1.Polymorphism in the IL-1 Receptor Antagonist Gene in Vitiligo.

BACKGROUND: The severity of several chronic inflammatory diseases was reported to be associated with polymorphism of the IL-1 receptor antagonist gene(IL-lrn). OBJECTIVE: This study was performed to study the polymorphism of the IL-1rn in vitiligo and in the normal Korean population. METHODS: Thirty one cases of vitiligo and seventy nine normal Koreans as control were studied for the polymorphism of IL-1 rn. RESULTS: The frequency of allele 2 of the IL- I rn in 31 patients with vitiligo was compared with that of the 79 healthy controls. The frequency of allele 2 was 1.6% in vitiligo patients and 3.8% in the normal controls. CONCLUSION: There was no significant difference in the frequency of allele 2 between the vitiligo patients and normal controls.

2.Effects of Calcipotriol ( MC 903 ) on Proliferation and Differentiation of Cultured Human Epidermal Keratinocytes.

BACKGROUND: Calcipotriol(MC903), a new vitamin D(3) analogue, has been reported to be effective in the treatment of patients with psoriasis. OBJECTIVE: The purpose of this study is to examine the effects of calcipotriol on proliferation and differentiation of the keratinocytes in monolayer cultures and three-dimensional cultures. METHODS: Using moriolayer cultures, we examined morphological changes of keratinocytes and performed [(3)H]thymidine incorporation after calcipotriol was added into the medium. Using three dimensional cultures, we performed two experiments: one with cultures treated with calcipotriol immediately after the keratinocytes had been exposed to the air and another set of cultures treated with calcipotriol after three dimensional morphogenesis of the keratinocytes. We examined morphological changes of keraitinocytes and performed a immunohistochemical study for proliferation differentiation markers RESULTS: In monolayer cultures, at calcipotriol concentrations of 10(-9)M-10(-6)M, keratinocytes became larger, more irregular, and flattened in a dose-dependent manner. At 10(-9)M-10(-6)M, [3Hl thymidine incorporatiorn was decreased dose-dependently as compared to the control culture. In the first experiment using three-dimensional cultures, at 10(-9)M-10(-6)M, total epidermal layers were thinned. This was associated with thinnings of nucleated and horny layers in a dose dependent manner. In the seconcd experiment using three-dimensional cultures, at 10(-8)M-10(-6)M, nucleated layers were thinned in a dose dependent manner, but the horny layer was slightly thickened, as compared to the control culture. Immunohistochemical studies showed a reduction of differentiation markers such as keratin 1, involucrin, filaggrin, loricrin consistent with a thinning of nucleated layers in the epidermal architecture in both experiments. In the basal layer, at 10(-9)M-10(-6), PCNA-positive cells were and BrdU-positive cells were decreased dose-dependently as compared to the control culture. CONCLUSIONS: In this study, we demonstrated that at 10(-9)M-10(-6) calcipotriol inhibited keratinocytes proliferation and stimulated keratinocytes differentiation in a dose-dependent manner.

3.Chronological Changes of Surgical Strategy for Early Gastric Cancer: Our 10 Years' Experience.

PURPOSE: To analysis the chronological changes of surgical strategy for early gastric cancer (EGC) in our hospital over a 10-year period. METHODS: From November 1993 to August 2003, we experienced 201 patients with EGC at Ewha Womans University Mokdong Hospital. The medical and pathologic records of these patients were reviewed retrospectively. The patients were categorized into three groups in chronological order: P1 (1993~1996), P2 (1997~2000), and P3 (2001~2003). The clinicopathological characteristics and treatment modalities among these three groups were compared. RESULTS: Of the 683 gastric cancer patients, 201 patients (29.7%) were pathologically confirmed to have EGC. The proportion of EGC gradually increased in chronological order: 19.2% in P1, 33.2% in P2 and 32.9% in P3. There were no significant differences in clinicopathological characteristics among three groups, including age, sex, location, size, gross type, histology, depth of invasion, and lymph node metastasis. Open surgery was performed in 134 patients, laparoscopic surgery in 52 and endoscopic mucosal resection (EMR) in 15. The proportion of minimally invasive surgery such as EMR, laparoscopic wedge resection, laparoscopy- assisted distal gastrectomy (LADG), and hand-assisted laparoscopic surgery (HALS) increased in chronological order: 0% in P1, 22.4% in P2 and 58.5% in P3. The postoperative mortality was 1.49% (3/201): 2 cases after open surgery, and 1 case after laparoscopic wedge resection. One case after open conventional surgery recurred. There were no significant difference in postoperative complication, postoperative mortality and survival rate among the three groups. Overall 5-year survival rate was 95.4%. CONCLUSION: The proportion of minimally invasive surgery increased during a 10-year period in our hospital. However, there were no significant differences in postoperative complication, postoperative mortality, recurrence rate, and survival rate among three chronological groups.

