1.Ethnography on Isolation Unit for Hematopoietic Stem Cell Transplantation: Focusing on Patients.

PURPOSE: The purpose of the study was to understand how patients experience everyday life in an isolation unit for hematopoietic stem cell transplantation (HSCT). METHOD: The data were collected from 25 patients with HSCT at the isolation unit from January to March in 2008 in one general hospital in Korea. The data were collected by participant observations and ethnographic interviews and were analyzed using ethnographic method. RESULTS: Four themes regarding environmental area emerged: 'barrier pulling up the drawbridge', 'very strange world', 'small and restricted space tied by IV and other treatment lines', and 'loud noise in a silent space.' Three themes regarding patients emerged: 'facing fear and anxiety', 'continuation of loneliness and lethargy', and 'compromising with a very long, dull, and boring time'. These themes describe how patients with HSCT suffer from continuous physical and psychosocial problems in a confined space, while endeavoring to control these problems and to search for hope for a new life. CONCLUSION: The results of the study provide an in-depth understanding of the experience and culture of patients in an isolation unit for HSCT. They would be used in developing practical programs to decrease patient's culture shock including fear and anxiety at isolation unit for HSCT.

2.Knowledge of Stroke Symptoms and Risk Factors Among Older Adults.

PURPOSE: This descriptive study was conducted to identify the level of knowledge of stroke symptoms and risk factors among older adults. METHODS: A total of 200 older adults over 65 years of age were conveniently recruited from out patient departments of two hospitals and a health care center from October to November 2008. The level of knowledge was assessed using both open-ended questions and a structured questionnaire based on semi-structured interviews. Data were analyzed by t-tests and ANOVA using the SPSS program. RESULTS: 52.5% of the sample had hypertension and 30% had diabetes. The mean knowledge scores for symptoms and risk factors were 8.4 +/- 3.1(out of 15) and 9.5 +/- 3.9 (out of 16), respectively. The older adults who had lower education, lower family income, and who lived in rural areas were more likely to have less knowledge of stroke symptom and risk factors(p < .05). There was no significant knowledge difference between the older adults who had at least one risk factor and those who had no risk factor for stroke. CONCLUSIONS: Educational intervention should be focused on informing older adults who are at risk for stroke about the early symptoms and management of risk factors, especially those who have low education and low social status.

3.A Study of Ni-resistant bacteria isolated from gingival crevicular fluid on the patients wearing Ni-Cr alloy prosthesis (In terms of molecular biological aspects).

As a material of metal-ceramic prosthesis, nickel as a form of Ni-Cr alloy has been used for many dental prostheses in many cases. However, several problems in use of the alloy have been revealed (ex ; tissue stimulation, skin allergy, hypersensitivity, cytotoxicity and carcinogenecity). Little is known about nickel with respect to the relationship between Ni-prosthesis and gaining of Ni-resistance in oral microorganisms. The present study was undertaken to check wheather use of Ni-prosthesis leads to occurrence of Ni-resistant microorganisms. So this study may suggest the possible relationships between the oral microorganisms and nickel-resistance in oral enviroment. Bacteria were isolated from the gingival crevicular fluid on the patients wearing Ni-Cr prosthesis. The isolated bacteria were tested for their Ni-resistance in nickel containing media at different concentration from 3mM to 110mM. E. coli HB101 was used as control. The Ni-resistant bacteria were isolated and biochemically identified. The Ni-resistant bacteria were tested several biochemical, molecular-biological tests. Performed tests were ; measuring the growth curve, antibiotic test, growth ability test in liquid media, isolation of the chromosome and plasmid, digestion of DNA by restriction enzyme, electrophoresis of chromosome and plasmid DNA, identification of Ni-resistant genes by the DNA hybridization. The results were as follows: 1) The bacteria isolated from gingival crevicular fluid on the patients wearing Ni-Cr alloy prosthesis showed nickel-resistance. 2) The isolated microorganisms grew at nickel containing media of high concentrations (60mM-110mM). 3) Based on the biochemical tests, the isolated microorganisms were identified as Enterococcus faecalis(13 cases), Klebsiella pneumoniae(1 case) and Enterobacter gergoviae(1 case). 4) Enterococcus faecalis expressed not only nickel resistance but also the multi-drug resistanceto several antibiotics ; chloramphenicol, kanamicin, streptomycin, lincomycin, clindamycin. However all strain showed the sensitivity against the tetracycline. 5) DNA hybridization result suggest that there is no homology between the previousely known gene of nickel resistance in Klebsiella pneumoniae and chromosomal DNA Enterococcus faecalis.

