2.Growth of Human Melanocytes in Human Epidermis Reconstructed by Culture.

BACKGROUND: Melanocytes grown in pure monolayer culure lack many of the cellular interactions that exist in vivo. This can be partially overcome by growing melanocytes together with other epidermal cells in skin equivalent models. OBJECTIVE: The objective of the present study was to grow human melanocytes in human epidermis reconstructed on dermal substrates in vitro and to examine their response to UV radiation. METHODS: The skin equivalents were prepared by seeding cultured human keratinocytes together with cultured human melanocytes(in a ratio of 5%) onto de-epidermized dermis. After 7 days of culture, they were exposed to UVB irradiation(total 150m J/cm over 5days). On day 12 of air exposure the sections of the skin equivalents were prepared for histology. The structure of the skin equivalents was studied following staining with hematoxylin and eosin. Melanocytes were characterized by DOPA staining and by immunohistochemistry. RESULTS: Melanocytes were localized singly within the basal layer of the reconstructs. Melanin was also visible both in the melanocytes and in neighboring keratinocytes. There was an increase in melanocyte size and dendricity following UV irradiation. Melanocytes became positive to staining with HMB-45 antibody following UV irradiation. CONCLUSION: Our results indicate that melanocytes grown in reconstructed human epidermis are functional and capable of responding to UV irradiation.

3.Filtering Surgery in Aphakia and Pseudophakia.

Aphakic or pseudophakic glau-oma is known to be at high risk for failure after filtration surgery. The authors retrospectively investigated the results of trabeculectomy in 14 eyes of 14 patients with glaucoma, seven aphakic and seven pseudophakic eyes, which were followed from six months to three years with an average ot 16.5 months. A conventional trabeculectomy without using antifibrotic agents was performed in all the eyes. The site of conjunctival incision was prepared on the virgin area of the conjunctiva; 8 superonasal, 4 inferonasal and 2 inferotemporal approaches. An average postoperative intraocular pressure reduction was 13.4 mmHg. The number of medications decreased in 11 eyes. A spontaneously developed functioning bleb was found in seven eyes. In other four eyes, the filtering bleb was formed with an aid of massage or needling. The remallllllg three eyes did not show a functioning bleb. Postoperative complications included hyphema in 3 eyes, overfiltration in 2 eyes, choroidal detachment in 1 eye, corneal edema in 1 eye and endophthalmitis in 1 eye.

Angiolymphoid hyperplasia with eosinophilia(ALHE) is a benign angiomatous neoplasm which usually arises from skin, blood vessel, soft tissue, heart and rarely from bone. The authors experienced a case of ALHE which involved the distal femur of 35-years old male and treated by marginal excision and autogenous cancellous bone graft with plate fixation.

5.The Study on the Menisci of Korean Adults

The menisci or semilunar cartilages of the knee provide the function of mechanical lubrication and they are particularly important in allowing rotational movements of the knee joint. The study was carried out on 123 Korean adult knees for measurement of the menisci and observation of surrounding ligamentous structures through anatomical dissection of formalin embalmed cadavers. Detection of abnormal meniscal lesions was also included in this study. The following results were obtained: 1. A-P diameter, transverse diameter, maximal width and thickness were measured in 123 knees of 62 cadavers. 2. Among 62 cadavers,' the anterior menisco-femoral ligament was present in 11 subjects (17.7%), the posterior menisco-femoral ligament in 59 (95.2%), and the transverse ligament in 36 (58.0%). 3. Discoid lateral meniscus was found in 5 knees (4.1%), and tear was associated in 3 of them. 4. Meniscus tear was found in 13 knees (10.6%), and the site of tear was medial in 9 knees and lateral in 4 including 3 torn discoid menisci. The oblique pattern of tear was most common.

7.Clinical and Pathological Study After Ligation of Common Carotid Artery in the Rat.

This study examined the late pathological finding, clinical manifestation, and mortality in thirty-two rats with cerebral infarction induced by common carotied artery ligation. Thirty-two rats were divided into three group : Group A-a unilateral common carotid artery ligation group of twelve animals, Group B-a total common carotid artery ligation one side and partial common carotid artery ligation another side of eighteen animals, and Group C-bilateral common carotid arteries ligation of two animal. All animals surviving for 3 weeks were sacrified by intracardial injection of 2 M KCI 5cc and 2.5cc of 2% T.T.C. solution was infused into each common carotid arteries. The brain was rapidly rmoved and sectioned coronally and examined with the aid of a stereoscopic microscope for degree of T.T.C. stain and infarction. Brain was fixed and examined histologically. The results were as follows : 1) Mortality rate of each group : a unilateral common carotid artery ligation group was 16.67%, a total. Common carotid artery ligation on side and partial common carotid artery ligation another side group was 77.78%, and bilateral common arteries ligation group was 100%. 2) Clinical manifestation was showed as follows : hemiparesis, circling movement, abnormal seizure like activity, and ptosis. 3) Percent hemispheric volume infarction(%HVI) was 40%(mean value) in unilateral total common carotid artery ligation and 60% in total ligation one side and 46.25% in partial ligation another side. 4) The pathological change was signigicantly visible in bilateral ligation group.