4.Bone marrow-derived stem cells contribute to regeneration of the endometrium.

Stem cells are undifferentiated cells capable of self-renewal and differentiation into various cell lineages. Stem cells are responsible for the development of organs and regeneration of damaged tissues. The highly regenerative nature of the human endometrium during reproductive age suggests that stem cells play a critical role in endometrial physiology. Bone marrow-derived cells migrate to the uterus and participate in the healing and restoration of functionally or structurally damaged endometrium. This review summarizes recent research into the potential therapeutic effects of bone marrow-derived stem cells in conditions involving endometrial impairment.

5.Senior Baccalaureate Nursing Students’ Knowledge and Visual Differentiation Ability for the Pressure Injury Classification System and Incontinence-associated Dermatitis

Purpose:
This study aimed to evaluate senior baccalaureate nursing students’ knowledge and visual differentiation ability for the pressure injury classification system (PICS) and incontinence-associated dermatitis (IAD).
Methods:
A cross-sectional survey was conducted using a paper-based questionnaire with 120 senior baccalaureate nursing students in South Korea. Data were collected using a knowledge test (KT) and a visual differentiation tool (VDT) based on 21 photographs with clinical information related to PICS and IAD.
Results:
Students had relatively high KT scores (62.6%), and low VDT scores (35.6%). Critical issues in this study were nursing students’ lack of an adequate visual differentiation ability to accurately assess the stage of pressure injury and IAD and their higher level of confusion in identifying unstageable, deep tissue injuries, and IAD compared to stage 1-4 injuries. Satisfaction with education (r=.20, p<.05) and exposure to various teaching methods (r=.21, p<.05) exhibited significant correlations with higher VDT scores.
Conclusion
The findings of this study suggest that increasing students’ satisfaction with their education and applying various teaching methods can contribute to the improvement of visual differentiation ability in PICS.
6.Nasal Deformity Due to Tuberculous Chondritis.

Tuberculosis (TB) is a common disease worldwide. However, nasal TB is quite rare, and the diagnosis of nasal TB requires a high index of suspicion. The most common symptoms of this unusual presentation are nasal obstruction and nasal discharge. We present a case of nasal TB with involvement of the hard palate presenting with a chronically progressive nasal deformity and ulceration of the hard palate. A biopsy confirmed the diagnosis, and medication for TB was started and the lesions resolved. When a patient presents with chronic ulcerative lesions that do not respond to antibiotic treatment, TB should be included in the differential diagnosis. Biopsy of the lesion can aid in the confirmation of the diagnosis.

7.Studies on the anti-parasitic efficacy and safety of ivermectin and pyrantel pamoate compound against Dirofilaria immitis in dogs.

Dirofilaria (D.) immitis is an important canine parasitic nematode in dogs. D. immitis parasitizes the right ventricle and pulmonary artery of dogs. An ivermectin and pyrantel pamoate compound (IPPC) was administered to dogs naturally infected with this parasite. IPPC is composed of 68.0, 136.0 and 272.0 microg of ivermectin and 57.0, 114.0 and 227.0 mg pyrantel pamoate for small, middle, and large animals. Ivermectin has activity against nematodes and ectoparasites in dogs. Pyrantel pamoate is also effective against nematodes in dogs. Our results showed that this drug combination has good efficacy in D. immitis infected dogs.

Toxocara (T.) canis and Trichuris (T.) vulpis are very important canine parasitic nematodes. T. canis parasitize in small intestine and T. vulpis parasitize in large intestine. In order to control of these nematodes, ivermectin and pyrantel pamoate compound was applied to the dogs infected with these parasites naturally and artificially. This drug was composed of 68.0 microg of ivermectin and 57.0 mg of pyrantel pamoate for small animal, 136.0 microg of ivermectin and 114.0 mg of pyrantel pamoate for middle animal, and 272.0 microg of ivermectin and 227.0 mg of pyrantel pamoate for large animal. Ivermectin in this drug is activity to nematodes and ectoparisites. Pyrantel pamoate in this drug is also activity to nematodes. In this experiment, this drug had a good efficacy against T. canis and T. vulpis in the infected dogs.

