2.The Effect of Mitomycin on the Experimental Filtering Surgery.

In tiis study, an expenmental model of glaucoma filtering surgery in rabblts was used to evaluate the effect of single intraoperative application of Mitomycin Full-thickness filtering surgery, posterior lip sclerectomy, was performed after application of 0.5mg/ml Mitomycin-soaked Week-Cel(TM) sponge for 5 minutes in one eye of 15 pigmented rabbits and was irrigated copiously with balanced salt solution(BSS(TM)). The same procedure was done in the contralateral eye as a control, using BSS(TM). In Mitomycin-treated group, the filtering blebs were more prominent with sparse vascular tissues and were maintained through postoperative 8 weeks or more. Histologically, there loas was amount of inflammatory reactions in Mitomycin-treated group. The fibroblastic activity and proliferations of the collagen were also suppessed markedly in Mitomycin-treated group. These results suggest that a single intraoperative application of Mitomycin can be used adjunctively to improve the success rate of glaucoma filtering surgery.

3.A Study on UVI - induced DNA Synthesis in Mouse Skin in Vivo Studied by Autoradiography.

The status of DNA synthesis and the effect of UV radiation on the DNA synthesis were studied in mouse skin by microautoradiography. Mice exposed to 100mJ/cm of UVB were injected intradermally with tritiated thymidine, 5 minutes, 2, 6 and 24 hours after irradiation and biopsies were processed for light microscopic autoradiography, A total of 25 ICR female albino haired mice were used as subjects. We compared heavily labeled cells(>10 grains/nucleus) and sparsely labeled cells (3-10 grains/nucleus) in UVL irradiated skin with nonirradiated control skin. 1. Within 5 minutes after UVL exposure an apparent depression in the number of heavily labeled cells occurred. This reduction was statistically significant and remained so for at least six hours post,irradiation. By 24 hours after UVL exposure, the reduction was recovered to nearly control level. 2. By five minutes after UVL exposure, sparsely labeled cells were observed in basal cell layer and differentiated cell layer. The nurriber of sparsely labeled cells appeared highest at five minutes after UVL exposure and then tended to fall to nearly control level at 24 hours. This aberrant type of TdR H incorporation is thought to represent unscheduled DNA synthesis to repair pyrimidine dimers formed in UVL injured DNA molecules,

6.The Pretreatment Effects of Morphine, Propofol, Atropine, and Midazolam on Fentanyl Cough Response.

BACKGROUND: The afferent and efferent pathways of fentanyl cough response (FCR) and central organization are poorly understood at present. The aim of this study was to investigate the pretreatment effects of morphine, propofol, atropine, and midazolam on FCR. METHOD: The 120 healthy patients were randomly assigned to six equal pretreatment groups. They received 2ug/kg fentanyl rapidly through a peripheral venous catheter. The patients in each group were pretreated before the time necessary for peak plasma levels with different drugs as follows: group 1, no premedication; group 2, morphine 0.05 mg/kg iv; group 3, morphine 0.05 mg/kg iv naloxone 0.01mg/kg iv; group 4, propofol 0.5 mg/kg iv; group 5, atropine 0.01 mg/kg iv; group 6, midazolam 0.05 mg/kg iv. The patients were observed for any coughing or side effects, including oxygen desaturation, bronchoconstriction, chest wall rigidity and seizure. RESULT: 40% of patients in group 1 (control) had a cough response to fentanyl. Group 2 (morphine) and group 3 (morphine naloxone) showed a reduced FCR of 10%. The incidence of coughing was 60% of the patients in group 4 (propofol), 30% in group 5 (atropine), and 40% in group 6 (midazolam). These were not statistically significant. CONCLUSION: FCR is not altered by pretreatment with propofol, atropine, or midazolam, but morphine inhibits cough response and this antitussive effect was not antagonized by naloxone.

7.Usefulness of Lung Perfusion Scan for the Evaluation of Pulmonary Arteries after Total Correction of Right Ventricular Outflow Obstructive Disease.

BACKGROUND: For the evaluation of pulmonary nrtery branches after operation of right ventricular outflow obstructive disease, we reviewed the usefulness of both simple X-ray and lung perfusion scan with 99mTc-macroaggregated albumin compared with cardiac angiography. METHODS: From March, 1986 to July, 1993, at Seoul National University Children;s Hospital, we chose 23 cases who underwent both simple X-ray and cardiac angiography among the patients who had distinct difference between right and left pulmonary blood flow in lung perfusion scan after operation of right ventricular outflow obstructive disease. So we calculated the ratio of right and left pulmonary blood flow and Spearman correlation coefficient. RESULTS: Of the 23 cases, 17 cases were tetralogy of Fallot, 4 cases pulmonary atresia, 1 case double outlet right ventricle, and 1 case truncus arteriosus. The mean age at poeration was 27 months, and the mean postoperative follow-up period was for 68 months. The correlation coefficient between cardiac angiography and simple X-ray was 0.86(p<0.01), and that beteen cardiac angiography and lung perfusion scan was 0.80(p<0.01). CONCLUSION: The cardiac angiography has been known as the best diagnostic tool for the evaluation of pulmonary artery branches after operation of right ventricular outflow obstructive ddisease, but we proposed that the lung perfusion scan should be widely used because its results were similar with those of cardiac angiography.

