2.Immunohistochemical Study of bcl - 2 Expression in Basal Cell Carcinoma.

BACKGROUND: Basal cell carcinoma(BCC) is the most common tumor of light-skinned people. Most of them are relatively innocuous, but some may cause extensive tissue destruction, and result in death by local infiltration of vital structure or metastatic disease. The bcl-2 is a protooncogene involved in tumorigenesis by blocking apoptosis or programmed cell death. OBJECTIVE: To investigate whether there was a difference in bcl-2 expression in different histopathological types of BCC. Method: We performed immunohistochemical staining by using a monoclonal mouse anti-human bcl-2 oncoprotein in 28 BCC(nodular: 7 cases, micronodular; 3 cases, adenoid; 1 case, infiltrative; 9 cases, and mixed; 8 cases). Result: All 28 BCC expressed bcl-2, and the degree of staining was different according to histopathological types; poor prognostic micronodular(100%) and infiltrative(73.3%) types were relatively strong, and nodular(66.7% ) and adenoid(75.0%) types were moderate. Conclusion . These results suggest that the degree of bcl-2 expression in BCC seems to be dependent to the histopathological types together with clinical behavior.

3.A case of recurrent infantile digital fibroma.

Recurrent infantile digital fibroma is a peculiar fibrous tumiir that may be single or multiple on fingers and toes of infants and children. We report a 5-year-old gril with recurrent infantile digital fiber oma which was recurrent on the left 5th toe. The lesion had noted at the age of 6 months. and was excised surgically when the child was 3 year old. 2 years after operation, the lesion was recurred in operation site. Microscopically, nodular fibrous mass is attached to flattened,pidermis. The underlying nodule is composed of proliferating fibroblasts surrounded by derse collagenous tissue. We could find eosinophilic inclusion bodies in the cytoplasm of fibro ilast. It was stained pink with H&E, bright red with Massons trichrome and purple with PTAH.

5.The efficacy of fetal genetic diagnosis using fetal nucleated erythrocytes in maternal blood.

OBJECTIVE: The conventional methods to determine fetal genetic status, such as amniocentesis or chorionic villi sampling(CVS) have small procedure-related risk of abortion. Recently, several researchers reported that fetal genetic status, such as sex, can be confirmed by fetal nucleated erythrocytes in maternal blood and this method might reduce such risk. Therefore, in this study, we attempted to determine the basic fetal genetic status, sex, with fetal nucleated erythrocytes. METHODS: In twelve pregnant women who undertook amniocentesis or CVS, 20 ml of venous blood was drawn immediately before the procedure and the nucleated erythrocytes were recovered by magnetic activated cell sorting(MACS). After MACS, DNA was extracted from 200 microliter of sample and single nucleated erythrocyte was obtained by additional procedure, immunostaining, and microdissection. After recovery of nucleated erythrocytes by microdissection, nested polymerase chain reaction(PCR) and fluorescent PCR of amelogenin gene were performed to identify the fetal gender. RESULTS: The DNA of enriched erythrocytes after MACS could identify the fetal gender in the 58.3% of the samples by nested PCR. After the recovery of single nucleated erythrocyte by MACS, immunostaining and microdissection, the minute DNA in a single cell could be amplified by primer extension preamplification(PEP), nested PCR, and fluorescent PCR. Fetal genders were correctly identified in 8 out of 12 (66.7 %). CONCLUSION: Through this study, we could conclude that fetal nucleated erythrocytes in maternal blood might be sufficient sample to determine fetal sex. And single cell isolation by microdissection could get the better results than nested PCR after MACS only. However, in spite of the pregnancy of male fetus, female specific bands were obtained after nested PCR of amelogenin in several cells, which might suggest that part of nucleated erythrocytes in maternal blood might be maternal origin. Therefore, to determine fetal genetic condition by nucleated erythrocytes in maternal blood, further improvements of methods to identify the nucleated erythrocytes of fetal origin are needed.

