1.Non-Union of Greater Trochanter Following Low Friction Torque Arthroplasly (LFTA)

This paper is intended to draw attention to the cases of non-union of reattached greater trochanter after LFTA for hip disorders with an our short experience of this subject. Previous information on this subject is scanty. Charnley mentioned on his paper of the long time result of LFTA of the hip for non-union of the great trochanter was reported in 4.2% of 379 hips followed for a minium of four years. Of these, a quarter went on to separation of the bony fragment. With his report, wire breakage occured in 28 cases in the presence of a united trochanter, a phenmenon which results from loops of the wire passing through soft tissues, out side the bone, and becoming fatigued by repeated bending movements. However, he pointed that the clinical result does not seems to be impaired althrough such patients occasionally complain of a feeling of insecurity when weight bearing. Among our series, two cases of non-union of the greater trochanter, after LFTA, is reported. Those of two patient were bilateral hip disorders who has indicated both hip total hip replacement. But we have done only one hip each because of inevitable patients physical conditions during the surgery. Presumably, the principal cause of non-union appeared to be excessive force on the operated one hip. The type of internal fixation for reattachment of the greater trochanter was investigated, six types which we used in this series, two separate knots for security of wire breakage after Bechtol resulted satisfactory union of the detached greater trochanter. A case of detached trochanter for revision which we used original Charnley technique with additional figure of eight wire fixation for re-enforcement. Eventually, we agree that advantage of bilateral total hip replacement at the same operative session.

4.A Case of Orbital Liposarcoma.

Liposarcoma is extremely rare malignant tumor which develops primary in the orbit. Here, we report a case of primary orbital liposarcoma in 17 year old female patient. She visited our hospital due to decreased visual acuity and protrusion of Rt. eyeball. Malignant tumor was suspected by many examination. So exenteration was performed and tumor mass was confirmed as liposarcoma by pathologic examination. She was received cobalt 60 radiation therapy, but recurrent tumor mass appeared at the orbital apex 1 year after operation.

5.Hyperlipidemia and Erection Response by Intracorporeal Injection of Papaverine.

The majority of cases of impotence are closely associated with vascular risk factors and previous studies suggested that hypercholesterolemia contributed to the vasculogenic impotence by inducing impairment of endothelium dependent relaxation of corporeal smooth muscles. However, the roles of individual parameters of lipid metabolism in the development of the vasculogenic impotence are ill defined. Herein we investigated the relationship between the erectile response induced by intracorporeal injection of papaverine and serum levels of various parameters of lipid metabolism ; total cholesterol (TC), triglyceride (TG), low density lipoprotein (LDL), high density lipoprotein (HDL), lipoprotein(a). phospholipid, apoprotein A1(ApoA1), apoprotein B(ApoB), and ratio of serum level of LDL/HDL, TC/HDL, and ApoA1/ApoB. Higher incidence of abnormal erectile response was found in patients with increased serum level of TC, LDL, ApoA1, phospholipid and increased LDL/HDL ratio, However, the erectile response to intracorporeal papaverine between the patients with abnormal parameters and those with normal levels did not show significant difference(p>0.15). These results suggest that the individual parameters of lipid metabolism by themselves may not have direct relationship with papaverine induced erectile response.

10.Experimental Study on the Effect of Combination Therapy of Interferon Alpha and Retinoic Acid in Renal Cell Carcinoma.

Experimental study was done to investigate the effect of combination therapy of recombinant interferon alpha-2a (IFN-alpha) and 13-cis-retinoic acid(13cRA) or all-transretinoic acid(TRA) on the in vitro and in vivo proliferation of human renal carcinoma cell line(CURC-2). 13cRA inhibited the in vitro proliferation of CURC-2 significantly at tolerable serum concentration in human(0.000001mole) and IC50 of 13cRA was found to be about 0.000003 mole. TRA did not inhibit the in vitro proliferation of CURC-2 significantly at tolerable serum concentration and IC50 of TRA was found to be too high(0.00003mole) to be administered in vivo. IFN-alpha inhibited in vitro proliferation of CURC-2 significantly with IC50 of about 500 unit/ml. Combined administration of low concentration of IFN-alpha(300 unit/ml) and 13cRA (0.000001mole) showed significant synergistic antiproliferative effect (79%, p<0.01 ) compared to single administration of IFN-alpha(29%) and 13cRA(29%). Combined administration of IFN-alpha and TRA showed underadditive effect. Combined administration of IFN-alpha(50,000 unit s.c./mouse/day) and 13cRA (0.5 mg p.o./mouse/day) to nude mouse significantly decreased the incidence(p<0.05) and tumor weight(p<0.001) of subcutaneously implanted CURC-2 cells and showed significant synergistic effect(p<0.05) compared to 13cRA or IFN-alpha single administration. 13cRA-administered animals did not show toxic sign of hypervitaminosis A. These results suggest that IFN-alpha and 13cRA show significant synergistic antiproliferative effect both in vitro and in vivo on human renal carcinoma cells and that combination therapy of IFN-alpha and 13cRA may become effective and safe adjuvant therapy for renal cell carcinoma. Based on the results of this study, clinical trials of combination therapy of IFN-alpha and 13cRA are ongoing in patients with renal cell carcinoma.
