1.Mesenchymal Cell Tumors of the Gastrointestinal Tract.

Mesenchymal cell tumors of the intestinal tract originate from connective tissues, muscles, fat, lymphatic tissues, blood vessels, and nerves in and beneath the submucosa. The most common site is the stomach, followed by the small bowel. Among the benign gastric neoplasms of mesodermal origin, those derived from smooth muscles constitute over 90%. Leiomyosarcomas of the stomach represent about 1% to 3% of primary malignant tumors and approximately 20% of submucosal tumors of the stomach. In the small bowel, leiomyoma is the most common benign tumor, and leiomyosarcoma represents about 19% of malignant tumors. The principle of surgical treatment of gastric mesenchymal tumors is local excision with a 2 to 3 cm margin in the surrounding gastric wall. We reviewed 48 cases of mesenchymal cell tumors of the gastrointestinal tract from January 1988 to March 1997 at the Department of Surgery, Namkwang General Hospital and Chonnam University Hospital School. The results are as follows : 1) There were 23 benign and 25 malignant tumors. 2) The most common benign tumor was leiomyoma (41.7%) and the most common malignant tumor was a leiomyosarcoma (47.9%). 3) The most common site was the stomach (52.1%), followed by the small bowel (25%); in the gastric tumor cases, the most common site was the body (60%). 4) The most common clinical manifestation was abdominal pain (37.5%), followed by bleeding (27.1%), a palpable mass, dyspepsia, and weight loss. 5) A wedge resection, a subtotal gastrectomy, or an enucleation of the mass was mainly performed on the stomach. Segmental resection and anastomosis was primarily performed in the small bowel. 6) Among the malignant tumors, a recurrence was noted in 8 patients(32%) : 7 leiomyosarcomas and 1 malignant histiocytoma. 7) The prognosis for a malignant leiomyosarcoma was better than that for an adenocarcinoma and was associated with tumor size and histologic grade (mitosis/10 HFP). However, we could not find any correlation between the prognosis and lymph node metastasis.

2.Implantation of ICD in a Ventricular Fibrillation Patient with a High Defibrillation Threshold.

ICD (implantable cardioverter-defibrillator) has been recently accepted as the most effective treatment for patients with medically refractory or life threatening ventricular tachycardia or ventricular fibrillation. Determination of the DFT (defibrillation threshold) is important in the implantation of ICD. DFT is the lowest shock energy that successfully terminates ventricular fibrillation. Preoperative amiodarone use, increased body size, and left ventricular dilatation are associated with high DFT. Recently, we experienced a 66 year-old male patient presenting with syncope due to idiopathic ventricular fibrillation in whom DFT was high and unstable, probably due to preoperative use of amiodarone.

3.A Case Study Related to Identifying the Driver of Motorcycle.

It is a frequently asked question who was the driver of a vehicle when a traffic accident occur. To give an expert opinion who was the driver of vehicle, expert witness must inspect not only trace evidences remained in vehicle and occupant's clothing but also occupant's injuries. Thus, the collaboration of forensic engineering and forensic medicine is very fruitful in this kind of case. Due to open sitting position of occupants, deciding the driver of motorcycle is a very difficult and case dependent problem and it is very rare that the driver of motorcycle can be decided only by the estimation of occupant's injuries. In this report, we introduce the case that the driver of motorcycle can be decided by the collaboration of forensic engineering and forensic medicine.

5.Monitoring Culicine Mosquitoes (Diptera: Culicidae) as a Vector of Flavivirus in Incheon Metropolitan City and Hwaseong-Si, Gyeonggi-Do, Korea, during 2019

The flaviviruses are small single-stranded RNA viruses that are typically transmitted by mosquitoes or tick vectors and are etiological agents of acute zoonotic infections. The viruses are found around the world and account for significant cases of human diseases. We investigated population of culicine mosquitoes in central region of Korean Peninsula, Incheon Metropolitan City and Hwaseong-si. Aedes vexans nipponii was the most frequently collected mosquitoes (56.5%), followed by Ochlerotatus dorsalis (23.6%), Anopheles spp. (10.9%), and Culex pipiens complex (5.9%). In rural regions of Hwaseong, Aedes vexans nipponii was the highest population (62.9%), followed by Ochlerotatus dorsalis (23.9%) and Anopheles spp. (12.0%). In another rural region of Incheon (habitat of migratory birds), Culex pipiens complex was the highest population (31.4%), followed by Ochlerotatus dorsalis (30.5%), and Aedes vexans vexans (27.5%). Culex pipiens complex was the predominant species in the urban region (84.7%). Culicine mosquitoes were identified at the species level, pooled up to 30 mosquitoes each, and tested for flaviviral RNA using the SYBR Green-based RT-PCR and confirmed by cDNA sequencing. Three of the assayed 2,683 pools (989 pools without Anopheles spp.) were positive for Culex flaviviruses, an insect-specific virus, from Culex pipiens pallens collected at the habitats for migratory birds in Incheon. The maximum likelihood estimation (the estimated number) for Culex pipiens pallens positive for Culex flavivirus was 25. Although viruses responsible for mosquito-borne diseases were not identified, we encourage intensified monitoring and long-term surveillance of both vector and viruses in the interest of global public health.
