2.Percutaneous transluminal angioplsty in arteriosclerosis

Percutaneous transluminal angioplasty(PTA) is a non-operative therapeutic procedure to the dilation ofstenosis or to the recanalization of occlusion in artherosclesrotic atherosclerotic arteries using of dilatationcatheters. PTA was performed 37 times in 34 patients with arteriosclerosis for 1 year and 4 months from March.,1982 to June, 1983 at department of radiology, Klinikum Barmen/west Germany. The results were as follows; 1. Themale to female ratio was 2:1 and peak age range was from 61 to 70. 2. The most common indication was clinicalstage II b with 19 cases(51.3%) 3. PTA was performed most commonly in superficial femoral artery in 25cases(67.5%). 4. Acute major complication occurred in 2 cases(5.4%). PTA is a alternative or complementarytherapeutic procedure to vascular surgery.

3.Influence of Endothelin-1 on Cultured Vascular Smooth Muscle Cell Proliferation.

BACKGROUND: Proliferation of vascular smooth muscle cells(VSMC) is a critical event in the development of atherosclerosis. Endothelin-1(ET-1), a vasoconstrictor peptide produced by endothelial cells and VSMC, might play a role in vascular remodeling. To investigate the proposed 'mitogenic' potential of ET-1, we examined the effects of ET-1 on the proliferation if cultured porcine aortic VSMC and on the potential synergism with platelet-derived growth factor(PDGF). MATERIALS AND METHODS: VSMC were obtained from porcine aorta and cultured in Dulbecco's modified Eagle medium supplmented with 10% fetal bovine serum(FBS). VSMC grown subconfluently in 12-well plate were stimulated by ET-1, PDGF, and ET-1 & PDGF and DNA synthesis was determined as the uptake of 3H-thymidine into cell cultures. We also examined the effects of BQ123, a selective ETA receptor antagonist, and NG-methyl-L-arginine(NMLA), a nitric oxide synthase (NOS) inhibitor. RESULTS: ET-1 elicited a 2.5-fold increase of cultured VSMC DNA synthesis, comparing with basal medium, and PDGF elicited a 4.8-fold increase, whereas ET-1 and PDGF elicited a 8.8-fold increase, showing synergistic effect. Proliferative activity of ET-1 on VSMC was blocked(39%) by BQ123, however, the synergistic effect of ET-1 and PDGF was not blocked by BQ123. The synergistic effect of ET-1 and PDGF was increased when co-stimulated with NMLA. CONCLUSION: ET-1 is a co-mitogen for VSMC from porcine aorta, whose proliferative activity requires serum or other growth factors such as PDGF for its maximal activity. The proliferative activity of ET-1 is considered to be transduced partly by selective activation of the ETA receptor, however, the synergistic effect of of ET-1 and PDGF is to be stimulated by non-ETA receptor.

4.A Study on the Serum Triglyceride Values in Children.

The concentration of serum triglyceride values were studied at department of pediatrics, Chungnam National University Hospital. With the total 742 cases, among them 462 boys and 280 girls who were at the age of 2 months through 15 years old. Serum triglycerde values were measured to determine the distribution of values and evaluated on the basis of sex, age, height and weight percentile. The following results were obtained: 1) The mean value of the serum triglycerides were 88.11+/-35.76 mg/dl in boys and 94.82+/-34.56 mg/dl in girls. 2) In distribution of trglyceride level by percentile, the value of 90th percentile was 136 mg/dl. 3) The values of the serum triglyceride from 6 to 11 year old age group were significant low values in boys and girls.

6.Increased Expression of Prostaglandin H synthase by Aspirin in Cultured Cells from Amnionic Cell Line WISH Cells.

