5.Diganostic Values of Bone Scan Followed by CT Scan in Undetected Pelvic bone Fracture.

Pelvic bone is complex in its shape, so we often miss to find the fractures in cases with unclear fracture line. We studied 25 patients who were suspected to have occult fractures in pelvic area. We were able to find lots of occult fractures with bone scan followed by CT scan. The results were as follows; 1. There were 31 unexpected hot spots in 25 patients in bone scan. 2. Six cases (19.3%j of new injuries were found on re-evaluation of plain film or additionally rechecked plain films. 3. Twenty-one new fractures (67.7%) were found on CT film study. 4. Total 27 additional fractures (89.1%) were found after bone scan followed CT scan. In conclusion Bone Scan followed by CT is a effective method of detecting occult fracture in pelvic area.

6.The bone formation around anodic oxidized titanium implants in the tinbiae of ovarectomized rats.

Anodic spark deposition method(ASD) surface treated titanium implant possesses a considerable osteoconductive potential that promoting a high level of implant osseointegration in normal bone. The purpose of this study was to observe the ASD implant's osseointegration in the osteoporosis-induced animal model. Twenty four rats, 10 weeks of age, were ovarectomized and 5 weeks later divided into two groups : ASD implant group and control implant group. Titanium screw implants (diameter; 2.0 mm, length, 3.5 mm; pitch-height, 0.4 mm) were designed for this study. Experimental implants were ASD treated and no treatment on control implants. ASD implants and control implants were placed in to left tibiae of rats. The rats were sacrificed at different time interval(1, 2, 4 and 8 weeks after implantation) for histopathologic observation and immunohisto -chemistrical observation, with collagen type I, fibronectin, integrin alpha2beta1 and integrin alpha5beta1 antibodies. The results obtained from this study were as follow: 1. Histopathologic findings, overall tissue response and the pattern of bone formation in both groups were similar. In ASD group, more newly formed bone was seen at 1 week and 2weeks than control group. 2. The levels of type I collagen and fibronectin expression were the most abundant at 2weeks and decreased gradually in both groups. Fibronectin and type I collagen expression in ASD group were stronger than control group but no significance. 3. The levels of integrin alpha2beta1 and Integrin alpha5beta1 expression were most abundant at 2 weeks and decreased gradually in both groups. No significant difference was observed in both groups. From this results, anodic oxidized titanium implants were more advantages in early stage of bone formation than control group, but have no significance in tissue responses and late bone formations. It could be stated that although anodic oxidized titanium implant possesses considerable osteoconductive potential but in osteoporotic bone condition dental implant procedure should performed after improving or treating the osteoporotic bone condition.

7.A Clinical and Statistical Study on Pregnancy of Elderly Primipara.

OBJECTIVE: Recently, the incidence of elderly primipara is increasing. Thus, impact of old age on pregnancy outcomes became important. Our purpose was to assess the effect of maternal age on pregnancy outcomes. METHODS: 73 cases of the elderly primipara over 35 years at the department of obstetrics and gynecology, Chon-ju presbyterian medical center, from Jan. 1, 1995 to Dec. 31, 1998, were compared with 73 cases randomly chosen young primiparas during same period. RESULTS: Elderly primipara had higher rate of infertility and spontaneous abortion history, antenatal complications, cesarean section, poorer neonatal outcome than young primipara. CONCLUSION: The pregnancy outcome of elderly primipara is considered to be less favorable than those of young primipara. Careful antenatal and intrapartal care should be performed for the elderly primipara. We think that many clinical study of congenital anomaly in elderly primipara will be necessary.

8.Comparisons of Corneal Topographic Change Between 20-Gauge and 23-Gauge Pars Plana Vitrectomy.

PURPOSE: To compare the changes in corneal topography between 23 gauge (G) pars plana vitrectomy (PPV) and 20 G PPV. METHODS: Forty-three eyes of 42 patients who underwent PPV were divided into 2 groups: 20G PPV (Group I, n=18) and 23 G PPV (Group II, n=25). Both groups were followed up over 3 months postoperatively. ORBSCAN II was performed to compare the corneal topographic change of 20 G PPV and 23 G PPV. RESULTS: In a 5 mm zone, significant postoperative change in the mean corneal surface cylinder was found between the 1st day and the 1st month in Group I, while no significant change was found in Group II. Postoperatively, the increase in the mean corneal surface cylinder in Group I was significantly higher than in Group II between the 1st day and the 1st month. Clinically significant astigmatism, induced by surgery, was found only in Group I one week after the operation. In a 3 mm zone, there was no significant change in the mean corneal surface cylinder in both groups postoperatively. The difference in the mean corneal surface cylinder between the 2 groups at the postoperative periods showed no significant change. Surgically induced astigmatism was significantly higher in Group I than in Group II between the 1st day and the 1st month postoperatively. CONCLUSIONS: PPV with the 23 G system is a sutureless transconjunctival surgery, which appears to be more effective due to insignificant corneal surface and astigmatic changes in the early postoperative period.

Leiomyoma is a neoplasm of smooth muscle relatively common in the stomach, but very rare in the duodenal location. Recurrent bouts of severe melana remain the most frequent presenting symptom for leiomyoma. A 69-year-old man presented to us with a 4-day history of melena. Endoscopy revealed norinal mucosa covered ovoid elevated lesion with central linear depression and ulcer at the second portion of duodenum. Endoscopic biopsy showed chronic inflammation only. Duodenal mass was excised through a duodenotomy. Pathological diagnosis was leiomycena.

10.Effect of Salicylate on DPOAEs and ABRs in Guinea Pigs.

BACKGROUND AND OBJECTIVES: Salicylates are well-known for producing reversible hearing loss and tinnitus. However, the site and mechanism of salicylate ototoxicity remain unresolved. Recent experiments suggest that reversible biochemical and/or metabolic changes in the cochlea seem to play an important role in salicylate ototoxicity. The purpose of this study was to investigate the site of lesion in salicylate ototoxicity by audiometric study. MATERIALS AND METHOD: ABRs and DPOAEs were observed after intraperitoneal injection of 500 mg/kg of sodium salicylate on 24 ears of guinea pigs. RESULTS: Salicylate produced a significant increase in the ABR threshold. Maximum changes were obtained in 4 hours, and recovered to the baseline in 24 hours after salicylate administration. The pattern of hearing loss shown by latency-intensity function was compatible with the cochlear type of hearing loss. The echo amplitude on DPOAEs at f2=2002, 4004 Hz was significantly decreased at 2, 4, 6, 8 hours, and returned to the baseline in 24 hours after salicylate administration. The time course of the change of DPOAEs was parallel with that of ABRs. CONCLUSION: These results reflect that the cochlear outer hair cells may be the main site of lesion in salicylate ototoxicity.
