There are strong evidences suggesting that Thl and Th2 lymphocytes develop from the same Thlymphocyte precursor under the influence of environmental or genetic factors acting at the level of antigen presentation, but it remains to be answered whether it is possible to change the cytokine profile of established or ongoing Th1 and Th2 response. The purpose of this study is to reveal whether it is possible to reverse the cytokine profile of human Th lymphocytes by the modulation of antigen presentation. Using a multiparameter flow cytometric assay that allows simultaneous determination of surface CD4 and intracellular IFN-r or IL-4, we have studied the emergence of Th1 or Th2 lymphocytes in response to tetanus toxoid exposure and the patterns of cytokine synthesis in established T lymphocyte clones. Th2 populations arising after 4 wk of stimulation in IL-2, PHA, tetanus toxoid and irradiated autogeneic peripheral blood mononuclear cells as antigen presenting cells (APC) could give rise to IFN-r-producing Th1 lymphocytes when stimulated in IL-2 plus PHA in the absence of antigen and APC. These IFN-r-producing Th1 lymphocytes nearly disappeared and IL-4-producing Th2 lymphocytes predominated again when cultured again in the presence of antigen and APC. In contrast, prolonged culture in the absence of antigen and APC induced relative predominance of IFN-r-producing The lymphocytes. The cytokine profile of long-term Th2 population arising originally from the repeated stimulation in the presence of antigen and APC appeared more homogeneous and less reversible, although they could convert to Th1 lymphocytes when cultured without antigen and APC. These findings may explain that the polarized Th response is reversible depending on the presence of antigen presentation.

The acetabular angles were measured in 2 groups of selected 100 infants and children each, of whom one group aged 14 months and younger and the other group aged 15 months of 5 yrs. and who were proved to be normal by all the physical examination and whose acetabular angles were below 40 according to Coleman's diagnostic criteria. The measurements were broken down into right and left, and male and female because of the known difference in the incidence of CDH in 2 members of each of these paired categories and analysed statistically. The following statistic significances resulted: l. All the measured normal acetabular angles vary between 10 and 31 degrees. 2. The acetabular angles decrease after weight bearing by the statistical significant difference between 0-14 mo. group and 15 mo-5yr. group. The angles are 22,61±3,47 (M±S,D) and 18.68±3.64 (M±S.D) respectively. 3. The acetabular angles of female of 15mo.-5yr. group are higher than male by the statistical significant difference. The angles are 17.77±3.54 (M±S.D) and 20.18±3.29 (M±S.D) respectively. 4. Statistical significant differences were absent between right and left.

BACKGROUND: The purpose of this study was to analyze the relationships between cardiorespiratory fitness and body mass index (BMI), metabolic syndrome risk factors, homeostasis model assessment-insulin resistance (HOMA-IR), and high sensitivity C-reactive protein (hsCRP) in male high school students in Korea. Furthermore, the effects of the cardiorespiratory fitness levels on these four factors were examined. METHODS: The level of cardiorespiratory fitness, BMI, percent body fat, waist circumference (WC), systolic blood pressure, glucose, triglycerides (TG), high-density lipoprotein-cholesterol (HDL-C), HOMA-IR, and hsCRP of 227 male high school students were measured. Analyses of multiple correlation and simple regression were applied. RESULTS: The results showed that the level of cardiorespiratory fitness correlated negatively with BMI, percent body fat, WC, TG, and HDL-C, in addition to HOMA-IR and hsCRP, and strongly influenced BMI, percent body fat, WC, TG, HDL-C, HOMA-IR, and hsCRP. Comparative analysis of our subjects showed that higher cardiorespiratory fitness levels had positive effects on BMI, metabolic syndrome risk factors, HOMA-IR, and hsCRP. CONCLUSIONS: This study found that there are close correlations between cardiorespiratory fitness and BMI, metabolic syndrome risk factors, HOMA-IR, and hsCRP. It also showed that, compared to lower levels of cardiorespiratory fitness, higher levels had beneficial effects on BMI, metabolic syndrome risk factors, diabetes, and cardiovascular disease risk factors.

A survey was conducted to study attitudes of physician, nurse and patient towards physician's and nurse's uniform, from March 1 to March 31, 1996. The study population was 130 physicians and 147 nurses engaged in Yeungnam University Medical Center and 211 inpatients of Yeungnam University Medical Center. A questionnaire method was used to collect data. The following are summaries of findings: In the respect of physician's uniform, both physicians and nurses preferred other type of gown to the traditional coat-typed one and especially, nurses preferred more than physicians. Patients showed no difference in the preference of the traditional one and other form of gown as a whole but those who had higher educational level preferred other form of gown(p<0.01). It tended to agree wearing casual wear of physician during the working time of weekend and holiday as a whole. Younger physicians showed significantly higher preference for it(p<0.05). Regarding the nurse's uniform, both physicians and nurses preferred trousers, and 96% of the nurses did. Especially, nurses who were forty years old or more and who served at outpatient department and administrative and aid parts expressed 100 percent partiality to trousers. For the patients, those who had lower educational level preferred skirt and those who had higher educational level preferred trousers. As to the color of nurse's gown, 46.7% of the physicians and nurses liked white color. The physicians preferred white and nurses preferred other color(p<0.01). Of the patients, 79.1% liked white color. Regarding the wearing cap, 95.9% of the nurses replied it didn't have to wear the cap. The nurses who were fifty or more and who served at outpatient department and special parts gave whole answers of not having to wear the cap. On the other hand, 77.7% of the patients answered nurse had to wear the cap. From the above findings, it would be advisable to give a change to the forms and colors of the gowns to match with the trend and sense of the time instead of insisting on the traditional typical ones.

OBJECTIVE: The purpose of this study was to analyse clinical manifestations and laboratory findings in childhood patients with polyarticularonset juvenile rheumatoid arthritis (JRA). METHODS: Eleven cases of polyarticular JRA who were diagnosed and treated in the Department of Pediatrics, Seoul National University Children's Hospital from June 1988 to May 1995 were investigated for clinical manifestations and laboratory findings. RESULTS: 1) There were 6 males and 5 females and their ages of onset were 4 years to 15.1 years(mean 10.9 years). 2) Systemic manifestations were not observed, but low-grade fever was noted in 5 patients. 3) The involvement of joints was symmetric in 9 patients and asymmetric in 2 patients. 4) The most commonly affected joints were knees and ankles, followed by proximal interphalangeal joints of hand, shoulder, elbow, temporomandibular joint, and other joints. 5) Roentgenographic changes of joints were detected in 6 patients and bone scan in 7 patients showed increased uptake in the involved joints. 6) The main laboratory findings observed were microcytic and hypochromic anemia (64%), thrombocytosis (82%), elevated eryhtrocyte sedimentation rate (100%), positive or increased C-reactive protein(100%), positive rheumatoid factor(RF) (18%), positive antinuclear antibody(ANA) (27%). RF was positive in 2 girls with later age of onset and the pattern of immunofluorescent ANA were all homogeneous. 7) Nonsteroid antiinflammatory drugs (NSAIDs) were used most frequently and steroid with or without sulfasalazine was tried in 4 patients unresponsive to NSAIDs. 8) At last follow-up, 6 cases(55%) were classified as functional class I, 4 cases(36%) as class II, and 1 case(9%) as class III. CONCLUSION: These data showed the clinical manifestations and laboratory findings of polyarticularonset juvenile rheumatoid arthritis in Korean children.
