused by ultraviolet light(UVL), and the effect of sunscreens on the impairment of immune function by UVL irradiation is controversial. OBJECTIVE: We try to evaluate the efficiency of sunscreens for blocking the depletion of LC induced by UVB irradiation. METHOD: The ATPase positive LCs were observed in the skin of hairless mice(Hr+/Kud) irradiated by UVB with or without topical application of sunscreens. Two commercially available sunscreens with respective SPF 8 and SPF 30 were applied to the dorsal trunk skin. The mice were irradiated with different increasing doses of UVB at a single time. RESULTS: The ATPase positive LCs in the irradiated dorsal and ear skin were significantly de-creased in densities according to the dosage, and apparently revealed a loss of their dendrites, granulation, and clumping from a UVB dose of more than 60mJ/Cm2. With both sun-screen treatment on the dorsal trunk before irradiation, the densities of LCs on the dorsal skin were significantly higher compared to the un-treated groups at all ranges of UVB doses in spite of a dose dependent decrease in their density. However there was no significant difference on their preventive effect between both sunscreens(SPF 8 and SPF 30) except at high UVB dos-es of more than 240mJ/Cm². CONCLUSION: The LC depletion induced by UVB can be partially protected through the topical application of a sunscreen at a UVB dose dependent fashion. However SPF(sun protective factor) dose not appear to be a good indicator for evaluating sunscreens immunologically.

7.Clinical and histopathologic study of malignant melanoma(1980-1990).

A total of 39 malignant melanoma cases diagnosed at Chonnam National University Hospital for past 10 years from 1980 to April, 1990 were included for the clinicopathologic study. Among the 29 primary cutaneous melanomas, 18 cases(62.1 % ) occurred on the hands and the feet, especially on the heel. The average age of the patients was 54.3 years at the time of diagnosis and the male to female ratio was 1 to 1,4. Acral lentiginous melanoma was the most common (62.1% ) histopathologic type and the invasion level c)f 22 cases(78.4% ) was over 1.50mm by Rreslows thickness.

8.A case of acute lymphocytic leukemia with initial manifestations in the skin.

The leukemias are a heterogeneous group of neoplasms arising from the malignant transformation of hematopoietic cells. The cutaneous lesions associated with leukemia may be divided into the specific (leukemic infiltration) and nonspecific (leukemids). These may occur separately or in combination with each other. A 28 year old man admitted to the Department of Dermatology in December 1989, for diffuse infiltrated skin lesions of five months duration. In July 1989, he had experienced hyperpigmented diffuse infiltration on the face, which gradually spread to involve the neck and trunk. In November 1989, he felt cervical, submandibular, and inguinal lymph node enlargement. Skin biopsy specimen showed diffuse dermal and subcutaneous infiltration of atypical lymphocytes. Bone marrow biopsy and peripheral blood smear specimen showed changes of acute lymphoblastic leukemia(L). In addition, we studied infiltrated cell.; by immunoperoxidase staining with a pannel of monoclonal antibodies. The results revealed that the atypical cells expressed mainly helper/inducer T cell marker.

9.A case of sebaceous carcinoma arising from nevus sebaceus of jadassohn.

We report a case of sebaceous carcinoma arising from nevus sebaceus of Jadassohn ocurring in a 68-year-old male, who showed a pedunculated cauliflower like tumor mass on verrucoid linear plaque in the fronto-parietal scalp. Histopathologic findings revealed mult,iple irregular tumor lobules composed mainly of foamy cytoplasmic atypical cells suggesting undifferrntiated sebaceous cells. Good result was obtained with wide local excision and primary closure without recurrence for 9 months after surgiral excision.

10.Clinical Observation on Tumors of the Genito-urinary Tract.

A clinical observation was made on 80 cases of genito-urinary tract tumors of the in-patients in the Department of Urology, Chungnam University Hospital during 3.5 years, from January 1974 to June 1977. The following results were obtained. 1. During this period, of 348 cases hospitalized, 80 cases (76 males. 4 females) had tumors of the genito-urinary tracts, giving a rate of 22.9%.2. In 80 cases of genito-urinary tract tumors, 46 cases (57.5%) were benign tumors and 32 cases (40.0%) were malignant tumors. Of the benign tumors, B.P.H. was the most common tumors and of the malignant tumors, bladder cancer was the most common tumors. 3. The majority of the patients(87.5%) were distributed over age of 40 years, showing highest incidence at 60-69 years. 4. In 80 cases of the genito-urinary tract tumors, 46 cases(57.5%) were prostatic, 17 cases (21.2%) vesical, 9 cases(11.2%) renal, each 3 cases(3.7%) penile and testicular, and 2 cases (2.5%) urethral. 5. Histopsthologically, B.P.H. was observed in 49.3% of all operated tumors, transitional cell carcinoma of bladder in 16.9% and adenocarcinoma of prostate in 5.7%. 6. Operation was performed in 71 cases (88.7%).
