1.Factors Related to Suicidal Ldeation in Community-Dwelling Older Adults with Multimorbidity Using Data From the 2017 Korean Community Health Survey

Background:
Multimorbidity and suicide rates are on the rising among older Korean population. Recent studies have shown that multimorbidity is associated with increased suicidal ideation. Thus, this study was to investigate individual factors of suicidal ideation among older Korean adults with multimorbidity.
Methods:
A cross-sectional study was conducted using a subsample (n=20,533) with multimorbidity aged 65 over of the 2017 Korean community health survey. Binary logistic regression model assessed the association of physical activity, sitting times, self-reported sleep duration, and depression with suicidal ideation in older adults with multimorbidity.
Results:
Overall, 2,790 (13.6%) of the sample reported suicidal ideation within the past year. Logistic regression analysis showed that women, low monthly family income, low physical activity, long sitting times, inappropriate sleep duration and depression were significantly associated with increased risk of suicidal ideation in older adults with multimorbidity.
Conclusions
To reduce the rate of suicidal ideation in older adults with multimorbidity, healthcare providers should be aware of the importance on routine assessment for physical activities, sleep pattern, and mental status of older adults in clinical settings. Particularly, female and low-income older adults with multimorbidity should be prioritized for early detection and individualized suicide prevention based on our findings.
2.Trajectories of Cognitive Function and Their Associated Factors in Community-Dwelling Older Adults by Living Arrangement Using the Korean Longitudinal Study of Aging

Background:
The purpose of this study was to describe the distinct cognitive trajectories for 8 years according to living arrangement among community-dwelling older adults using the Korean longitudinal study of ageing.
Methods:
We analyzed data from a final sample of 3,049 community-dwelling older adults aged 65 years or older who participated in at least three cognitive function measures in an 8-year period (2012-2020). Cognitive function was assessed by Korean mini mental state examination. Latent class growth model analysis was applied to determine the number and shape of trajectories of older adults according to living arrangement.Multinomial logistic regression analysis was used to determine risk factors for cognitive function trajectories.
Results:
We identified five trajectory patterns in cognitive function among older adults living alone; ‘high-stable (35.6%)’, ‘high-declining (32.5%)’, ‘moderate-steep declining (6.2%)’, ‘low-stable (18.9%)’, and ‘low declining (6.8%)’. In older adults living with family, there were four trajectory patterns; ‘high-stable (46.4%)’, ‘high-declining (32.0%)’, ‘low-stable (15.9%)’, and ‘low-declining (5.7%)’. Age, instrumental activities of daily living, and depression were common factors of cognitive changes in both older adults living alone and those living with family members. However, monthly incomes was only associated with the risk of cognitive function in older adults living alone.
Conclusions
We found that cognitive function trajectories among older adults may differ depending on the type of living arrangement. Therefore, further studies and health policies are required for avoiding cognitive impairment by living arrangement among older adults based on the results of this study.
3.The effect of casein phosphopeptide amorphous calcium phosphate on the in vitro shear bond strength of orthodontic brackets.

OBJECTIVE: The purpose of this study was to evaluate the effect of casein phosphopeptide amorphous calcium phosphate (CPP-ACP) on the shear bond strength (SBS) of brackets bonded to non-demineralized teeth with either phosphoric acid etching or self-etching primer. METHODS: Sixty human premolars were randomly assigned to 1 of 4 treatment groups (n = 15 each): phosphoric acid etching (group 1); self-etching primer (group 2); CPP-ACP for 2 weeks + phosphoric acid etching (group 3), and CPP-ACP for 2 weeks + self-etching primer (group 4). After bonding of the maxillary premolar metal brackets, specimens were subjected to shear forces in a testing machine. Scanning electron microscopy was used to observe etching patterns on the enamel surfaces of all teeth. A 2-way analysis of variance was used to test for effects of CPP-ACP and etching system on SBS. RESULTS: Significantly higher mean SBSs were observed in groups subjected to phosphoric acid etching (i.e., groups 1 and 3; p < 0.05). On the other hand, SBSs did not appear to be influenced by CPP-ACP (i.e., groups 3 and 4; p > 0.05). We observed a uniform and clear etched pattern on the enamel surface of the phosphoric acid etching groups. CONCLUSIONS: CPP-ACP does not significantly affect the SBS of orthodontic brackets bonded to non-demineralized teeth, regardless of which adhesive method is used to bond the brackets.

4.A Case of Neonatal Lupus Syndrome with Congenital Heart Block.

Neonatal lupus erythematous is a rare syndrome, which is characterized by a transient lupus dermatitis, hematologic abnormalities and isolated congenital heart block. We detected the atrioventricular dissociation, bradycardia and pericardial effusion by fetal echocardiography in a female fetus at 25th weeks of gestational age. The baby was born in the 38th week of pregnancy by Cesarean section with 1790 gram of body weight. In spite of atrioventricular dissociation with bradycardia and pericardial effusion, cardiac pacemaker was not needed during neonatal period because she was doing well, no evidence of congestive heart failure and around 80 beat per minute of heart rate. The serologic markers for diagnosis of neonatal lupus are the autoantibodies specific to SS-A/Ro and/or SS-B/La. These antibodies are produced by the mother and passed to the fetus through the placenta. Autoantibodies of cytoplasmic SS-A antigens or SS-B antigen were found in the blood of this patient and her mother. We report a case of neonatal lupus syndrome with congenital atrioventricular dissociation with Rt. Bundle branch block and hematologic abnormality.

