1.Clinical Analysis of 137 Eyes of Retinal Detachment Surgery with Drainage of Subretinal Fluid.

Subretinal fluid drainage produces the indentation of the sclera by facilitating firm adhesion between the retina and either retinal pigment epithelium or Bruch's membrane and has the advantage that show the state of fundus and degree of indentation of sclera accurately during the operation. Becaulse it has complications such as choroidal bleeding, retinal incarceration, iatrogenic retinal break, choroidal detachment and loss of vitreous, it should be done carefully. The authors analyzed the clinical data of 128 cases(137 eyes) of retinal detachment surgery with subretinal fluid drainage at the Department of Ophthalmology, Pusan Paik Hospital, College of medicine, Inje University from May, 1988 to April, 1990. And the results were as follows: 1. The most common operative method of rhegmatogenous retinal detachment were explant buckling and encirclement with subretinal fluid drainage, and the success rate of first operation was 86.13% and the overall success rate of rhegmatogemous retinal detachment was 91.2% in 137 eyes of operation. 2. In 116 cases(92.8%) who were done subretinal fluid drainage, absorption of residual subretinal fluid took in a day. 3. The most common cause of surgical failure was proliferative vitreoretinopathy. 4. The most common complication was choroidal hemorrhage and it was developed 14 eyes(10.2%) mong 137 eyes. 5. The preoperative duration of symptom and extent of detachment were influenced to the surgical success rate and they had statistical significance(p<0.05). 6. The power of refraction was changed to myopia and ERG b-wave amplitude were increased and 89 eyes out of 137 eyes(69.6%) showed an improvement of vision at the three months after retinal detachment surgery.

3.A Study of Psychiatric Treatment Compliance in Referred Patients at a General Hospital.

OBJECTIVES: The present study investigates the status of inpatient psychiatric consultations at a general hospital in order to find factors that contribute to treatment compliance related to psychiatric consultations. METHODS: The subjects were 333 patients who were hospitalized at Korea University Medical Center Ansan Hospital from 1 September 2009 to 31 July 2010.The patients were referred for psychiatric consultation during hospitalization. This study investigates demographic data, request department, referral causes, requestor, psychiatric history and diagnosis, andpsychiatric treatment compliance. Treatment compliance was defined as whether or not the patient had accepted psychiatric treatment during hospitalization or outpatient department(OPD) follow-up. This study ascertains the factors that have impact on compliance, by taking binary logistic regression with compliance and other variables. RESULTS: Among the patients that were offered psychiatric treatment during hospitalization(N=310), treatment compliance was 82.9%. Among the patients that were offered OPD treatment(N=111), compliance was 55.8%. Elderly group(>65 years) showed better compliance to treatment during hospitalization than the younger patient group(OR=4.838, p=0.004). Patients with secondary psychiatric disorders showed better OPD follow-up compliance than patients with secondary psychiatric disorders(OR=8.520, p=.008). CONCLUSION: Elderly patients showed better compliance for psychiatric treatment during hospitalization. However they commonly have disorders such as delirium and mood disorders that have impact on the patient's physical state, hence further active measures should be carried out. Patients referred due to primary psychiatric disorders showed poor OPD compliance. Therefore clinicians have to suggest multidisciplinary interventions that will improve treatment compliance of such patients.

4.The Stress Perception, Depressive Symptoms and Medical Comorbidity in Healthcare Center.

OBJECTIVES: We surveyed this study for knowing the relation within stress, depression and medical comorbidities, and finding the risk factors of major depression. METHODS: 1764 subjects were enrolled from Jan. 2009 to Dec. 2009 who visit Korea University Guro hospital healthcare center. The subjects answered the questionnaire of PSS(Perceived Stress Scale), PHQ-9(Patient Health Questionnaire-9) and the demographic data. We categorized them as the stress group, depressive group, medical comorbidity group and analyzed the correlation analyses and logistic regression analyses. RESULTS: 198 of 1764 subjects(11.8%) were applied to major depression, and the depressive group showed the higher mean stress score(23.19) and mean depression score(12.95) than the normal group. The total PHQ-9 score was increased by perceiving more stress, having more medical comorbidities. The subjects with female, visiting due to recent health problems, irregular exercise, current smoking, history of angina and cerebrovascular disease showed the increased risk of major depression. CONCLUSIONS: In this study, we find the PSS, PHQ-9 were valuable for mental health screening in healthcare center. As perceiving more stress and having more medical comorbidity, risk of major depression were increased. Accordingly the individuals with medical diseases or unhealthy lifestyle would need the mental health screen.