8.A traumatic Giant Intracavernous Aneurysm Associated with Fatal Epistaxis: A Case Report.

The autors reported a giant aneurysm in the cavernous portion of the internal carotid artery which occurred after head trauma. The patient was a 24 year old man who had suffered massive epistaxis with hyoptension about 25 days after a traffic accident. On plain skull X-ray, there was evidence of a basal skull fracture and a linear fracture at the right frontotemporal area. The computed tomographic scan revealed round, well enhanced density in the sellar region. During a reattack of massive epistaxis, emergency carotid angiography showed a right cavernous-internal carotid artery aneurysm with a small extravasation of dye into the sphenoid sinus.

9.Relationship between Blood Pressure and Insulin Level or Red Cell Membrane Na+ Transport in Acromegaly

To test the hypothesis that hyperinsulinemia and/or abnormalities of red cell membrane Na^+ transport are concerned in the pathogenesis of hypertension, we investigated the relationship between blood pressure, insulin level and red cell membrane Na^+ transport in patients with acromegaly which is frequently associated with hypertension, hyperinsulinemia and abnormalities of red cell membrane Na^+ transport.The results were as follows;1) BMI and both systolic and diastolic blood pressure were significantly higher in patients with acromegaly than in control subjects.2) Fasting glucose and insulin levels were higher, and both serum glucose and insulin responses after a 75g glucose load were significantly increased in patients with acromegaly as compared with control subjects.3) Vmax of Na^+-Li^+ countertransport were significantly higher in patients with acromegaly than in control subjects while red cell Na^+ concentration and Vmax of Na^+, K^+ ATPase were similar in the two groups.4) In multiple stepwise regression analysis, age was directly correlated with both systolic and diastolic blood pressure in acromegaly. On the other hand both insulin level and red cell membrane Na^+ transport showed little correlation with either systolic or diastolic pressure. 5) Prevalence of hypertension in acromegaly was 39%(9 out of 23) and only age except for blood pressure was significantly higher in hypertensive acromegalic patients than in normotensive acromegalic patients while GH level, insulin levels and red cell membrane Na^+ transport were similar in the two groupsThese results suggest that hyperinsulinemia or abnormalities of red cell membrane Na^+ transport are not causally related to hypertension in patients with acromegaly.

10.A Study on the Urinary Iodine Excretion in Normal subjects and Patients with Thyroid disease

An adequate supply of dietary iodine is essential for the synthesis of the thyroid hormons. The measurement of dietary iodine intake is important for the clinical assessment of thyroid disease, especially in areas where iodine intake is excessive or deficient.To evaluate dietary iodine intake in Korean and its effects on thyroid function, we measured urinary iodine excretion with morning urine by electrode method in 184 normal subjects, 96 postpartum women and 181 patients with thyroid disease from October 1994 to February 1995. The results were as follows;1) In normal control, the mean value of urinary iodine excretion was 3.8+-2.7mg/L (range 0.1-15.0mg/L). However, there was no sex and age differences in the urinary iodine excretion.2) In postpartum women, the urinary iodine excretion was 9.0+-10.8mg/L who were not taken high iodine diet(Miyok-Guk), the mean value was statistically higher than normal control(p<0.01) and significant increased the urinary iodine excretion after eating of high iodine diet(p<0.01).3) In volunteer, there were increase of urinary iodine excretion more than 10 folds after high iodine diet and medication.4) The urinary iodine excretion in patients with thyroid diseases was not different from normal control, and there were no significant differences of urinary iodine excretion among the patient groups. The urinary iodine excretion in the acute stage of patients with subactue thyroiditis or painless thyroiditis was significantly increased compared to the recovery stage. However, it was not significantly different from that of normal control.In conclusion, urinary iodine excretion in Korean population is very high comparing to the reported data in Western population but similar with Japanese. The urinary iodine excretion is significant increase( more than 10-folds of basal level) after high iodine diet or high iodine containing medication in postpartum women or healthy persons. As a clue of destruction induced thyrotoxicosis, the urinary iodine excretion measurement is not valid in area where iodine intake is excessive like Korea.
