1.Correlation of CT Findings and Pathologic Nuclear Grading in Renal Cell Carcinoma.

PURPOSE: To correlate the CT findings of renal cell carcinomas with nuclear grading in histopathology. MATERIALS AND METHODS: Preoperative CT scans and pathologic nuclear grading of 60 surgically resected renal cell carcinomas in 55 patients were retrospectively and independently reviewed. RESULTS: As nuclear grade increased, renal cell carcinomas were more likely to be of higher stage(92% of nuclear grade III renal cell carcinomas was of stage Ill, all Grade IV tumors were of stage IIIb or higher) and greater size(84% of grade III tumors and 100% of grade IV tumors>5cm in size) at presentation, and appeared more heterogeneous(84% of grade III tumors and 88% of grade IV tumors showed moderate or severe heterogeneity) and less well marginated(84% of grade Ill tumors and 100% of grade IV tumors had irregular or imperceptible margins). CONCLUSION: Small well-marginated homogeneous renal cell carcinomas were either nuclear grade I or II, and were distinguishable from the more aggressive nuclear grade III or IV lesions, which generally displayed irregular margins and greater inhomogeneity.

2.Comparative Study of the Roentgenographic Methods for the Measurement of the Femoral Anteversion

The angle of the femoral anteverison has been recognized as an important consideration in the orthopedic Geld. The angle of the femoral anteversion can be measured roentgenographically by 3 methods, such as fluoroscopic, biplanar and axial methods. It is the purpose of this study to investigate the comparative accuracy of the roentgenographic methods determining the angle of the femoral anteversion. The measurements by the fluoroscopic(Rogers), biplanar (Magilligan) and axial (Dunn) methods were compared with the direct measurement of 42 adult dried femora and the measured values were statistically analyzed. The results were as follows: l. Among fluoroscopic, biplanar and axial methods, the biplanar method using Magilligan technique was the statistically reliable method to determine the angle of the femoral anteversion. 2. The axial method showed tendency to underestimate the angle of the femoral anteversion. 3. Though the fluoroscopic method was not so accurate as the biplanar method statistically, the 2 methods showed no difference in the ratio of measurement error within ±5°and ±10° range.

4.Nurses' Perception of Organizational Commitment, Nursing Work Environment, and Social Support in a General Hospital.

PURPOSE: This study was done to identify the influence of nursing work environment and social support on multidimensional organizational commitment among nurses. METHODS: The survey was conducted in August 2012 with self-report questionnaire. Participants were 480 registered nurses working in one general hospital. Data were analyzed with descriptive statistics, t-test, ANOVA, Pearson correlation, and multiple regression using SPSS/WIN(16.0). RESULTS: The average scores were; for affective commitment 2.84, for continuance commitment 2.48, and for normative commitment 2.57. There were significant relationships between affective, continuance, normative commitment and sub-factors of nursing work environment and social support. The affective, continuance, and normative commitment were all influenced by 'staffing and resource adequacy' and 'nursing foundations for quality of care', sub-factors of nursing work environment. Distinctively, affective commitment was influenced by 'staffing and resource adequacy', nurses' age, 'nurse-doctor relations', 'supervisor's emotional support', 'nurse participation in hospital affairs', 'nursing foundations for quality of care', and 'supervisor's informational support', which explained 30.9% of variance in affective commitment(F=31.57, p<.001). CONCLUSION: The findings show that programs which promote supervisors' emotional and informational support are important to enhance nurses' affective commitment. Also, it is necessary to improve nursing work environment to improve nurses' organizational commitment.

5.Upregulation of Proinflammatory Cytokines in the Fetal Brain of the Gaucher Mouse.

Gaucher disease is caused by a deficiency of glucocerebrosidase. Patients with Gaucher disease are divided into three major phenotypes: chronic nonneuronopathic, acute neuronopathic, and chronic neuronopathic, based on symptoms of the nervous system, the severity of symptoms, and the age of disease onset. The characteristics of patients with acute neuronopathic- and chronic neuronopathic-type Gaucher disease include oculomotor abnormalities, bulbar signs, limb rigidity, seizures and occasional choreoathetoid movements, and neuronal loss. However, the mechanisms leading to the neurodegeneration of this disorder remain unknown. To investigate brain dysfunction in Gaucher disease, we studied the possible role of inflammation in neurodegeneration during development of Gaucher disease in a mouse model. Elevated levels of the proinflammatory cytokines, IL-1alpha, IL-1beta, IL-6, and TNF-alpha, were detected in the fetal brains of Gaucher mice. Moreover, the levels of secreted nitric oxide and reactive oxygen species in the brains of Gaucher mice were higher than in wild-type mice. Thus, accumulated glucocerebroside or glucosylsphingosine, caused by glucocerebrosidase deficiency, may mediate brain inflammation in the Gaucher mouse via the elevation of proinflammatory cytokines, nitric oxide, and reactive oxygen species.

9.Current Trends of the Hyperbilirubinemia and the Results of Auditory Evoked Potential.

OBJECTIVE : The purpose of this study was to identify the current trends of the incidence of hyperbilirubinemia and the results of the auditory evoked potential and to evaluate the correlation with early and late evidence of bilirubin-induced brain injury. METHODS : We reviewed the medical records of all neonates who were admitted to the Holy family hospital of CUMC exclusively for management of neonatal jaundice from January, 2002 to December, 2006. The gestational age, the age at peak bilirubin level, the peak bilirubin level, feeding method and brainstem auditory-evoked potential (AEP) were reviewed. RESULTS : We investigated 121 term and 101 preterm infants treated for management of neonatal jaundice. The mean peak bilirubin values of term and preterm infants were 22.7+/-0.5 and 13.7+/-0.3 mg/dL. The mean age at peak bilirubin level of term and preterm infants were 7.5+/-0.5 and 4.7+/-0.2 days. All were breastfed (46.3% vs. 30.7%) or fed a combination of breast and bottle feedings (53.7% vs. 44.5%) in term and preterm infants. Comparison of peak bilirubin levels and age at peak bilirubin level in the serum of term and preterm group with AEP results showed no significant relationship. Trend in peak serum indirect bilirubin level in term infants were significantly increased in the recent 3 years. Trend in AEP abnormality showed no significant changes during the study periods. Only two preterm infant showed abnormal AEP at follow up study. CONCLUSION : Recently, increased frequency of breast-feeding, shortened hospital stays, and inconsistent follow-up after hospital discharge may cause the increased incidence of severe hyperbilirubinemia in healthy preterm and term newborn infants. But it is not clear such early onset hyperbilirubinemia may cause acute bilirubin encephalopathy or permanent brain injury.

10.A Case of Multiple Nevus Sebaceus.

The nevus sebaceus is an organoid nevus generally noted on the scalp or the face as a single lesion. The frequent association of nevus sebaceus with other appendage tumors and with apocrine glands suggests that nevus sebaceus is derived from the primary epithelial germ. We report a 20-day-old boy with multiple nevus sebaceus on his face, scalp and external ear. The lesions were symmetrical and midline involved yellowish plaques. On the microscopic view of biopsy specimen from the face and scalp, there is a dilated keratin-filled infundibula showing a few buds of undifferentiated cells representing malformed hair germs. The dermis contains many fibroblasts and immature hair structures. Numerous sebaceous glands and eccrine glands lie in upper dermis. Diagnosis was confirmed by clinical appearance and histological picture.
