1.Finger Prints in Schizophrenia.

Finger print patterns were studied in a series of 99 male schizophrenics who met with the diagnostic criterias of International Pilot Study of schizophrenia except items associated with chronicity in exclusion criteria, As controls, the autbors used 400 Korean men who were free of congenital, dermatological, neurological and psychiatric disorders, The frequency distribution of the finger print patterns in the 99 s'chizophrenics were whorls 52.45%, loops 49.09% and arches 3.46%. and that of controls were vrhorls 49.65%, loops 46.38% and arches 3.97%, The diffeence in pattern frequency was statistically significant (P<0.05). However, the striking differences were found in the frequency of the ridge dissociation. The observed frequency was very significantly high (p<0.005) in the schizophrenics (16.16%) compared to controls (6.25%). This findings supported the rcport of the Raphael (18%) but the frequency of our controls(6.25%) were not matched with previous study of Abel (The frequency for his 4,000 control was negative. No plausible explanation is not found for the extraordinary high frequency of ridge clissociat,ions in the controls of tbis study. Although rnore extensive investigation in needed for the explanation of significantly high frequency of ridge dissociation in thes chizophrenics, the findings of the present study give us another evidence of the constitutional factors in schizophrenics.

3.A Case of Scrotal Cancer.

We presented a case of non-occupational scrotal cancer in 66-year-old male. He was a farmer and did not contact with any known carcinogen on past history. He had several erythematous walnut-sized nodules on scrotum and diffuse oozing erythema on penoscrotal and hoth crural areas. And his sclera was icteric and liver was palpable to the 3-flnger-breadth from the costal margin and had hard consistency and nodular surface. Spleen was palpable to the 4-finger-breadth. On laboratorary exarninations marked liver furtction impairment was noted. On histologic examinations of biopsied scrotal skin and ingoiaal lymyhnode undifferentiated squamous cell carcinoma was noted. On the 5th day of admission he became dull and irritable and tbe condition was deteriorated from day to day. At that time blood ammonia level was elevated to 140.63mcg% He was discharged in comatose state without any treatrnent for the scrotal cancer itself and died 4 days later.

4.A Case of Isolated Left Ventricular Diverticulum in an Adult.

Congenital diverticulum of the left ventricle is a very rare disease. Ventricular diverticulum is usually associated with other anomalies including intracardiac and midline thoracoabdominal defect. We describe a case with congenital left ventricular diverticulum presenting as an isolated lesion. A 37-year-old man presented with 4-year history of chest pain. Diverticulum was diagnosed by echocardiography and left ventriculogram.

5.Characterization of Heparin:PF4 Isoantibody Interaction to Platelets in Heparin-Induced Thrombocytopenia/ Thrombosis.

BACKGROUND: Heparin-induced thrombocytopenia/ thrombosis (HITT) is recognized as the most frequent and fatal symptom complexes in patients receiving heparin therapy. The antibodies of HITT are not directly bound to heparin but bound to complexes of heparin and platelet factor 4 (PF4) derived from platelet alpha-granules. That is, HITT IgG antibody-heparin-PF4 immune complexes are bound to FcgammaRII receptor of platelets, which induced thrombocytopenia. Some researches showed the antibodies reactive to platelets could be IgM or IgA as well as IgG. So in this study, the authors tried to explain the molecular basis of heparin-PF4-isoantibody complexes . METHODS: In HITT patients who had received long-term heparin therapy, we determined HITT isoantibodies and titers using heparin:PF4 ELISA. When fifteen HITT patients with high titer antibodies (more than 1 : 100) were selected, reaction patterns of isoantibodies with the platelets were examined through serotonin release test and flow cytometry. RESULTS: All patients showed one or more isotype antibodies and the most frequent isotype was IgGl (nine patients) . In the presence of optimal concentra pion of heparin and PF4, ten patients had antibodies which activated platelets, and all of them were positive in serotonin release test. Reactive plasmas had IgGl, IgG3, IgA or IgM antibodies, and each of them except one had IgGl. These platelet activations could be blocked in vitro by anti-IV.3 antibody. Non-reactive plasmas were negative In serotonin release assay nor had TgGl. The plasmas 4hat had two or more isoantibodies showed a similar pattern of the IgG antibody by flow cytometry. CONCLUSIONS: The HITT antibodies can be all kinds of antibody isotopes, but IgA and IgM may not bind to the platelets directly. It seems to be possible only after reacting with heparin-PF4-IgG complexes.

6.One Case of Erythromelalgia.

Erythromelalgia is a rare disease characterized by burning pain of the extremities associated with erythema and increased temperature of the skin. A case of erythromelalgia was reported with review of literatures. It seemed that this is the first reported case in Korean literatures. patient was 12 years old male having erythematous swelling associated with burning pain on his feet and lower legs for five years. Immersion of his feet in warm water quickly, increased the pain but cold water gave prompt relief that he soaked his feet for many hours during the day and night. No specific abnormalities were noticed in Iaboratory studies, Treatment with various topical, systemic medication revealed of unfavorable result.

7.Collateral Ligament Injury of the Fingers

Collateral ligament injury of the IP joint of the fingers are common and almost always occurs at the PIP joint. But the rupture of collateral ligament is often miss-diagnosed as sprain and it's seriousness is often underestimated, of such injuries frequently result in prolonged and even permanent disability of the injured fingers. During the period of Sept. 1986 to Aug. 1988, 16 collateral ligament injuries in 15 patients were treated surgically at Holy Family Hospital, Bu Cheon and following results were obtained: 1. Among 15 patients, 14 were male and the age was 32.1 years on an average. 2. The 7 cases had associated injuries in the same fingers. 3. In 6 cases ligaments were ruptured at the proximal attachment, in 5 at the distal attachment, and in 5 at the ligament substance. 4. Average range of joint motion at the final observation was 69.2°. The average flexion contracture was 9.3°, and the average further flexion was 78.5°. 5. The flexion was limited more than the average when the ligament was ruptured at the insertion(66°) and when there were other associated injuries in the same finger(58.1°). The extension was limited more than the average when the ligament was ruptured at the substance(18°) and when patients were over the age of 31 years(12.5°).

9.Minimal Change Nephrotic Syndrome Presented with Acute Renal Failure in a Child.

Hepatoblastoma (HB) is a rare embryonic malignant tumor of the liver. Most morphological studies on HB have limited to the histological characteristics and only 3 cases of HB have been described in the cytology literature. We present 2 cases of HB occurring in children aged 1 year and 3 years, respectively. The distinctive cytologic features of fine needle aspiration of HB were clusters of tumor cells showing acinar and trabecular pattern, smaller tumor cells with a high nuclear-cytopalsmic ratio and hyperchromatic nuclei having prominent nucleoli, and the presence of extramedullary hematopoiesis and osteoid material. These features were also found in the cell block and the biopsy specimen, and appeared very useful in the differentiation of HB from hepatocellular carcinoma.