Prostaglandins which are produced from amnionic cells are known to play a major role in uterine contraction and cervical dilatation in human. Recently it is reported that aspirin increases the expression of PGHS-1 and PGHS-2 in trophoblast cells from placenta. We examined here the changes of immunoreactive prostaglandin H synthase (PGHS) level by aspirin in cultured cells from amniotic fluid and human amnionic WISH cells. PMA (10-7 M), an activator of protein kinase C increased the induction of PGHS-2 in both cells with or without fetal calf serum. PGHS-2 protein was also increased significantly by 10-4 M aspirin at 6 hours in both cells in the presence of serum but it was not increased in the absence of serum. The expression of PGHS-1 protein was enhanced by asprin but not by PMA in the absence of serum. Other anti-inflammatory drugs such as acetaminophen, indomethacin, dexame- thasone, and mefenamic acid increased the PGHS-2 protein level in WISH cells. PMA-induced PGHS-2 expression in WISH cells was not decreased by aspirin, on the contrary, the level was increased additively. Our results show that the increased expression of PGHS in amnion cells or other amniotic fluid cells by aspirin and other several anti-inflammatory drugs is through an unidentified effect rather than feedback effect by depletion of prostaglandin.

7.A Clinical Study on the Fractures of the Femoral Neck in Children

The femoral neck fracture in children is rare and occurred by severe trauma and the treatment method and prognosis are different from adult, and it is difficult to treat due to frequent complication, which is so called “unsolved fracture”. Twelve cases of children's femoral neck fracture were treated at Chon Buk National University Hospital from July, 1981 to May, 1988 were analysed clinically and radiologically and following results were obtained 1. The commonest age group was between the age of 14 and 16 years and the ratio of boys and girls was 2:l. 2. The main cause of fracture was traffic accident(6 cases) and fall down was 5 cases and slip down was 1 case. 3. According to the Delbet and Colona's classification, the transcervical fracture was most common type and displaced fracture was 8 cases. 4. Associated injuries were extremity fracture(3 cases) and pelvic bone fracture(2 cases) and head injury(2 cases), etc. 5. Seven cases were treated by open reduction and internal fixation and five cases by cast immobilization after skin or skeletal traction. 6. The commonest complication was coxa vara(5 cases) and other complication was avascular necrosis of femoral head(3 cases) premature epiphyseal closure(3 cases), nonunion (2 cases). 7. According to Ratliff's assessment of results, eight cases were good result and two fair results and two poor results. 8. Secondary operation (muscle pedicle bone graft, corrective subtrochanteric valgus osteotomy) was done in three cases and showed one good result, one fair result and one poor result.

Purpose:
To design a scoring system to predict malignancy of additional MRI-detected lesions in breast cancer patients.
Materials and Methods:
Eighty-six lesions (64 benign and 22 malignant) detected on preoperative MRI of 68 breast cancer patients were retrospectively included. The clinico-radiologic features were correlated with the histopathologic results using the Student's t-test, Fisher's exact test, and logistic regression analysis. The scoring system was designed based on the significant predictive features of malignancy, and its diagnostic performance was compared with that of the Breast Imaging-Reporting and Data System (BI-RADS) category.
Results:
Lesion size ≥ 8 mm (p < 0.001), location in the same quadrant as the primary cancer (p = 0.005), delayed plateau kinetics (p = 0.010), T2 isointense (p = 0.034) and hypointense (p = 0.024) signals, and irregular mass shape (p = 0.028) were associated with malignancy. In comparison with the BI-RADS category, the scoring system based on these features with suspicious non-mass internal enhancement increased the diagnostic performance (area under the receiver operating characteristic curve: 0.918 vs. 0.727) and detected three false-negative cases. With this scoring system, 22 second-look ultrasound examinations (22/66, 33.3%) could have been avoided.
Conclusion
The scoring system based on the lesion size, location relative to the primary cancer, delayed kinetic features, T2 signal intensity, mass shape, and non-mass internal enhancement can provide a more accurate approach to evaluate MRI-detected lesions in breast cancer patients.

10.Congenital Adrenogenital Syndrome: Two Cases Report.
