1.MR Findings of Cervical Lymphadenopathy: Tuberculosis Versus Metastasis.

PURPOSE: To determine the MR findings of cervical lymphadenopathy which distinguish tuberculouslymphadenitis(TL) from metastatic lymphadenopathy(ML). MATERIALS AND METHODS: We retrospectively analyzed the MRfindings of 14 patients with TL and 19 with ML. Thirty-nine lesions in 14 patients with TL and 80 lesions in 19with ML were classified into three categories: conglomerate lesion (TL, 7; ML, 8); discrete lesion withoutnecrosis (TL, 3; ML, 15); and discrete lesion with necrosis (TL, 29; ML, 57). Between TL and ML, we compared age,sex, bilaterality, multiplicity, associated pulmonary tuberculosis, infiltration of adjacent fat, muscle, skin andsubcutaneous tissue, margin, and signal intensities on T1- and T2-weighted images. In discrete lesion, we alsocompared the minimal to maximal dimension ratio, and in discrete necrotic lesion, the thickness and evenness ofrim enhancement, and the location and extent of necrosis. RESULTS: There were statistically significantdifferences between TL and ML in age(34.7+/-13.1 years; 56.1+/-14.3 years), male to female ratio(3:11; 13:6),bilaterality(1/14; 10/19), multiplicity(5/14; 14/19), and associated active pulmonary tuberculosis(7/14; 1/19). OnT2-weighted images, TL showed predominantly homogeneous (30/39) or high (20/39) signal intensity, similar to thatof cerebrospinal fluid, but the signal inteusity of ML was predominantly heterogeneous(58/80), or lower than orsimilar to that of fat(41/80). Between TL and ML, however, there were no statistically significant differences ininfiltration of adjacent tissue, margin, signal intensity and its heterogeneity on T1-weighted image, minimal tomaximal dimension ratio, thickness and evenness of rim enhancement, or the location and extent of necrosis. CONCLUSION: In cervical lymphadenopathy, the existence of TL rather than ML is suggested by single or unilaterallesion in a young woman, by associated active pulmonary tuberculosis, or by homogeneous high signal intensity onT2-weighted MR images.

2.A Case Report of Correction of Mandibular Prognathism by Intraoral Oblique Splitting Osteotomy of Mandibular Rami.

This is a report of 2-cases of mandibular prognathism corrected by Intraoral oblique splitting osteotomy of mandibular rami. The Intraoral oblique splitting osteotomy is a modification of sagittal split osteotomy of ramus and it is documented by Yoshida, on 1985. By this method, authors obtained the following results. 1. The patients' esthetic, psychological and functional problems were dissolved by setback of mandibular prognathism. 2. The postoperative infection splitted bone segments fracture, paresthesia of the face and T.M.J. dysfunction were not appeared. 3. Postoperative intermaxillary fixation was maintained for 8 weeks. The patients could open their mouths in normal range after a week of intermaxillary fixation removal. 4. The soft tissue changes of lower lip and chin were about 1:1 to the hard tissue changes. 5. During intermaxillary fixation period and postoperative orthodontic treatment, slight relapse was observed. Now, the patients are under postoperative orthodontic treatment.

3.The Expression of Adhesion Molecules in Experimental Allergic and Chemical Conjunctivitis.

The purpose of this study was to assess the expression of ICAM-1, VCAM-1 and PECAM-1 in allergic and chemical conjunctivitis. The allergic and chemical conjunctivitis were induced in C57BL/6 mouse by compound 48/80(C48/80) and 2% characterized clinically by blepharospasm 100%, chemosis 80%, injection AgNO3, respectively. The allergic conjunctivitis was characterized clinically by blepharospasm 100%, chemosis 80%, injection, 40%, mucous discharge 20%, but the chemical conjunctivitis by blepharospasm 80%, chemosis 60%, infection 40% and no mucous discharge at 30 minute after topical application. On the endothelial cells, ICAM-1 was expressed from 1 to 48 hours, VCAM-1 from 6 to 72 hours and PECAM-1 from 1 to 72 hours in allergic conjunctivitis. In chemical conjuctivitis, the expression of ICAM-1 was observed from 6 to 72 hours. The expression of VCAM-1 was observed from 24 and 72 hours. The expression of PECAM-1 was demonstrated from 6 to 72 hours. The expression of cell adhesion molecules, particulary VCAM-1 and PECAM-1, was slighter in chemical conjunctivitis compared to allergic conjunctivitis. In conclusion, experimental allergic and chemical conjunctivitis demonstrate that cell adhesion molecules play a role in part in ocular inflammation and that there are differences between the adhesion molecule expression of two types of confunctivitis.

4.One Case of Menetrier's Disease.

Menetrier's disease is a rare disease characterized by the presence of large rugal folds involving part or all of the stomach. Patients with hypertrophic gastropathy often have distressing abdominal symptoms, weight loss and edema due to gastric protein loss. The 48-year-old male patient was admitted to the Hangang Sacred Heart Hospital with cheif complaints of indigestion and epigastric pain. The diagnosis of Menetriers disease is established by radiologic, endoscopic, and pathologic examination. He was treated with soft diet, antacid, H2- receptor antagonist, and IV albumin. We report a case of Menetriers disease with brief review of literatures.

5.A Clinical Study of Disruption of the Deltoid Ligament Associated with Fractures of Distal Fibula.