4.The Effects of Zinc Ion on the Adriamycin-Induced Apoptosis of HL-60 Cells.

PURPOSE: The widely used chemotherapeutic agents exert their anti-cancer effects by the inducing of apoptosis in sensitive tumor cells. Recently, the protective effect of zinc ion on apoptosis has been reproted. However, it is not well understood about the effects of zinc ion on the anticancer drug-induced apoptosis. In general, zinc inhibits a nuclear endonuclease, thereby causing inhibition of apoptosis. In addition, there is other possibility that zinc can prevent apoptosis at earlier stage such as the activation of caspase-3 than that of the activation of endonuclease. Therefore, we investigated the effects of zinc ion on the apoptosis of HL-60 cells caused by adriamycin (ADR). METHODS: HL-60 cells were cultured in RPMI 1640 and treated with various concentrations and time periods of ADR with or without pretreatment of zinc ion. Cellular DNA was extracted and analyzed by electrophoresis on a 1.5% agarose gel to detect DNA fragmentation. The activity of caspase-3 was measured by the proteolytic cleavage of the fluorogenic substrate DEVD-AMC. Poly-ADP-ribose polymerase (PARP) cleavage was analyzed by western blotting using anti-PARP antibody. RESULTS: ADR induced the apoptotic death of HL-60 cells in a dose and time dependent manner, which was characterized by increasing ladder-pattern DNA fragmentation. Pretreatment of HL-60 cells with zinc ion caused potent inhibition of ADR-induced apoptosis. Consistent with apoptotic death of HL-60 cells, ADR induced the catalytic activation of caspase-3. After pretreatment of zinc ion, the activation of caspase-3 and the proteolysis of PARP induced by ADR were markedly inhibited. CONCLUSION: These results indicate that zinc ion prevents the ADR-induced apoptosis of HL-60 cells through an inhibition of caspase-3 activity, which occurs upstream from the activation of endonuclease.

STATEMENT OF PROBLEM: Periotest. and OsstellTM were known as the most objective and quantitative mobility tests available for evaluating stability of implant in vivo. Although a correlation between PTV widely used and ISQ recently introduced exist, a PTV was corresponded to various ISQ. A correct evaluation of implant stability could be obtained only after one has a thorough understanding of the limitations of devices and factors that affect measurements. PURPOSE: The purpose of this study was to investigate the causes of variables in the values obtained with these two tests. MATERIAL AND METHOD: A total of 333 implants: 134 Bra.nemark, 5 Silhouette and 194 ITI implants were investigated. Result: 1. There was a correlation between PTV and ISQ (Spearman correlation =0.39, p<0.0001) 2. The factors that affected ISQ were diameter of implant fixture, location of implant and implant system (submerged type vs non-submerged type). 3. The factors that affected PTV were diameter of implant fixture, location of implant, and elapsed time after implant placement. 4. There was no significant difference between different surface treatments of RBM, smooth surface and ti-unite on PTV and ISQ. 5. In radiographic finding, no saucerization or bone resorption has been detected in implants with ISQ values that were above the average level of each PTV. These higher values had higher bone densities around the implant fixture. Saucerization was observed in the most impants with ISQ values that were below the average level of each PTV. CONCLUSION: There was a correlation between ISQ and PTV. However, each measuring methods had factors influencing the measured values. PTV were less sensitive to marginal bone resorption and influenced with the striking point on an implant to the level of bone. With ISQ, the height of implant from bone level to transducer should be considered.

