2.A clinical study on infants of diabetic mothers.

Thirty one infants of diabetic mothers(IDM) who had been admitted in Neonatal Intensive Care Unit at Chonnam University Hospital from January 1987 to July 1991 were studied for evaluation of their perinatal outcome and prognosis. The results were as follows; 1) The distribution of diabetic mothers according to modified White's classification was GD & class A in 12 cases(38.7%), B in 16 cases(51.6%), C in one case, D4 in one case, and R in one case. 2) Of the 31 diabetic mothers, 8(25.8%) had a history of infertility or stillbirth, 20(64.5%) had prognostically bad signs of pregnancy (PBSP), and 12(38.7%) were treated with insulin and/or oral hypoglycemic agent during pregnancy. 3) C-section rate was 32.2%, and the rates of shoulder dystocia and birth asphyxia were 35.5% and 29.0%, respectively. 4) The rates of large for gestational age were 75.0% in class GD & A, 68.8% in class B, and the rates of small for gestational age were 8.3% and, 6.3% respectively, Characteristic face with body composition was noted in all babies, hepatosplenomegaly in 90.3%, heart murmur in 41.9%, respiratory distress syndrome in 12.9%, transient tachypnea of mewborn in 19.4%, hypertrophic cardiomyopathy in 40.0%, and seizure in 6.5%. 5) Laboratory abnormalities were hypoglycemia in 77.4%, hyperbilirubinemia in 83.9%, polycythemia in 16.1%, hyperinsulinemia in 45.5%, hypercalcemia in 22.6%, and hypomagnesemia in 23.1%. 6) Birth injuries were noted in 61.3%, The most common were caput succedaneum and/or cephal-hematoma, brachial palsy, and fracture. Congenital anomalies were noted in 16.1%. The most common were musculoskeletal, cardiovascular, and gastrointestinal anomalies. 7) In the short term follow up of 19 patients, there were mild developmental delay in 2 cases, cerebral palsy in 2 cases, and epilepsy in 1 case. These results suggested that infants of diabetic mothers had greater morbidity than infants of nondiabetic mothers. Therefore, long term follow up is recommended to achieve their normal growth and development.

4.Pathologic Analysis of 39 Cases of Epilepsy Surgery.

Pharmacologic therapy is still the primary management for epilpsy; however, surgical treatment is a reasonable therapeutic option for patients suffering from medically intractable seizures, especially temporal lobe epilepsy having a documented unilateral epileptogenic area. Thirty nine patients with pharmaco-resistant complex partial seizures underwent anterior temporal lobectomy and hippocampectomy in 38 cases and frontal cortisectomy in one case. On pathological examination, hippocampal sclerosis was a predominent pathologic finding and was identified in 18 cases. Other non-neoplastic lesions consisted of 5 cases of vascular lesions(2 cavernous angiomas, 2 arteriovenous malformations and 1 angiomatosis), 3 cases of fibrous nodule, 2 cases of cicatrical changes of cerebral cortex, and 1 case of parasitic infection. Neoplasms including two cases of oligodendroglioma and one case of anaplastic astrocytoma were also noted. In seven cases, there was no detectable lesion on gross and microscopic examination. On post-operative follow-up, seizures were completely terminated in most cases(31 cases, 79%). The rest of the patients also displayed marked alleviation of symptoms. The seizures tended to recur more aften among the patients with neoplasm or no pathologically detectable lesion. In order to detect any minute pathological lesion, thorough gross and microcsopic examinations are considered to be essential.

6.Expression of Transforming Growth Factor-beta1 , beta2 by Immunohistochemical Staining method: In Human Endometrium through the Menstrual Cycle.