During the period from July 1975 to June 1982 we performed 17 Putti-Platt operations on 16 males and one female, ranging from 19 to 39 years. The average follow-up period was 3.8 years in 13 cases of them. The results were as follows: 1. The ages in initial dislocation were ranged from 14 to 31 with an average of 18.5 years. 2. Right side was involved in 15 cases and. left in 2 cases. 3. On radiological finding Hill-Sachs lesion was found in 41%.0n pathological finding at operation of 15 cases, Bankart lesion was seen in 73%, Hill-Sachs lesion in 60%, glenoid erosion in 33%, capsular loosening in 33%, and capsular tear in 13%. 4. Limitation of external rotation compared with sound opposite side was measured in 12 to 40 with an average of 19.2 degrees. 5. The Putti-Platt operation is considered as sound, surgical procedure that gives excellent functional results.

8.A Study on the General Public’s Perceptions of Dental Fear Using Unstructured Big Data

Background:
This study used text mining techniques to determine public perceptions of dental fear, extracted keywords relatedto dental fear, identified the connection between the keywords, and categorized and visualized perceptions related to dental fear.
Methods:
Keywords in texts posted on Internet portal sites (NAVER and Google) between 1 January, 2000, and 31 December, 2022,were collected. The four stages of analysis were used to explore the keywords: frequency analysis, term frequency-inverse document frequency (TF-IDF), centrality analysis and co-occurrence analysis, and convergent correlations.
Results:
In the top ten keywords based on frequency analysis, the most frequently used keyword was ‘treatment,’ followed by ‘fear,’ ‘dental implant,’ ‘conscious sedation,’ ‘pain,’ ‘dental fear,’ ‘comfort,’ ‘taking medication,’ ‘experience,’ and ‘tooth.’ In the TF-IDF analysis, the top three keywords were dental implant, conscious sedation, and dental fear. The co-occurrence analysis was used to explore keywords that appear together and showed that ‘fear and treatment’ and ‘treatment and pain’ appeared the most frequently.
Conclusion
Texts collected via unstructured big data were analyzed to identify general perceptions related to dental fear, andthis study is valuable as a source data for understanding public perceptions of dental fear by grouping associated keywords. The results of this study will be helpful to understand dental fear and used as factors affecting oral health in the future.
9.MR Appearance of Synovial Plica in Patients with Plica Syndrome and Normal Plica.

PURPOSE: To compare MRI appearance between plicae syndrome and normal plicae. MATERIALS AND METHODS: MR images of 60 cases of arthroscopically-confirmed plicae syndrome and 18 of arthroscopically-proven normal plicae were retrospectively analyzed. Sagittal T2-weighted MR images in all cases and MPGR(200) in 37 cases of plicae syndrome were obtained. Statistical analysis was performed using the chi-square test. RESULTS: On the basis of operatingresults, we observed 55 medial plicae, eight combined medial and suprapatellar plicae, four suprapatellar plicae,and one lateral plica. T2-weighted sagittal MR scans of the 60 cases demonstrated 37 medial plicae, 8 suprapatellar and one lateral plica. Joint effusion was found in 26 cases of 55 medial plicae. In T2-weighted sagittal MR scans, the identification of medial plicae was superior in the presence of joint effusion than its absence(plicae syndrome, p < 0.001 ; normal plicae group, p < 0.05). Medial plicae were well demonstrated onMPGR(200) axial images; on T2-weighted sagittal MR scans, they could be more frequently identified in the plicae syndrome group than in the normal control group(p < 0.001). Plicae syndrome-associated pathology included degenerative change of the articular cartilage of the medial femoral condyle in eight cases(14.5%), discoidmeniscus in nine(16.4%), lateral meniscus tear in 12(21.8%), medial meniscus tear in 21(38.1%), anterior cruciate ligament tear in three(5.5%), medial collateral ligament tear and osteochondritis dissecans in one case. CONCLUSION: The present study revealed that synovial plicae were well demonstrated in T2-weighted sagittal images, particularly on the presence of joint effusion. Medial plicae could be more frequently identified in the plicae syndrome group than in the normal control group, especially on T2-weighted sagittal MR scans.

10.High-Resolution MRI Study on Mouse Brain Using Micro-Imaging.

PURPOSE: By using the micro-imaging unit modified from NMR spectrometer, the high resolution MRI protocols of finer than 100 micron in 5 minutes, is sought for mouse, which plays a central role in animal studies MATERIALS AND METHODS: C57BL/6 mouse, lighter than 50 gram, is used for the experiments. The superconducting magnet is vertical type with 89 mm inner diameter at 4.9 Tesla. The diameter of rf-coil is 30 mm. Mostly used techniques are the fast spin echo and the gradient echo pulse sequence. RESULTS: For 2D images, proton density and T2 weighted images are obtained and their optimum experimental variables were sought. Minute structure of mouse brain can be recognized and 3D brain image is also obtained additionally. 3D image will be useful particularly for the dynamic contrast study using various contrast agents. CONCLUSION: Like the case of human and other small animals, the high resolution of mouse brain is enough to recognize the minute structure of it. Recently, similar studies are reported domestically, but it seems only a beginning stage. Due to easiness of breeding /control, mouse MRI study will soon play a vital part in brain study.
