1.Effects of Strontium on Norepinephrine Induced Positive Inotropic Effect of Isolated Perfused Rat Hearts.

This study was designed to investigate the effect of substitution of strontium for calcium on mechanical activity in isolated perfused spontaneously beating rat hearts. The mechanical activity of the hearts of Langendorff's preparation in conditions of low calcium and strontium-substitution for calcium was compared. The effect of norepinephrine and verapamil were also observed in those conditions. The results were as follows: 1. In low calcium, the mechanical activity of the heart preparation was significantly reduced, but when the equimolar strontium was substituted for the reduced calcium, the activity was kept at similar level to the normal condition. 2. When equimolar strontium was substituted for the total calcium in perfusate, the heart preparation stopped its beating, and it was not restored in spite of reperfusion with normal calcium perfusate. 3. Norepinephrine-induced positive inotropic effect was inhibited in low-calcium condition especially with low concentration of norepinephrine, but not in strontium-substitution for calcium. 4. Verapamil reduced the activity of the heart both in low-calcium and strontium-substitution as well as in normal calcium conditions. From above results, it was concluded that strontium served as a substitute of calcium in maintaining mechanical activity and in responsiveness to norepinephrine, and the influx of strontium through cell membrane is inhibited by verapamil as the influx of calcium.

2.A Clinical Observation of 148 Complications in the Open Fracture

One hundred forty eight complications in 135 cases of long bone fractures experienced during the period from May 1971 to December 1979, at the Chosun University Hospital and analysed its retrospectively. The results were obtained as follows. 1. The order of incidence of complications were as follows; infection (39.8%), delayed union(23.6%). malunion (13.5%), ankylosis of joint (12.2%). nonunion (8.1%) and peripheral nerve injury (2. 8%). 2. Of all 127 patients, 97 patients were male and 30 patients were female, and 72 patients (56.7%) were cauesd by traffic accidents. The sex ratio between male and female was 3.2: l. 3. The major complications were frequently associated with other fractures and dislocations, brain injuries. 4. Those of open, comminuted fracture and delayed transfer cases to the Orthopedic Surgeon were predisposing factors to development of the severe complication.

Intestinal malrotation with volvulus is generally presented as a bilious vomiting and acute intestinal obstruction in the newborn period. It could compromise vascular supply of the small bowel secondary to torsion of superior mesenteric artery (SMA) and without urgent surgical management, it could lead to detrimental outcomes such as transmural bowel infarction and sepsis. However, in chronic cases, it is rarely obstructs the vascular supply and propagates to an acute bowel infarction. Rarely, chronic malrotation with midgut volvulus may not reduce the mesenteric blood supply because of collateral vessels, and the chronically stagnated blood flow of the superior mesenteric vein (SMV) favors thrombus formation within the lumen. The recommended treatment is Ladd's procedure and anticoagulation therapy. The authors present an unusual case of intestinal malrotation with chronic volvulus resulting in superior mesenteric vein and portal vein thrombosis in a 28-year-old patient.

6.Cutaneous Leiomyoma of the Nipple in a Male.

Cutaneous leiomyoma is a benign tnmor originating from smooth muscle fibers which are derived from arrector pili muscle, media of blood vessels and dartoic muscle of scrotum, vulvar, or nipple. It is classified into 3 types according to the site of origin, namely pilar leiomyoma, angioleiomyoma and genital leiomyoma. Leiomyoma of the nipple is the less kquent type of cutaneous leiomyoma. We report a case of cutaneous leiomyoma on the nipple in a 53-year-old man.

10.Therapeutic Uses of T-lens(Soflens(R)) in Corneal Diseases.

We have investigated the therapeutic effectiveness of T-lens(Soflens(R)) in 105 patients(110 eyes) with various corneal diseases. The results are as follows: 1. Cornea diseases, which revealed the good therapeutic effectiveness, are ccrneal perforation, postoperative uses, corneal ulcer, corneal erosion, and corneal edema. 2. Corneal disease, which revealed the rapid therapeutic effectiveness, is corneal edema. 3. Corneal diseases, which revealed no response, are neuroparalytic keratitis and radiation keratitis.
