1.Factors Influencing Regeneration of Calvarial Defects in Rats.

An experimental study was done to evaluate factors influencing guided regeneration of bone in standardized calvarial bony defect. An 8 mm circular transosseous calvarial bony defect was made. Various material such as demineralized freeze-dried bone (DFDB), BioMesh , Millipore filter and its combination was placed in the bony defect. A sequential histopathologic, histochemical, immunohistochemical, and histomorphometric studies were done on the guided bone regeneration in the calvarial bony defect. Bone formation was sigificantly enhanced when the DFDB was retained within the bony defect with a protective bioabsorbable membrane. Inframembranous DFDB-filling was required to prevent collapse of the membrane and preserve spaces for bone regeneration. The bioabsorbable membrane should presumably remain intact for longer than at least 5 weeks to facilitate bone regeneration. The new bone formation was dependent on the barrier-effect (preserving secluded spaces) and inflammation-inducing property of membrane, and guiding bone regeneration of the grafts. Macrophages recruited by grafts were partly involved in decrease of bone regeneration via the sequential events of release of fibronectin, chemotactic effect of the fibronectin to fibroblasts, and collagen lay-down.

4.Antioxidants in Serum and Induced Sputum of COPD Patients.

BACKGROUND: Although an oxidants and antioxidants imbalane has been considered in the pathogenesis of chronic obstructive pulmonary disease (COPD), there is a paucity of reports focussing on the smoking-induced changes of oxidants and antioxidants in COPD. METHOD: The concentration of antioxidants (ascorbic acid, uric acid, retinol, and α- &γ-tocopherol) was measured in the serum and induced sputum of 30 healthy controls and 34 stable COPD patients using high performance liquid chromatography (HPLC). The inhibition of lipid peroxidation as an index of antioxidant capacity was measured in the serum by a TBA assay. RESULTS: The serum concentration of ascorbic acid, α-tocopherol, and retinol were significantly lower in the patients with COPD than in healthy controls (484.8±473.3 vs 1497.8±819.2 pmol/L, p<0.001, 48.38±17.34 vs 73.96±26.29 pmol/L, p<0.001, and 9.51±8.33 vs 15.01±5.88 pmol/L, p<0.05, respectively, mean±SD). However, there were little differences in the ascorbic acid and uric acid concentrations in the induced sputum between the COPD patients and the controls. The induced sputum to serum ratio of ascorbic acid was significantly higher in COPD patients compared with healthy control (0.375 vs 0.085, p<0.05). In the normal controls, the serum ascorbic acid concentration was lower in smokers than in nonsmokers (1073±536 vs 1757±845 pmol/L, p<0.05), but the level was still higher than that of the COPD patients (p<0.05). The serum retinol levels were correlated with FEV1 in COPD patients (r=0.58, p<0.05). The products of lipid peroxidation were increased in normal smokers and COPD compared with normal nonsmokers (115.56±19.93 and 120.02±24.56 vs 91.87±20.71 µmol/µmol Pi of liposome, p<0.05). CONCLUSION: Cigarette smoking may induce the depletion of serum antioxidants and this depletion of antioxidants is suggested to play a role in the pathogenesis of COPD.

5.Prognostic Significance of the Expression of MRP and p53 in Colorectal Carcinoma.

Although MDR was previously thought to be predominantly caused by the expression of the MDR1 gene, it is now increasingly believed to be caused by other mechanism. Recently, over-expression of the multidrug resistance-associated protein (MRP) was suggested a possible mechanism for non-Pgp mediated MDR. Recent studies showed that MRP can confer resistance to a wide spectrum of natural product drug, but the clinical relevance of MRP-mediated MDR in human cancer is poorly understood. p53 is the most widely known tumor suppressor gene. It has been suggested that mutant p53 is related to abnormal proliferation of cell and some what is been related to cellular apoptosis. To determine the clinical significance of MRP and/or p53 expression in colorectal carcinoma, the authors investigated the expression of the MRP and p53 in 81 cases of primary colorectal carcinoma, the relationship between the MRP and/or p53 expression and clinical parameters including 5-yr. survival rate, and the relationship between the expression of MRP and p53. The results were as follows: 1) Of the 81 colorectal carcinomas, 36 (42%) were MRP positive and 28 (34%) were p53 positive. 2) The expression of MRP and/or p53 was not significantly correlated with sex, age, histologic grades, tumor invasion, tumor location, tumor size, lymph node metastasis, TNM stage and survival of patients. In conclusion, these results suggest that expression of MRP and/or p53 is neither related to the known prognostic factors nor a prognostic factor by itself.

6.A Case of Nonsurgical Retrieval of Foreign Body from Rt. Atrium.

