We report a case of atrophie blanche which was accompanied by cryoglobulinemia. The patient, 17-year-old female, have had recurrent painful ulcerations and ivory-white atrophic scars with telangiectases surrounded by hyperpigmentation. On histological examination, vessel wall thickening and hyalinization of the intima, partial vascular occlusion, vascular proliferation and mild perivascular chronic inflammatory cells infiltration and hemorrhage in the upper dermis were noted. The patient had been treated with dipyridarnol(Persantin) and acetylsalicylic acid hut the lesions had been waxed and waned.

Both irritation and staining reaction of anthralin on the skin are the two most important problem of therapy. Irritation, such as erythema, edema and staining by p. lg anthralin ointment were studied with the chamber-testing technique in 21 psoriatic patients. We campared the skin reaction of short exposure time, such as 1 hour, 2 hours, and 3 hours with those of exposure of 24 hours through 3 days after application. The results were as follows: l. Incidence of erytherna reaction was 81.9% in exposure of 1 hour, 85.7%, in 2 hours, 90.5%, in 3 hours and 100% in exposure of 24 hours. Degree of erythema reaction was increased according to duration of application. 2. There were no edematous reactions in exposure of 1 hour or 2 hours, 9,5% in 3 hours and 28,6% of grade 1 reaction in 24 hours. 3. There was no staining reaction in exposure of 1 hour, 48% in 2 hours, 9.5%, in 3 hours and 71.4% in 24 hours. Degree of staining reaction elicited by short exposure time were all weaker than reaction caused by 24 hours exposure.

Anthralin is highly efficacious in the treatment of psoriasia, but it has two major drawbacks: staining of the skin and clothing and irritation of the skin. There have been several studies that the effect of the anthralin with short contact therapy(SCT) is nearly the same as that with the simple application(SA). This study was planned to compare the effect and side effects of SCT with those of SA of anthralin in 27 psoriatic patients visited between November 1982 and June 1983 at the Department of Dermatology, Seoul National University Hospital. The results could be summerized as follows: 1. For 4 weeks of treatment, the longer the duration of treatment, the more similar the effect between the SCT and the SA. 2. After 2 weeks of treatment, the effect of the SA was better than that of the SCT in 5 cases, the same in 18 cases and worse in 1 case. 3. For 4 weeks of treatment, all side effects(irritation, ataining and pruritus) were more frequent in the SA than in the SCT. And irritation was observed earlier than staining. 4. After 2 weeks of treatment, the side effects of the SA were more than those of the SCT in 12 cases and the same in 12 cases. With all our results, we have concluded that the patients who cannot tolerate the SA could be tried with the SCT.

We speculate that this is case of an intermediate state between granuloma annulare and actinic granuloma. This intermediate state would be a variant of granuloma annulare related to actinic damage. Therefore this case can be termed actinic granuloma annulare with generalized distribution.

Arteriovenous hemangioma is a type of hemangiomas which are derived from or are composed of vessels. Recently, authors experienced a case of arteriovenous hemangioma, which clinically suspected a granuloma pyogenicurn but could be confirmed histopathologically.

The present study was undertaken to evaluate the efficacy of white petrolatum-UVB therapy on patients with psoriasis. Nineteen patients with chronic plaque type psoriasis selected in the Department of Dermatology, Seoul National University Hospital were treated with white petrolatum followed by exposure to UVB. Clinical response to therapy was graded as follows: Grade 1: Minimal improvement, Grade 2: Definite improvement Grade 3 Considerable improvement, Grade 4: Clearing. We campared the clinical response according to involved site: trunk and extremity. A responsive case was defined as a case with G4 in trunk and/or extremities. The results can be summarized as follows: 1. Fourteen of the 19 patients(73. 7%) were classified as being responsive cases. 2. Among the responsive cases, the 11 cases of trunk lesion all showed G4 clea- ring. The extremities lesion cases showed G4 clearing in 9 out of 14 cases, the other 5 cases showing GR. This showed that the trunk lesions were more responsive to the treatment than the extremities lesions. 3. In the cases of trunk lesions, the mean number of therapy was 6. 3 for G2 response, 11. 2 for GR and 16. 7 for GR. In the cases of extremities lesions, the mean number of the therapy was 8. 6 for G2, 16. 1 for GR and 19. 2 for Gl. 4. Pruritus was a complaint in 13 patients (68. 4%) during therapy. Among them moderate to severe pruritus was noticed in 6 cases (51. 6%). From the above results the emollient phototherapy seems to be an effective therapy in the treatment of psoriasis.

The purpose of this study is to evaluate the effectiveness of these para-amino- benzoate, benzophenone and cinnamate sunscreens in the same base(cream) and the effect of concentration on sun protection with three different concentrations(2 5%, 5. 0% and 7. 5%.). Twenty-one normal healthy Korean-men, majority(2. 5%) of whom have skin type iv or v, were applied to their untanned back with above prepared nine different sunscreens and calculated individual sun protection factor (SPF) of these different sunscreens. BACKGROUND: Many of the major life events that appear to affect biologic system and health occur within the context of the family and can have a profound psychosocial and biologic impact on family memebers. So, the present study attempted to examine the relations of family function with stressful life events, depression, and physical and psychological symptoms reported by patients. METHODS: Design : Cross sectional, observational study. Participants : 41 healthy adult males from a family medicine ambulatory care unit in a tertiary hospital located in Guro completed a series of psychometric instruments that include the Family APGAR(Adaptation, Partnership, Growth, Affection, and Resolve), FACESIII(Family Adaptability and Cohesion Evaluation Scale), Modified 46-item Stressful Life Events Scale, Lee's 98 items Stressful Life Event Scale, Korean version of BEPSI(Brief Encounter Psychosocial Instrument) and BDI(Beck's Depression Inventory Scale). RESULTS: Family APGAR was negatively correlated with stress related life events(both in total and negative events, -0.30, -0.37, respectively; p<0.05) and BEPSI(-0.56; p<0.0001). No significant correlation could be found between family function and positive life events. Family APGAR also revealed significant negative correlation with BDI(-0.61; p<0.0001). Among the three family types, severe dysfunctional family demonstrated highest level of stressful life events score, perceived stress, and depression(p<0.05). CONCLUSIONS: The higher the assessment of the family function(Family APGAR), the lower the level of stressful life events, perceived stress and depression. There, knowledge of the family function, stress, and coping could be useful to family physicians in the whole person approach of their patients.

Neonatal infective endocarditis is fatal and rare disease but recently there is a trend of an increase in incidence even without congenital heart disease when there are high risk as prematurity, central vein catheterization and neonatal cardiopulmonary resuscitation. Symchych reported that in about 3% of autopsy cases of an year, the findings of infective endocarditis were noted. In Korea, there was a case report but it was case of a premature infant who had umbilical vein catheterization and respiratory ventilator treatment. We experienced a case of neonatal infective endocarditis in a female infant who was delivered at 34 weeks gestational age without receiving central vein catheterization or neonatal cardiopulmonary resuscitation. Although she received intensive antibiotic therapy for staphylococcus cultured in blood, she died necessarily. We reported a case of neonatal infective endocarditis without cardiac anomaly with brief review of related literature.