5.Clinical characteristics of patients with acute bronchiolitis who visited 146 Emergency Department in Korea in 2012.

PURPOSE: This study was aimed to investigate the clinical and epidemiological characteristics of the patients with acute bronchiolitis who visited in 146 Emergency Departments (EDs) in Korea in 2012. METHODS: We used the NEDIS (National Emergency Department Information System) database to obtain all children with acute bronchiolitis who visited ED under the age of 3 between January 1, 2012 and December 31, 2012. RESULTS: Totally 18,313 children with acute bronchiolitis were enrolled at this study. The male to female ratio was 1.55:1 and mean age was 10 months. The peak proportion was 17.3% in November during the whole year. The most common chief complaints were fever (38.5%), cough (37.4%), dyspnea (9.5%), gastrointestinal symptom (6.7%), and wheezing (2.5%). The most common accompanying disease was pneumonia (13.3%). The rate of hospital admission and intensive care unit admission were 34.5% and 0.3%, respectively. A predictor for admission via ED was respiratory difficulty. CONCLUSION: These data expand our understanding of clinical characteristics of patients with acute bronchiolitis who visited all ED in Korea in 2012.

7.Effects of Short-Term Supplementation of Carnitine on Plasma and Urinary Carnitine and Plasma Lipid Levels of Healthy Male Adults.

This study was conducted to evaluate changes in plasma concentration and urinary excretion of carnitine, as well as plasma lipid level and fatty acid composition, caused by short term supplementation of carnitine in humans. Ten healthy male subjects (21.2 +/- 0.5 years old) received oral carnitine supplementation (4 g/day) as tablets for two weeks. Fasting blood and random urine samples were collected from each subject both prior to and at the end of carnitine supplemention program. Following the 2 weeks of carnitine supplementation, plasma total carnitine (TCNE) concentration increased 20% (85.1 +/- 7.4 vs 67.3 +/- 9.1 micro mol/1, p>0.05), while urinary excretion of total carnitine increased ten times compared to the value measured prior to the supplementation (3051 +/- 692 vs 278 +/- 90.1 micro mol/g creatinine, p<0.01). Non-esterified carnitine (NEC) comprised from 71 to 88% of TCNE in plasma, and from 32 to 40% of TCNE excreted in the urine. Two weeks of carnitine supplementation in healthy adults significantly elevated plasma level of acid soluble acylcarnitine (ASAC) which is esterified mostly with short chain fatty acids (21.6 +/- 1.6 micro mol/l) compared to the value measured prior to the supplementation (6.4 +/- 0.8 micro mol/l) (p<0.05). Carnitine supplementation significantly increased plasma HDL-cholesterol level (p<0.05), and decreased the atherogenic index (p<0.05), but failed to cause any significant change in plasma levels of total cholesterol, triglyceride, and free fatty acids. Plasma triglyceride and phospholipid fatty acid compositions were not significaly affected as well by the oral supplementation of carnitine in subjects with normal range of blood lipid levels.

8.Genetic Relatedness of Korean Isolates of Pythium Using Restriction Fragment Length Polymorphism of PCR Amplified Ribosomal DNA and M-13 Marker.

Twenty-five isolates of Pythium species were identi6ed and classified on the basis of RFLP(restriction fragment length polymorphism) of ITS(internal transcribed spacer) region in ribosomal DNA. and M-13 PCR markers. Eight restriction endonucleases were used to investigate the genetic relatedness among isolates P. graminicola and P. arrhenomanes as well as P. aphanidermatum and P. delience produced identical band patterns with all restriction endonucleases used and M-13 markers. P. myriotylum and P. catenulatum also formed tight cluster on the basis of RFLP of ribosomal DNA but produced distinct band pattern with M-13 PCR markers. No intraspecitic variations were observed with RFLP of ITS region in ribosomal DNA. Molecular analysis based on M-13 marker demonstrated that each species produced distinct band patterns. Intraspecific variation of P. ultimum and P. torulosum was observed with M-13 markers.

9.A Case of Mycobacterium Marinum Tenosynovitis Diagnosed by the PCR-restriction Fragment Length Polymorphism of the rpoB Gene.

Mycobacterium marinum is an uncommon cause of skin and soft-tissue infection. The diagnosis of M. marinum infection is often delayed when only a conventional tissue culture method is used. PCR-restriction fragment length polymorphism (RFLP) analysis using the novel region of the rpoB gene is now available for the rapid identification of Mycobacteria. We report a case of hand infection caused by M. marinum that was identified by PCR-RFLP analysis. The PCR-RFLP assay is a specific and rapid method for the identification of Mycobacteria that facilitates the early diagnosis of non-tuberculous Mycobacterium infection.