9.Changes of Biochemical Bone Markers and Bone Mineral Density after Hormone Replacement Therapy in Korean Women.

BACKGROUND: Biochemical bone markers have been suggested to reflect postmenopausal high bone turnover. These markers could be useful in following response to hormone replacement therapy (HRT). But we have few studies about the sequential changes of biochemical bone markers and bone mass after HRT in Korean women, and it is unclear whether women with early menopause have different response to HRT from women with normal menopause. The aims of the present study were to see the sequential changes of biochemical bone markers and bone mass after HRT in Korean women, to examine whether a single baseline biochemical bone marker or a change in biochemical bone marker could predict subsequent bone mass, and to determine the difference of response to HRT between women with early menopause and women with normal menopause. METHODS: Postmenopausal women (n=21) were divided with into three groups according to their age at menopause (AAM): the first group with AAM < or = 43 years (early menopause group, n=7), the second group with 43 years < or = AAM < or = 50 years (n=4), and the third group with AAM > or = 50 years (normal menopause group, n=10). For the HRT, conjugated estrogen (0.625mg per day) and continuous or cyclic medroxyprogesterone (2.5-10mg per day) were administered. Bone mineral density (BMD) was measured at baseline and 12 months and biochemical bone markers were measured at baseline and 3, 6, and 12 months during HRT. RESULTS: Deoxypyridinoline, type 1 collagen N-telopeptide, bone alkaline phosphatase, and osteocalcin were significantly decreased at 3 months, and mean percent changes from baseline of bone resorption markers were larger than those of bone formation markers. At 12 months, BMD was significantly increased at lumbar spine and Ward's triangle. But BMD was not significantly increased at femur neck and femur trochanter. Two baseline bone markers (bone alkaline phosphatase and type 1 collagen N-telopeptide) correlated with changes of BMD but any changes of bone markers at 3, 6 months didn't correlate with changes of BMD. In early menopause group, changes of bone markers and BMD were larger than those in normal menopause group, but the difference between the two groups was not significant. CONCLUSION: All four bone markers showed significant reduction at 3 months, but bone resorption markers were decreased more markedly and rapidly, and some baseline bone markers can predict the change of BMD after HRT. The difference of response to HRT between early menopause group and normal menopause group was not significant.

10.Factors Influencing the Time to Full Enteral Feeding in Very Low Birth Weight Infants.

PURPOSE: We investigated the factors influencing the time to reach full enteral feeding in very low birth weight infants. We compared the days of life to start tube or oral feeding, to regain birth weight, and to establish full enteral feeding in very low birth weight infants and determined the risk factors delaying full enteral feeding. METHODS: We reviewed the medical records of 214 newborn infants less than 1,500 g at birth, who were delivered at Chonnam National University Hospital from January 1, 1997 to May 31, 2000. These infants were divided into three groups by birth weight (less than 1,000 g, 1,000-1,249 g, and 1,250-1,499 g). Gestational age, birth weight, the days of life to start first tube or oral feeding, to regain bith weight and to reach full enteral feeding, and the possible risk factors delaying the progress of enteral feeding were analyzed. RESULTS: The gestational age was 29.69+/-2.33 weeks and the birth weight was 1,212+/-194.3 g. The time to start tube or oral feeding, to regain birth weight, and to establish full enteral feeding were significantly later and longer in the lower birth weight group. The time to reach full enteral feeding was related to the duration of mechanical ventilation and oxygen therapy, and was significantly longer in the infants with severe respiratory distress syndrome (RDS), chronic lung disease (CLD), apnea, intraventricular hemorrhage (IVH), cystic periventricular leukomalacia, sepsis/disseminated intravascular coagulopathy, ricketts, and total parenteral nutrition. CONCLUSION: The time to reach full enteral feeding was inversely related to the birth weight, and the risk factors for delayed full enteral feeding were the presence and/or the severity of diseases related to the prematurity such as the duration of mechanical ventilation and oxygen therapy, RDS, CLD, apnea, IVH, and sepsis.