Since Thomas et al reported the first instance of successful removal of a broken intravascular guidewire without surgical intervention by using a bronchoscopic forceps in 1964, there have been a number of nonsurgical removal of intravascular or intracardiac foreign bodies using the bronchoscopic forceps, loop snare or basket stone catchers. We experienced a successful retrieval of accidentally broken subclavian puncture catheter from right atrium in 76 year old male patient with cerebral infarction admitted to this hospital on May, 1989. By percutaneous catheterization via right basilic vein, using a loop snare which is modified manually by guidewire for PTCA(USIC(R) Teflon coated PTCA Guide wire : ".014"), we removed successfully foreign body from right atrium. Therefore, we report a case of nonsurgical retrieval of foreign body from right atrium with the review of the literature.

8.The Significance of CD24 Expression in Gastric Adenocarcinoma.

PURPOSE: CD24 is a small heavily glycosylated glycosyl- phosphatidylinositol-linked cell surface protein expressed in hematological malignancies as well as a large variety of solid tumors. It appears to function as a ligand of P-selectin, an adhesion molecule present in activated platelets and endothelial cells. The authors aimed to evaluate the expression of CD24 in adenomas and adenocarcinomas of the stomach and its correlation to clinicopathological data. METHODS: A series of 48 adenocarcinoma cases (advanced gastric carcinoma: 24 cases, early gastric carcinoma: 24 cases) and 12 adenoma cases of the stomach were immunohistochemically stained for correlation with the clinicopathological data. The staining was evaluated as stainability (negative, weak-, moderate-, strong-positive) and staining patterns (membranous vs. intracytoplasmic), which were used for statistical analyses. RESULTS: The present study clearly demonstrated that the stainability of CD24 expression increased with positive nodal status in advanced gastric carcinomas (P<0.005). The stainability of CD24 in the adenocarcinoma group was much higher than in the adenoma group, but this was not statistically significant. Intracytoplasmic CD24 expression was demonstrated only in the adenocarcinoma group, with the exception of the adenoma group, but this also was not statistically significant. CONCLUSION: The authors conclude that the stainability of CD24 could be a molecular marker for gastric adenocarcinomas which could help to predict lymphogenous metastasis.

9.Apolipoprotein E Polymorphism in Schizophrenic Patients.

OBJECTIVES: To explore the role of apolipoprotein E in schizophrenia, we investigated apoli-poprotein E polymorphism in groups of patients with schizophrenia and normal controls. We also examined the relationship of clinical characteristics of schizophrenia to apolipoprotein E genotypes. METHODS: Samples were obtained from 101 schizophrenic patients and 96 controls in Korea and apolipoprotein E polymorphisms were analysed using polymerase chain reaction. RESULTS: The genotype and allele frequencies did not differ from those of controls. The clinical variables of schizophrenia, such as positive and negative groups by PANSS, subtypes by DSM-IV, family history were not associated with each genotypes. CONCLUSION: We could not find the association of apolipoprotein E in Korean schizophrenic patients and it could be suggested that apolipoprotein E isoforms might not play a main role in expression of schizophrenia.

10.Effect of Epidural Catheter Direction on Epidural Anesthetic Spread.

BACKGROUND: The spread of local anesthetics in the epidural space varies significantly between patients. Studies have demonstrated radom cephalad or caudad migration of the epidural catheter. However the relationship between catheter direction and epidural spread of local anesthetic has not been well documented. METHODS: A single-orifice epidural catheter with an end hole was inserted caudad at L2-3(caudad group) or cephalad at L3-4 intervertebral space(cephalad group) in 38 patients. The epidural catheter was filled with 1 ml of Omnipaque dye, and fluoroscopic determination of the exact position of the epidural catheter tip was done to locate the catheter tip within the range of the third lumbar vertebral body. After test dose of 3 ml, 2% lidocaine 22 ml with epinephrine 1:200,000 was administerd at a rate of 0.5 ml/s in supine position. RESULTS: Age varied from 20 to 36 years(mean age, 24.2 3.9 years). There was no significant difference in the rate of spread of skin analgesia, the upper analgeseic level and the degree of the motor block between the two groups. Although the time to reach maximum motor blockade(33.8 11.0 min in the cephalad group, 27.4 11.4 min in the caudad group) and skin analgesia to S 5 segment(21.3 7.6 min in the cephalad group, 17.1 5.4 min in the caudad group) was slightly shorter in the caudad group, the differences were not statistically significant. CONCLUSIONS: This study indicates that the spread of skin analgesia is not affected by the epidural catheter tip direction when lidocaine is injected into the epidural space in young adult patients.
