Chronic complication of diving and working in compressed air is dysbaric osteonecrosis. Dysbaric osteonecrosis could be diagnosed by simple bone x-ray, in the case of exposure history to dysbaric atmosphere. The case is 44 years old man who worked as a shell fish diver for 20 years. He was exposed dysbaric atmosphere during diving. He had bone necrosis in both femur head, both distal femur and proximal tibia. He had multiple attacks of decompression sickness. Other causes of bone necrosis are ruled out. We conclude that diver's bone necrosis is dysbaric osteonecrosis.

A total 48 cases of myelography using Metrizamide were performed between August 1981 and July 1983 at the department of Orthopedic Surgery, Kwangju Christian Hospital. Of these cases, H.N.P was 26, tuberculous spondylitis 16, spondylolisthesis 4, and multiple sclero-sis 2 cases. The advantages of this study were as folows: 1. Complications were minimal in most of the cases except a slight headache. 2. Large amount of infusion was possible. 3. Rootlet was clearly seen. 4. Removal of dye was not necessary. 5. It gave a good contrast for the radiologic study.

Sternocostoclavicular hyperostosis is a benign ossifying diathetic disorder characterized by hyperostosis and soft tissue ossification of the clavicles, anterior portion of the first ribs, and manubrium, with variable hyperostosis or ankylosis in the spine and sacroiliac joints. A review of the literature and our own case describes the clinical findings and its characteristic features in RI, CT, and plain film.

Chung-Ang University Penile Squamous Carcinoma cell line (CUPE-1) was established from a lymph node metastasis of human penile squamous cell carcinoma (SCC). CUPE-1 grew as adherent monolayer with a defined doubling time of 24 hours. CUPE-1 showed epithelial characterization on inverted and light microscopy and showed well developed desmosomes and tonofilaments or electron microscopy. CUPE preserved cytokeratin on immunohistochemical staining. CUPE expressed the receptor of epidermal growth factor (EGF), which stimulated the proliferation of CUPE-1 CUPE-1 showed strong tumorigenecity and/or metastatic ability when subcutaneously and intraperitoneally implanted into the nude mouse. CUPE-1 produced tumor associated antigen-4 (TA-4), a tumor marker for SCC of uterine cervix, both in vitro and in vivo. In addition, the serum level-of TA-4 were specifically increased in patients with penile SCC. These results indicate that CUPE-1 retains the characteristics of human penile SCC and could provide an excellent model for the basic research and development of new therapeutic modalities of penile cancer, and that TA-4 may become a valuable tumor marker of penile SCC.

The composition of triglycerides of the axillary sebaceous glands in 10 patient, with osmidrosis and 10 patients with hyperhidrosis was analyzed by high pressure liquid chromatography. Sebaceous glands were isolated and dissected by Kellum's method, and lipids were extracted by Folchs method. Individual triglycerides were separated by high pressure liquid chromatography and fatty acid composition of triglycerides was analyzed by gas-liquid chromatography. A number of individual triglycerides were identified: Fraction 1 consisted of dimyristoarachidonin, dilinoleioarachidonin, and myristol inoleioarachidonin, fraction 2 was trilinolein, fraction 3 was dilinoleioolein, fraction 4 was dilinoleiopalmitin, fraction 5 consisted of dioleiolinolein, dipalmitolinoleiri, and palmitooleilinolein. In osmidrosis patients, cornpositions of fraction 1, 2, 3, 4, and 5 were 5, 4%. 18.5% 29.5% 32.0% 14.1% of total triglycerides, respectively. In hyperhidrosis patients, cornpositions of fraction 1, 2, '3, 4, and 5 were,5, 9%, 20. 6%, 30. 9% 32. 1%, 14. 5% of total triglycericles, resectively. There were no differences in composition of triglycerides of sekaceous glands between osmidrosis patients and hyperhidrosis patients and hyperhi.frosis patients.

From May 1988 to January 1997 authors had experienced 13 cases of recurred blepharoptosis. Previous operative procedures were frontalis muscle suspension in 10 cases and levator resection in 3 cases. Follow up period ranged from 11 months to 9 years. We have treated recurred blepharoptosis using frontalis myofascial flap, orbicularis oculi muscle flap and levator resection in accordance with the postoperative levator function and degree of ptosis of patient and considering previous operative technique. The results were that 12 patients have gained the levator excursion over 7 mm and reduced the height difference between both palpebral fissures less than 2 mm after reoperation (good in 6 cases and satisfactory in 6 cases). From these results we might conclude that the frontalis myofascial flap technique is a good secondary blepharoptosis operation for patient with less than 2 mm of levator function, orbicularis oculimuscle flap technique with 2-4 mm of levator function, levator resection with over than 4 mm of levator function. The expert technique and experience are also important factor for the treatment of recurred blepharoptosis.

Thanatophoric Dysplasia*

It is difficult to treat the complete cleft lip because of wide cleft and malaligned alveolar process, outward rotation of greater(medial) alveolar segment, and severe nasal deformity. Lip adhesion without presurgical orthopedic appliance was performed on 8 consecutive infants with unilateral complete cleft of the primary palate before denfinitive lip repair with Millard I procedure. Among 8 patients, 6 patients had complete unilateral cleft lip with alveolar cleft only and 2 patients were accompanied with complete cleft palate. All patients had more than a 10-mm-wide lip cleft with alveolar arch discrepancy. Lip adhesions were performed at 1-2 months of age and definitive repair was done at 5-6 months of age. Lip adhesions were performed by Randall's method and cheiloplasty was done by Millard I technique. Satisfactory results of lip and nose were obtained aesthetically in 8 cases after an average follow-up of 32 months. The vertical height of the medial and lateral lip segment were a symmetric appearance, while the vermilion tubercle, philtrum, and Cupid's bow were natural. Disadvantages included increased operating time and the sacrifice of same-lip tissue. In conclusion, preliminary lip adhesion can have better functional, esthetic and emotional results since the disadvantages are minor compared to the advantages.