7.Group Differences in Subjective Memory Complaint Content: Late Life Depression and Mild Cognitive Impairment.

OBJECTIVE: To investigate the group differences in contents of subjective memory problem and objective cognitive functions in late life depression and mild cognitive impairment. METHODS: 40 participants were classified into three groups [normal, amnestic mild cognitive impairment (MCI), and depression]. Differences in subjective memory complaints were examined using the Prospective and Retrospective Memory Questionnaire. And the Consortium to Establish a Registry for Alzheimer's Disease-Korean version neuropsychological assessment battery results were used for evaluating objective cognitive functions. Group differences were analyzed ANCOVA analysis. RESULTS: In post-hoc analysis, the questions "Do you fail to recognize a character in a radio or television show from scene to scene?" and "Do you forget something that you told a few minute before?" were significantly able to discriminate between the MCI and normal groups. In post-hoc analysis, late life depression group showed significantly higher score on the question "Do you intend to take something with you, before leaving a room or going out, but minutes later leave it behind, even though it's there in front of you?". CONCLUSION: The result demonstrates the presence of differences in the contents of subjective memory complaints according to groups. Subjectively reported memory complaints in MCI group are related to deficits in learning new information. In late life depression, memory complaints are more related to difficulty in executive functions.

8.Usefulness of Objective Hearing Tests for Screening Patients with Partial Deafness.

BACKGROUND AND OBJECTIVES: 'Partial deafness' characterized by normal or slightly impaired hearing in the low frequency band and nearly total deafness in the high frequency range, is difficult to assess with conventional behavioral tests in infants and young children. Therefore, this study aimed to assess the usefulness of objective hearing tests, such as auditory brainstem response (ABR) and auditory steady state response (ASSR) in evaluating patients with partial deafness. SUBJECTS AND METHOD: One hundred thirty three patients who underwent ASSR in Ajou University Hospital from January 2008 to January 2013 were enrolled to this study. Correlations between ASSR, ABR and pure tone audiometry (PTA) thresholds were analyzed. RESULTS: ASSR thresholds of 133 patients were highly correlated with both ABR and PTA thresholds in majority of the tested frequencies. Partial deafness was detected in 9 out of 133 patients, based on the results of PTA and ASSR. ASSR thresholds of patients with partial deafness were significantly correlated with PTA thresholds in all frequencies, with especially high correlation found at 1 and 4 kHz. However, there was no significant correlation between ASSR and ABR thresholds. ABR thresholds of partial deafness patients were measured at 65 dB in one patient, 70-90 dB in 3 patients, and no response at 90 dB in 5 patients, respectively. In 8 (6%) out of 124 patients, ABR thresholds were measurable with profound hearing loss and residual hearing was observed at low frequencies. CONCLUSION: ASSR is useful for predicting residual hearing at low frequencies of infants and young children for whom assessment of hearing is difficult using conventional behavioral tests.

9.Novel Mutation of the GNE Gene Presenting Atypical Mild Clinical Feature: A Korean Case Report.

Glucosamine (UDP-N-acetyl)-2-epimerase/N-acetylmannosamine kinase (GNE) myopathy is caused by mutations in GNE, a key enzyme in sialic acid biosynthesis. Here, we reported a case of GNE that presented with atypical mild clinical feature and slow progression. A 48-year-old female had a complaint of left foot drop since the age of 46 years. Electromyography (EMG) and muscle biopsy from left tibialis anterior muscle were compatible with myopathy. Genetic analysis led to the identification of c.1714G>C/c.527A>T compound heterozygous mutation, which is the second most frequent mutation in Japan as far as we know. Previous research has revealed that c.1714G>C/c.527A>T compound heterozygous mutation is a mild mutation as the onset of the disease is much later than the usual age of onset of GNE myopathy and the clinical course is slowly progressive. This was the first case report in Korea of the clinicopathological characteristics of GNE myopathy with GNE (c.1714G>C/c.527A>T compound heterozygous) mutation.
