1.Treatment of Palmoplantar Hyperhidrosis with Iontophoresis.

In the present study, tap water iontophoresis as a method of treatment of palmoplantar hyperhidrosis was evaluated. Eighteen patients, twelve males and six females, with singnificant palmoplantar hyperhidrosis were treated on every weekday for 2 weeks. The hand and foot of the same side were randomly selected for the treatment by tap water iontophoresis according to the the method described by Levit. The selected hand and foot were immersed in a pair of pans containing tap water and exposed to 30V and 3 to 14mA of direct current for 20 minutes. The results obtained were as follows . Among 18 patients treated, 14(77.8%) accomplishei3 sufficient control of hyperhidrosis after 6 12(average 9.1) treatments. Two patients showed a moderate response and one weak response. One patient showed no response. Several patients experienced untoward effects such as mild itching, erythema, tingling sensation, exfoliation, compensatory hyperhidrosis, but the discomfort was not so severe as to stop the treatment

2.A Clinical Study on Trochanteric Fractures of the Femur

Fifty seven consecutive trochanteric hip fracture treated with Smith-Petersen nail and Thornton plate who were treated at the Department of Orthopedic Surgery, St. Marys Hospital Catholic Medical College, during the period of January 1969 to September 1975 were reviewed. This covered a period of six years and nine months. Of the fifty seven cases treated with this method, only thirty four cases were able to be analysed completely. Of these thirty four cases, in two cases the nail penetrated to the hip joint, four cases showed a loosening bolt of the S-P nail, one case involved a delayed union and three cases resulted in malunion, showing a total failure rate of 29.4%. In addition, four cases of intertrochanteric hip fractures of children were reviewed. 1) The sex ratio was 2.1:1 (male to female). Those in their fifth decade showed the highest incidence at 26.3%, next came those in their third decade. 2) The most common cause of the trochanteric hip fractures was traffic accidents (57.9%), other cause were slipping or falling down, direct trauma and pathological fractures. 3) The ratio of left trochanteric hip fractures to right was 7:6. 4) In the group which received external immobilization with Buck's extension skin traction and delayed weight bearing method, the results were better than that group to which external immobilization was not applied and early weight bearing was allowed. 5) The group which received the external immobilization and delayed weight bearing method showed a total failure rate of 21.8% 6) The group to which external immobilization was not applied and early weight bearing was allowed a total failure rate of 64%.

3.Arthroscopy after Meniscectomy of the Knee: The Posterior Horn in 40 Cases

Diagnosis of the disorders following menlscectomy is occasionally difficult, as ther has frequently been involved many causes other than the torn menisci. The retained fragments of the menisci have been blamed to be responsible for postoperative knee disorders. Arthroscopic examination was carried out in 40 knees after meniscectomy and the frequency of retained posterior horn in relation to the postoperative trouble was observed. 1. In 11 out of total 40 knees, the retained posterior horn could be diagnosed, consisting of 27.5% of all. 2. Only flve of these 11 knees showed some kinds of postoperative disorders,while the reat had no trouble. 3. Those knees in which the excision of the posterior horn was incomplete, were often associated with Irregular healing pattern of the local tissue, which might be closely related to the occurrence of the secondary Intraarticular change.

4.Two Cases of Extramammary Paget' s Disease Showing a Good Response to Radiotherapy.

We report the clinical evolution of two elderly male patients with extramammary Paget's disease who were treated with radiotherapy. Both cases primarily involved the scrotum and penile root, and were not associated with any underlying or visceral malignancy. The total doses administered were 6000 cGy and 3000 cGy, respectively. A good clinical response was achieved in both cases. No major side effects were observed. The patients are being followed for any signs of recurrence and have been completely asymptomatic to date

7.The Maturation of Circulating Primitive Erythroblasts in Human Embryos. A Karyometrical Study.

The primitive erythroblasts in 21 cases of embryonic hearts from 4 to 9 weeks of gestation were studied with a light microscope. The nuclear diameter, the motosis, and the loss of nuclei of the primitive erythoblasts were analyzed quantitatively. The results obtained were as follows. 1. At 4 weeks of gestation, the blood cells consisted of proerythroblasts, along with basophilic polychromatophilic primitive erythroblasts. The nuclear diameter ranged from 3.20 µm to 9.20 µm, but the main range was from 4.20 µm to 6.00 µm. It was revealed that 9.50% had diameter of more than 6 µm. 2. At the fist half of the 7 week gestation when hepatic hemopoiesis developed, the blood cells consisted of basophilic, polychromatophilic, and eosinophilic erythroblasts. Cells of more than 6 µm in nuclear diameter were about 1.10% and thereafter gradually disappeared. The range of the nuclear diameters was from 2.60 µm to 7.00 µm, while a range from 3.40 µm to 5.20 µm wqs the main. The proportion of cells less than 4 µm in nuclear diameter was 39.58% and thereafter rapidly increased. 3. From the second half of 7 weeks to 9 weeks of gestation, the erythrocytes originating from hepatic hemopoiesis increasingly replaced the circulating primitive erythroblasts, which became mature during this time. The erythrocytes showed 72.88% at 9 weeks of gestation. The proportions of cells less than 4 µm in nuclear diameter in the first and second haIves of 8 weeks and 9 weeks were 52.73%, 80.02%, and 89.09%, which represented the rapid destruction of nuclei. 4. Mitosis in the primitive erythroblasts occurred principally up to the early 6th weeks, and very weakly at 8 weeks. 5. As the crown-rump length increased, the average nuclear diameter decreased very significantly (P<0.01, y=-0.2811X + 0.3171). The results suggest that distrilbution of the nuclear diameter, the maturity, the rate of nuclear loss, and the mitotic figure offer credible data for estimating embryonic age.