OBJECTIVES: The purpose of this study was to determine the differences of expression of TGF-betaS(TGF-beta1 and TGF-beta2) in the human proliferative endometrium, secretory endometrium, and the decidua during early pregnancy. And we also have studied the endometrial expression of TGF-beta1 and TGF-beta2 in the menopause and compared that to the expression in the endometrium and decidua. METHODS: We have studied the expression of TGF-beta1 and TGF-beta2 by immunohistochemical staining method in the proliferative endometrium, secretory endometrium, decidua during early pregnancy, and menopausal endometrium. RESULTS: In the epithelial cells, TGF-beta1 was moderately expressed in the secretory phase and was weakly expressed in the proliferative phase and menopause. In the stromal cells, TGF-beta1 was not expressed in the whole menstrual phase and menopause. And in the epithelial cells, TGF-beta2 was moderately expressed in the proliferative phase, secretory phase, and menopause. In the stromal cells, TGF-beta2 was not expressed in the whole menstrual phase and menopause. Especially, TGF-beta1 and TGF-beta2 were markedly expressed in the decidua during early pregnancy compared to the expression in the proliferative, secretory endometrium, and menopausal endometrium. CONCLUSIONS: These findings suggest that TGF-beta1 may have an important role in the epithelial cells during the secretory phase, not stromal cells. And TGF-beta1 and TGF-beta2 may have a paracrine and autocrine role in the decidua/trophoblast interaction during pregnancy , especially in the normal pregnancy.

7.A Clinical Study on BCG Lymphadenitis.

PURPOSE: We studied the incidence and clinical course of BCG lymphadenitis to investigate the BCG related problem. SUBJECT AND METHODS: Three hundred and eighty-six infants received BCG vaccine (Pasteur-1173p2) in the Well Baby Clinic, Dept. of Pediatrics, Chonnam University Hospital from May 1993 and April 1994. Among them, twenty three infants developed regional lymphadenitis. We investigated the clinical course in twenty six infants with BCG lymphadenitis (three infants were transferred from the Public Health Center and they also had taken same BCG preparations). According to the lymph node size, we divided them into three groups (A : below 1cm, 5 infants; B : 1 to 2cm, 10; C : above 2cm, 11), and evaluated the clinical outcome. RESULTS: 1) The incidence of BCG lymphadenitis was 6.0% (23/386), the differences according to sex or age at vaccination were not observed. 2) Twenty three of twenty six infants (88.5%) showed one lymph node (left axillary 19 infants, left cervical 3 infants, left supraclaviclar 1 infant), but three (11.5%) showed multiple lymph node involvement. 3) Eight (30.8%) showed spontaneous improvement, but eighteen (69.2%) received single or double antituberculous medication at the beginning or after a period of observation. Two infants were treated by incision and drainage. CONCLUSIONS: The incidence of BCG lymphadenitis was relatively higher (6.0%) than previous reports, and the two thirds of the cases received antituberculous medication and or surgical treatment. Further prospective study according to the recent recommendation by the Korean Health Administration should be needed.

8.A Case of Duplication 9q Syndrome.

Since the first description of the trisomy 9p in 1970, over one hundred cases have been described with the advanced chromosomal banding technique. Clinical findings include growth and mental retardation and facial dysmorphism. Crucial determinants of the classical features of this syndrome lie within the distal half of the chromosome 9 short arm. But this syndrome has not been reported in Korea, we are reporting a boy diagnosed by clinical features and chromosomal study that is trisomic for a partial short arm of a chromosome 9. A brief review of the literature is included.

9.Evaluation of insertion of torque and pull-out strength of mini-screws according to different thickness of artificial cortical bone.

OBJECTIVE: The purpose of this study was to evaluate the mechanical performance of mini-screws during insertion into artificial bone with use of the driving torque tester (Biomaterials Korea, Seoul, Korea), as well as testing of Pull-out Strength (POS). METHODS: Experimental bone blocks with different cortical bone thickness were used as specimens. Three modules of commercially available drill-free type mini-screws (Type A; pure cylindrical type, Biomaterials Korea, Seoul, Korea, Type B; partially cylindrical type, Jeil Medical, Seoul, Korea, Type C; combination type of cylindrical and tapered portions, Ortholution, Seoul, Korea), were used. RESULTS: Difference in the cortical bone thickness had little effect on the maximum insertion torque (MIT) in Type A mini-screws. But in Type B and C, MIT increased as the cortical bone thickness increased. MIT of Type C was highest in all situations, then Type B and Type A in order. Type C showed lower POS than Type A or B in all situations. There were statistically significant correlations between cortical bone thickness and MIT, and POS for each type of the mini-screws. CONCLUSION: Since different screw designs showed different insertion torques with increases in cortical bone thickness, the best suitable screw design should be selected according to the different cortical thicknesses at the implant sites
