AIM: To compare the use of the instruments' built-in normative databases, the sensitivities of time-domain optical coherence tomography (Stratus OCT) and spectral-domain OCT(Spectralis OCT) in the detection of retinal nerve fiber layer (RNFL) defects in patients with glaucoma. METHODS: Fifty-two eyes of 35 patients with open angle glaucoma were included. A total of 69 hemiretinas with photographically identified RFNL defects were analyzed using the fast RNFL scan of Stratus OCT and the circle scan in Spectralis OCT. The OCT parameters were evaluated at 5% and 1% abnormality levels using the instruments' built-in normative databases. The diagnostic sensitivity of each parameter was compared between the two devices. RESULTS: The Spectralis OCT detected RNFL defects within each quadrant more frequently than the Stratus OCT at both the 5% (79 7% vs 63 8%,P=0 01) and 1% (56 5% vs 40 6%, P = 0 01) abnormality levels. At the 1% abnormality level,the sensitivity was significantly higher in the standard sector of Spectralis OCT than in the clock-hour sector of the Stratus OCT(68 1% vs 39 1%,P<0 01). CONCLUSIONS: Using the instruments' built - in normative databases, the diagnostic sensitivity of the Spectralis OCT parameters was higher than that of the Stratus OCT parameters for detecting glaucomatous RNFL defects.

The clinical observation was performed on 32 patients of acetabular fracture, who had been admitted and treated at Department of Orthopedic Surgery, College of Medicine, Busan National University from January 1980 to December 1983. The results obtained were as follows. 1. The prevalent age ranged from 20 to 50 years, which comprised 71.9% and the ratio between males and females was 2.6: l. 2. The causes of injury were 16 cases of traffic accident, which comprised the highest incidence, 10 falling down and 6 direct blow. 3. According to Letournels classification, 21 cases (65.6%) were simple fractures, llcases(34.4%) were associated fractures. 4. Total patients associated injuries of another part of body were 24 cases#{75.0%) among them pelvic bone fracture was the most common fractured case and cerebral concussion was the most common soft tissue injury. 5. 2 types of treatment were performed, one was 18 conservative treatment and the other was 14 surgical treatment which was performed by open reduction and internal fixation. 6. The satisfactory result of conservative treatment was 61.6% and surgical treatment was 85.7. 7. The complications were 7 cases(21.9) of degenerative arthritis, ectopic ossification in 2cases (6.3%), avascular necrosis, superficial infection, and secondary displacement of fracture site in 1 case(3.1%) respectively. 8. The anatomical reduction and internal fixation was an important factor in treatment of acetabular fracture, especially in severe displaced articular fracture.

BACKGROUND: Desmopressin acetate (DDAVP) may improve hemostasis in congenital or acquired disorders of coagulation and reduce operative blood loss in patients undergoing surgical procedures. The effects of DDAVP on coagulation in hemorrhagic shock were studied in rabbits subjected to severe hemorrhage. METHODS: 20 rabbits were bled to shock and infused lactated Ringer's solution simultaneously so that the mean arterial pressure were maintained above 50 mmHg. Animals were randomly received DDAVP 0.3 ug/kg (group Dl), 3 ug/kg (group D10) or saline (group S) over 15 minutes. Bleeding time and factor VIII coagulant activity (FVIII:C) were measured before, after shock and 45 minutes after DDAVP or saline infusion. RESULTS: After shock, bleeding time was not changed but FVIII:C was decreased significantly in all groups. After saline or DDAVP infusion, bleeding time was not changed in group S and group Dl, whereas shortened in group D10 from 107.5+/-11.3 second to 87.5+/-17.5 second (P<0.05). After saline or DDAVP infusion, FVIII:C was decreased in group S and group Dl (P<0.05), but not changed in group D10. CONCLUSIONS: Infusion of DDAVP 3 ug/kg shortens bleeding time and causes an increase of FVIII:C than other groups in rabbits resuscitated from hemorrhagic shock.

OBJECTIVES: The purpose of this study was to get help in order to diagnose orthopaedic disease, measure its activity and determine treatment plan by measuring the beta-glucuronidase activity in urine, serum and joint fluid. METHODS: The beta-glucuronidase activity was determined in the serum, urine and joint fluid of the patients with degenerative arthritis, rheumatoid arthritis, osteomyelitis and osteogenic sarcoma, and some other disease to study the change of the enzyme activity. These values of each specimen were calculated by standard curve and treated by statistical analysis. RESULTS: The results obtained were summarized as follows. 1. The beta-glucuronidase activity in the serum, urine and joint fluid was increased in patients with degenerative arthritis, rheumatoid arthritis, osteomyelitis and osteogenic sarcoma etc. 2. The increased beta-glucuronidase activity in the serum and joint fluid of each disease does not show a specific finding about respective disease, but the increased beta-glucuronidase activity was statistically significant in the urine of all disease groups(male:p=0. 0041, female:p=0. 0001). CONCLUSIONS: On the basis of these results, it was suggested that beta-glucuronidase activity was affected by the orthopaedic disease and differed according to each specimen.

PURPOSE: To document the clinical features of disc hemorrhage in patients with branch retinal vein occlusion (BRVO) and normal tension glaucoma (NTG), and to evaluate the relationship between BRVO and NTG with disc hemorrhages. METHODS: From July 2001 to May 2006, sixteen patients with both NTG and BRVO in different eyes were successively collected from outpatient population of Seoul National University Hospital in this observational case series. The frequency and location of disc hemorrhages, history of associated systemic diseases, and the order of the time of diagnosis between NTG and BRVO were studied. RESULTS: All patients had unilateral BRVO, and their mean age was 63.3+/-10.6 years. Disc hemorrhages were detected in eight patients (50%) during the mean follow-up of 26.8 months (range, 3-96 months). Six patients (75%) had disc hemorrhages in the non-BRVO eyes and two patients (25%) in BRVO eyes. Five hemorrhages (62.5%) were located at inferior-temporal quadrant of the optic disc. History of systemic hypertension was identified in 12 patients (75.0%). In 11 patients (68.8%), NTG was diagnosed at the same time as BRVO. CONCLUSIONS: A higher frequency of disc hemorrhages was identified in patients with both BRVO and NTG. Therefore, some cases of NTG, especially with disc hemorrhages, may share a common vascular pathophysiology with BRVO.