It is been known that the deltoid ligament of the ankle joint plays an important role in the stabiliy of the ankle joint. In cases of deltoid ligament rupture, associated with lateral malleolar fractures, cannot be maintained the integrity of the mortise and the stability of the talus. Controversy remains about the treatment of deltoid ligament injuries. Many authors advocate an operative repair for deltoid ligament ruptures for optimal reduction of lateral malleolar fracture. However, according to recent cadaveric studies and many satisfactory results of clinical studies, excellent results have been reported regarding the ankle joint stability by anatomical reduction of the lateral structure, but only without surgical repair of the medial structure. Fourteen patients with lateral malleolar fractures with associated deltoid ligament injuries treated at Sun General Hospital between January 1990 and June 1995. There were examined clinically and radiologically. We concluded that deltoid ligament repairs should be considered unnecessary as long as fibular fracture are stabilized anatomically with normal medial joint space. However, in cases with higher fracture levels of lateral malleolus, associated with syndesmotic injury, we recommend syndesmotic screw fixation or deltoid ligament repair.

8.The Usefulness of Three-Dimensional Imaging with Spiral CT in the Evaluation of Upper Airway Stenosis.

PURPOSE: To assess the usefulness of three-dimensional (3D) spiral CT imaging in patients with upper airway stenosis. MATERIALS AND METHODS: We performed 3D spiral CT imagings in ten patients in whom upper airway stenosis was clinically suspected. Eight of these patients had upper airway stenosis caused by intubation or tracheostomy(n=6), tuberculosis (n=1), or extrinsic compression by a thyroid mass (n=1). Spiral CT scanning(30-second continuous exposure and 90-mm length) was performed with a table speed of 3mm/sec and a section thickness of 3mm.The selected starting point was the epiglottis. The resulting data were reformatted by multiplanar reformation(MPR) and shaded surface display (SSD) with peeling after reconstruction of 2mm interval. In the evaluation oflocation and extent of stenosis, we compared fidings of 3D imaging with those of baseline axial images (n=10), endoscopy (n=9) and operation (n=4). RESULTS: The locations of stenosis in eight patients were as follows: tracheostoma (n=4), subglottic region (n=3), and larynx (n=1). In all eight patients, 3D imaging demonstrated the location and extent of stenosis, which exactly correlated with endoscopic and operative findings. In one patient, however, another stenotic area in the tracheal bifurcation was not discovered because this lesion was not includedin the field of CT scan. In two patients, the diagnosis on 3D images of 'no stenosis' was comfirmed by clinical findings or operation. No differences in diagnostic accuracy were noted between axial images, MPR, and SSD when evalvating the location and extent of stenosis ; vertical extent was shown more easily by 3D imaging than by axialimages, however. CONCLUSION: 3D imaging with spiral CT may be an useful adjunctive method in the evaluation of upper airway stenosis with variable causes.

9.RPE Cell Apoptosis by the Combination Treatment of SAHA and Lactacystin.

PURPOSE: To establish a new therapeutic strategy for proliferative vitreoretinopathhy (PVR), we examined the effect of combined treatment with HDAC inhibitor SAHA and proteasome inhibitor lactacystin in human retinal pigment epithelial (RPE) cells, ARPE-19. METHODS: Viability was determined by trypan blue exclusion assay. Mitochondrial membrane potential (MMP) was measured by flow cytometry. Proteasome activity was measured by fluorophotometry. The expression and degradation of apoptosis-related proteins were assesssed by Western blotting. Subcellular location of apoptosis-related factors was monitored by confocal miscroscopy. RESULTS: A single treatment with 5 micro M SAHA or 10 micro M lactacystin did not reduce cell viability. However, combination treatment with 5 micro M SAHA and 10 micro M lactacystin substantially reduced the viability, because the mixture induced the reduction of MMP and nuclear condensation or fragmentation. Moreover, the combination treatment triggered the activation of caspase-3 and the production of PARP cleavage products. These data indicate that the combination treatment efficiently induces apoptosis in ARPE-19 cells. However, co-treatment of SAHA did not augment the proteasome inhibitory activity of lactacystin, nor did co-treatment of lactacystin augment acetylation of histones. It is notable that while p53 and CAD were observed in the mitochondria of cells treated with SAHA, they were translocated into the nucleus after the combination treatment. CONCLUSIONS: These results suggest that the combination treatment of SAHA and lactacystin effectively induced apoptosis in ARPE-19 cells. Further work is warranted to develop this combination therapy as a novel therapeutic strategy for PVR.

10.A Case of Hypocalcemia due to Vitamin D Deficiency in Exclusively Breast-fed Infant.

Hypocalcemia is due to Hypoparathyroidism, Vitamin D deficiency, Hypomagnesemia, Inadequate calcium intake. The benefits of breast-feeding are well established. There are no need to supply calcium or Vitamin D in breast-fed infant. We report a case of infantile hypocalcemia caused by Vitamin D deficiency in exclusively breast-fed infant. He had no hypocalcemic symptom and hypocalcemia was found incidentally by routine laboratory tests during pneumonia treatment. He was presented with a low serum calcium level and 1,25(OH)2 Vit D3 level and high PTH. He was improved by Calcium and Vitamin D supplement. After then his mother continued breast feeding exclusively and resisted to feed her baby weaning food. During follow up period, hypocalcemia was recheked after discontinuation of vitamin D supplement. At 11 months of age, the calcium level was normal without vitamin D supplement after he had eaten weaning food. This report describes a case of hypocalcemia induced by vitamin D deficiency in exclusively breast-fed infant, with review of the literature.