6.A Clinical Study on the Treatment of Tibial Shaft Fracture in Adult

Sixty four cases of tibial shaft fracture in adult were treated by various methods at Department of Orthopedic Surgery, Koryo General Haspital during 3 years from March 1983 to Feb. 1986. The resuIts of 'treatment of these cases were analysed according to method of treatment, type and level of fracture, and degree of injury, in attempt to seek the most suitable method of treatment. The following results were observed 1. Of 64 cases of tibial shaft fracture, 24 cases were treated by cast immobilization, 16 cases by 0/ R and plate, screw fixation, 17 cases by Ender I-M nailing, 7 cases by Hoffmans ext. fixator. 2. The most common site of fracture was middle 1/3 of the shaft (62.5%). 3. On the union time of fracture, I-M nailing by use of Ender nail was more rapid than other methods of treatment. (average: 16.2 Wks. POD) 4. On the time of weight bearing, earlier weight bearing was possible by Ender I-M nailing than other method of treatment. 5. On the complications of fracture, fewer complications were developed after Ender I-M nailing than other method of treatment. 6. According to Modified Ellis classification, more rapid union was obtained by C/R and cast imm-obilization in the case of Minor degree, no significant difference among each group in the case of Moderate degree.

8.Airway Obstrauction during Endotracheal Anestheaia - Case report.

Acute airway obstruction during endotracheal anesthesia is embarrassing and critical situation which requires early diagnosis and immediate management. The authors experienced extremely high inspiratory resistance and declared expiration time during endotracheal anesthesia for right pneumonectemy of lung cancer in the left decubitus position. A foreign material was suspected to be dislodged in the trachea distal to the endotrac-heal tube. But the surgical procedure did not allow to change the tube for examination. Ventilation was managed by rotating and moving the endotracheal tube frequently. After extubation, the patient expectorated a polypoid mass and the cause of the obstru-ction was proved to be a result of the detached and dislodged tumor mass of the lung, in diameter 3X1.5 cm.

9.Immunohistochemical Study of TGFalpha , EGF and EGF Receptor on the Epithelial Tumors of the Skin.

BACKGROUND: Several reports have demonstrated that TGFalpha and EGF are mitogenic for keratinocytes. Whenther its expression on epithelial tumors is a marker of malignancy or signifies an important step in the development of neoplasia is poorly understood. EGF receptors are also present in normal epidermis and epithelial tumors but their physiological roles are not yet understood. OBJECTIVE: Our purpose was to examine the staining patterns of TGFalpha, EGF and EGF receptors on the npithelial tumors of the skin, and to investigate kinetics of expression of EGF receptors. METHODS: We performed immunoperoxidase staining(ABC technique) with monoclonal anti-TGFalpha antibody, polyclonal anti-EGF antibody and polyclonal anti-EGF receptor antibody on the formalinfixed, paraffin-embedded tissue specimens of benign, premalignant and malignant skin tumors. RESULTS: The density of the expression of TGFalpha and EGF was not correlated with the degree of the malignancy of the epithelial tumors and is neither constant in any kind of the tumors. However the infiltrative type of basal cell carcinoma(BCC) is stronger that its solid type on the expression of TGFalpha and EGF. All benign tumors demonstrated a diffuse pattern within tumor lobules. pression of TGFalpha and EGF. All benign tumors demonstrated a diffuse pattern within tumor lobules. Focal TGFalpha immunostaining was seen in three of 10 squamous cell carcinomas(SCC) and four of 10 BCCs. TGFalpha immunostaining was absent from the outermost one to two layers of tumor lobules of all keratoacanthomas. The specimens which increased the expression of TGFalpha and EGF tended to decrease the expression of EGF receptor. CONCLUSION: These data suggest that the density of immunohistochemical expression of TGFalpha and EGF may be not dependent on the differentiation of tumor cells, and the pattern of immunohistochemical expression of TGFalpha can differentiate SCC from benign tumors such as keratoacanthoma. FGF receptor may be occupied by both of TGFalpha and EGF. With the receptors being occupied, a down regulation of the receptors may occur which results in decreased EGF receptor expression.

10.Two cases of atypical fibroxanthoma of the skin.

Atypical fibroxanthoma(AFX) occurs most on sun-exposed area of the head and neck of elderly person. It has an excellent prognosis after conservative, but complete, excision. However, because of its potential, albeit small, for metastasis, it is widely regarded as a low-grade sarcoma. We present herein two cases of atypical fibroxanthoma. The case 1 was a 86-year-old female who had a small egg-sized, dome shaped nodule with eroive surface on the left cheek. The other case was a 60-year-old male who had solitry bean-sized, nodulo-ulcerative lesion on the vertex. Immunohistochemical studies revealed positive reaction for vimntin and a-antichymo trypsin. These patients have received completely total surgical ecis on and remained free of recurrence for a period of about, 2 years follow up.
