1.The Incidence of Urinary Incontinence and influences on quality of life among elderly Women.

The purpose of this study was to identify the incidence of urinary incontinence of elderly women in Korea, to identify the life style for urinary incontinence. The sample consisted of 237 women of Seoul and 5 Provinces. Data for this study was collected from January 16 to June 23 by structured questionnaire. Urinary Symptom Questionnaire developed by Jackson and demographic questionnaire were used to collect the data. The data was analyzed by frequency, percentage, Chi-Square test, Chronbach alpha coefficient with SPSS/PC+ program. 1. It was reported that 64.5% of the subjects have experienced urinary incontinence. And stress, urge, and mixed type incontinence was 34.0%, 8.5%, 57.5% respectively. 2. Characteristics related to urinary incontinence are : Daily frequency 28.7%, nocturia 71.5%, urgency 74.0%, bladder pain 36.7%, unexplained incontinence 35.1%, noctural incontinence 23.5%, frequency of incontinence 23.3%. Wearing protection 42.3%, changing outer clothing 31.0%, hesitancy 17.1%, straining 24.7%, intermittent stream 27.2%, abnormal strength of stream 28.4%, retention 13.6%, burning 5.0%, incomplete emptying 13.6%, Inability to stop mid stream 32.5%. 3. The life style and incidence with urinary incontinence were as following. Fluid intake restriction 18.6%, interference in daily task 30.3%, avoidance of places and situation 28.0%, interference in physical activity 22.3%, interference in relationship with other people 16.6%, interference in relationship with husband/companion 4.5%, time after attack of urinary symptoms 81.5%. In terms of the feeling about the rest of life with urinary symptom, perfectly happy 5.0%, pleased 10.6%, mostly satisfied 18.7%, mixed feelings 28.6%, mostly dissatisfied 20.1%, very unhappy 16.8%, desperate 0.3%. 4. Significant relation between incidence of urinary incontinence and life style. Significant differences between Fluid intake restriction(X2=8.876, P=0.002), affected daily task(X2=32.113, P0.000), avoidance of places and situation(X2=30.155, P=0.000), interference in physical activity(X2=30.209, P=0.000), interference in relationship with other people(X2=26.091, P=0.000). In terms of the feeling about the rest of life with urinary symptom(X2=43.425, P=0.000) of life style and incidence of urinary incontinence. In conclusion, this study were preliminary study to provide nursing practices guidelines for elderly urinary incontinence. Nurses working with elderly should develop and provide adequate care for the incontinent elderly subjects.

5.A Case of Sparganosis Developedin Penile Shaft.

We experienced a case of sparganosis developed in the dorsum of penile shaft where, we think, is the very rare site of sparganosis reported in Korea. The patient, a 23 year-old Korean soldier, had slight tender thread-like linear subcutaneous mass on the dorsal aspect of penile shaft for 1 month prior to coming to us. The skin lesion was not palpated when the penis was erect. He had experience of intaking raw-snakes intermittently(5 times) since 2 years ago.

6.A Case of Tick Bite.

We describe herein a case of tick bite ocurring to a 23-year-old man who has a characteristic lesion clinically and histopathologically. The tick(Ixodes sp.) was still attached to the brownish and erythematous, indurated skin of the scrotum. The engorged tick which was carefully removed from the skin with a scalpel was 8.7mm in length and 5. 5 mm in width. Histopathologically, characteristic amorphous eosinophilic materigl is deposited around a round space where the hypostorne of the tick was inserted. Heavy inflammatory cell infiltratton in the mid and lower dermis is composed mainly of neutrophils and eosinophils. A certain area of the lower dermis shows edema and capillary dilatation along with the mixed inflammatory cell infiltration, TVide excision of the indurated lesion including the tick was done. The clinical and hsitopathological findings of the tick bite was discussed.

8.Solitary Morphea Profunda with Incidental Acantholysis.

Solitary morphea profunda is a rare form of scleroderma, characterized clinically by a solitary sclerotic plaque, and histologically by marked dermal and subcutaneous fibrosis with an inflammatory infiltrate. We describe another case of this entity presented with an ulcerative, indurated plaque on the left iliac crest, which histologically revealed a focal incidental acantholysis in the overlying epidermis and a marked eosinophilic infiltration through the dermis to the sub-cutaneous tissue.

9.Crossroad between inflammation and carcinogenesis in colon.

Current evidences have expended the concept that chronic inflammation might play a crucial role in the development and progression of colorectal cancer. For instance, chronic ulcerative colitis (UC) is associated with a 10- to 40-fold increased risk of developing colorectal cancer (CRC) compared to the general population. However, the specific mechanistic link between chronic inflammation and carcinogenesis in colon has not been integrated into molecular understanding. In this current review, we will provide an update on the molecular pathogenesis of colitis-associated colorectal cancer, focused on 1) the differences of molecular mechanism between the colitis-associated colorectal cancer (CAC) and the sporadic colorectal cancer (SCC), 2) the plausible and contributive role of chronic inflammation in colon carcinogenesis, and 3) lessons learned from colitis-associated animal model. Understanding of molecular pathogenic mechanism underlying the colitis-associated colorectal cancer will facilitate the development of novel treatment strategies for prevention of colitis-associated colorectal cancer.

10.Comparison of two in vitro assays for serum house dust mite - specific IgE with skin testiong in allergic subjects.

BACKGROUND: The skin prick test and in vitro allergen-specific IgE assays are commonly used to diagnose atopic diseases. However, there is still a need for comparison of their diagnostic efficiency. Objective and METHOD: To evaluate their clinical efficiency, the results of UniCAP and multiple antigen simultaneous test (MAST) were compared with skin prick test results. After 51 allergic patients completed skin prick test (SPT), serum sample was collected and UniCAP and MAST were performed to determine specific IgE to house dust mite (Dermatophagoides pteronyssinus : Dp and D. farinae. Df). Result : When SPT was used as a reference standard, UniCAP depicted higher sensitivity of 88.8% to Dp IgE and 91.4% to Df - IgE, but lower specificity of 73.3% to Dp IgE and 75.0% to Df - IgE. However, MAST had lower sensitivity of 75.1% to Dp-IgE and 71.4% to Df - IgE, higher specificity of 93.3% to Dp-IgE and 93.7% to Df - IgE. The values of UniCAP and MAST were significantly correlated with the reactivity grade of skin prick test, respectively. Additionally, the response of SPT was not apparently associated with ECP levels. CONCLUSION: These study results may suggest that both UniCAP and MAST are generally feasible for measuring house dust mite - specific IgE and that they are both replicable.
