6.A case of tuberculosis of the vagina.

Tuberculosis of the vagina is very rare disease reported to be involved in less than 1% of female genital tuberculosis. A 53-year-old woman presented with abnormal cervicographic finding at the private clinic. On vaginal physical examination, there were multiple elevated nodular lesion in the vagina and ectocervical area. There was no abnormal vessels or acetowhite epithelium on colposcopic examination. Direct biopsy for histologic examination revealed granulomatous lesion. We performed PCR of mycobacterial DNA and Ziehl-Neelsen staining for acid-fast bacilli (AFB). The Ziehl-Neelsen staining for AFB was negative, but mycobacterial DNA fragments were detected using the PCR method. Diagnosis was confirmed by the detection of acid-fast bacilli in histologic preparations. Anti-tuberculosis medication was effective for this patient.

7.Pseudo-Meigs' syndrome associateed with pedunculated subserosal myoma and elevated CA-125 level.

Pseudo-Meigs' syndrome consists of pleural effusion, ascites, and benign tumors of the ovary other than fibromas. These benign tumors include those of the fallopian tube or uterus and mature teratomas, struma ovarii, and ovarian leiomyomas. Although uterine leiomyomas are frequent gynecological finding, they are rarely cause pseudo-Meigs' syndrome with elevated serum CA 125 levels. A case of pseudo-Meigs' syndrome associated with subserosal leiomyoma is presented with a brief review of literatures.

8.Modified Iliofemoral Approach using ASIS Osteotomy in the Pelvis

Operations on the inner aspect of the pelvis are often necessary for irreducible fracture of the ilium, irreducible fracture of superior and medial portion of the acetabulum, tumors and infections involving iliac fossa, and fracture-dislocation and infection of the sacro-iliac joint. Until now Letournel and Judet's ilioinguinal and iliofemoral approaches provided access to anterior column of the pelvis by elevating muscle layer on inner aspect of the pelvis. We used modified iliofemoral approach using ASIS(anterior superior iliac spine) osteotomy to gain better access to anterior column, the pelvlc brim, and in some occasions as far as anterior sacroiliac joint while avoiding injury to the structures in the inguinal ligament and lateral femoral cutaneous nerve and reported six cases treated with this approach. In conclusion, a modified iliofemoral approach using ASIS osteotomy is a useful approach which can obtain an excellent exposure in the inner pelvis.

10.Papillary Adenocarcinoma of Nonpigmented Ciliary Epithelium of the Eye.

Adenocarcinoma of the ciliary epithelium is a rare tumor, usually occuring in elderly patients who have a history of severe ocular trauma or chronic inflammation. We report an adenocarcinoma of the nonpigmented ciliary epithelium found within the phthisical globe of a 36-year-old female whose eye had been loss of vision since infancy. The mass, measured 4.0 x 4.0 cm, was relatively limited by sclera but had invasion to posterior portion. Histologically, the tumor was a compact mass which consisted of tubular and papillary structures with foci of the pleomorphic area. Strands of cells and individual cells were invested with thick basement membrane that have positivity for periodic acid-Schiff stain. Immunohistochemical staining showed strong reactivity for cytokeratin and epithelial membrane antigen, and focal for neuron-specific enolase and S-100 protein.