8.The effect of radial keratotomy (RK) combined with double Ruiz procedure on the corneal curvature.

Under the postulation that the double Ruiz procedure could increase the effect of radial keratotomy (RK), RK combined with horizontal and vertical Ruiz procedure was performed in three groups of 22 rabbit eyes. In the group A of six eyes, RK with vertical Ruiz procedure was performed, and eight weeks later horizontal Ruiz procedure was added. In the group B of eight eyes, RK with horizontal Ruiz procedure was performed, and four weeks later vertical Ruiz procedure was added. In the group C of eight eyes, RK with horizontal and vertical Ruiz procedure was performed simultaneously. As a control, in the group D of eight eyes, RK without Ruiz procedure performed. Changes in keratometry for four weeks and eight weeks postoperatively were analyzed. The results were as follows: 1. At eight weeks postoperatively, mean changes in keratometry were 6.434D, 3.663D, 4.030D and 1.585D in groups A, B, C and D, respectively. 2. Mean changes in keratometry of groups A, B and C were significantly higher than that of group D. 3. Mean changes in keratometry of 6.434D in group A was significantly the highest of the three groups in which double Ruiz procedure was performed. For the above results, it was concluded that double Ruiz procedure increased the effect of RK, and the procedure was more effective when the secondary Ruiz procedure was added eight weeks later.

9.Collateral Ligament Injury of the Fingers

Collateral ligament injury of the IP joint of the fingers are common and almost always occurs at the PIP joint. But the rupture of collateral ligament is often miss-diagnosed as sprain and it's seriousness is often underestimated, of such injuries frequently result in prolonged and even permanent disability of the injured fingers. During the period of Sept. 1986 to Aug. 1988, 16 collateral ligament injuries in 15 patients were treated surgically at Holy Family Hospital, Bu Cheon and following results were obtained: 1. Among 15 patients, 14 were male and the age was 32.1 years on an average. 2. The 7 cases had associated injuries in the same fingers. 3. In 6 cases ligaments were ruptured at the proximal attachment, in 5 at the distal attachment, and in 5 at the ligament substance. 4. Average range of joint motion at the final observation was 69.2°. The average flexion contracture was 9.3°, and the average further flexion was 78.5°. 5. The flexion was limited more than the average when the ligament was ruptured at the insertion(66°) and when there were other associated injuries in the same finger(58.1°). The extension was limited more than the average when the ligament was ruptured at the substance(18°) and when patients were over the age of 31 years(12.5°).

10.Computed tomography of the isodense subdural hematomas

Most subdural hematoma with significantly differed attenuation from that of adjacent brain tissue can beaccurately diagnosed by CT. Difficulty arises when the hematoma is isodense that is exhibited similar attenuationto that of brain. Unilateral isodense subdural hematoma can be identified by indirect sign such as mass effect.Occasionally, the use of intravenous contrast material to aid in identifying isodense subdural hematomas has metwith variable success. Moreover, bilateral isodense subdural hematoma may be more difficult. We therfore considerdit of interest to evalute the diagnostic efficiency of CT in isodense subdural hematomas. We have analysed 13cases surgically provened cases of isodense subdural hematoma examined at Korea General Hospital from Oct. 1981 toApril. 1982. The resuls were as follows; 1. One hundred twenty seven cases of subdural hematomas were studied byCT, 13 cases (19.2%) of which were isodense. 2. The age distribution was from 29 years to 69 years and mean agewas 52 years. The sex ratio was 11 male to 2 female. 3. Seven(53.8%) of 13 cases has a history of head trauma. 4.The time interval which subdural hematoma became isodense was from 1 week to 4 months and peak time interval wasfrom 1 week to 3 weeks. 5. The precontrast CT of isodense subdural hematoma appeared shifting of midlinestructure, compression and deformity of the ventricles in all 13 cases, effacement of cerebral sulci in 10 cases(76.9%) and dilatation of contralateral ventricles in 4 cases (30.8%). 6. The postcontrast CT scan demonstratedenhancement of the medial margin of the lesion in 4(30.8%) of 13 cases and displacement of cortical vein away fromthe inner table of the skull in 3 (23.1%) of 13 cases. 7. Bilateral isodense subdural hematomas were 2 (15.4%) of13 cases.
